Perry’s Steakhouse & Grille, 11788 West Broad Street – The following violations were reported during a routine inspection: prep area hand sink doesn’t have paper towels; oyster tags are missing from containers in walk-in cooler; oyster tags are not marked with the date of service nor are they are in chronological order; raw chicken stored over beef and raw beef stored over cooked sausage; temperatures of turtle soup and spinach artichoke dip are elevated because of improper cooling methods; temperatures of crab in cups, pork cubes, shrimp, carmelized onions and potato wedges in bags are elevated; no date marks found on cooked salmon or potatoes; time is not being tracked for peppercorn sauce; spray chemical bottles and buckets of cleaner are unlabeled; facility is storing meat in reduced oxygen packaging without proper paperwork/HACCP plan; temperatures of lobster and potatoes are elevated because of improper cooling methods; water bottle is unlabeled; glass rim products at the bar are uncovered; no chemical residue found in wiping cloth bucket; bar utensils stored in standing water; ice receptacle, grill brush and single-service boxes found on the floor; to-go cup dispenser is unprotected; martini glasses are stored on linen; mops are not hung to air dry.

Chanello’s Pizza, 5239 South Laburnum Avenue – The following violations were reported during a routine inspection: employees haven’t been properly trained in food safety, proper hand washing, cleaning and sanitizing equipment, food and nonfood contact surfaces, and preventing cross contamination, as it relates to their assigned duties; person in charge has no knowledge of certified food protection managers and couldn’t provide proof of certification; a food employee failed to wash hands after handling personal phone and money and smoking outside before engaging in food prep; a hand sink isn’t provided near dishwashing area; observed dried food debris inside cheese scoop; badly broken containers are being used to store sauce and items in the prep unit; several pizza pans have jagged edges and are broken; door to the walk-in cooler is broken and doesn’t close tightly; lids on the prep unit are damaged; observed heavy buildup of dried food debris, flour and grime on surfaces of all wire racks, containers, pans, sauce bottles, cables, monitors, printers, equipment stored on shelves, fan system in walk-in cooler and inside prep unit; wall coving is missing or damaged in kitchen; floor around mixer is damaged; paint is chipping off the wall in several areas; entry door is badly damaged; observed litter and items not necessary to operation stored in back room; there is heavy buildup of food debris and grime on floors, walls and ceiling panels throughout kitchen and walk-in cooler; shields are missing from ceiling light fixtures.

Classic’s, 4820 Williamsburg Road – The following violations were reported during a routine inspection: sliced deli meat in walk-in cooler is covered before reaching required temperature; prep and single cutting boards are heavily scratched and stained; several plastic containers and lids are broken; observed food debris on surfaces of storage shelves on front line and fan systems in walk-in cooler; sauce bottles are stained; floor mat and floor in walk-in cooler is dirty; cement floor in walk-in cooler and ceiling panels on front service/make line are no longer smooth and easily cleanable.

Chianti Italian Restaurant & Pizzeria, 1304 Gaskins Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager (repeat violation); facility couldn’t provide verification that workers have been trained on employee health; facility doesn’t have written procedures for cleaning up vomit/diarrhea; can opener is soiled; there isn’t enough sanitizer in the three-compartment sink; slicer and cutting boards on the line are dirty; temperature of blue cheese on line is elevated; no date marks found on sausage links, lasagna, pasta, cooked peppers, pasta salad or fresh mozzarella; calamari is thawing on the counter; sauce bucket and box of cheese are stored directly on walk-in cooler floor; shelf under slicer is rusty; dishwasher racks have deteriorated; several items/areas are dirty (bar floor, floor drain at bar, gaskets to all units, hood filters, drip trays to cook line, side of fryer, keg box interior, can opener holder, two-door freezer interior); exterior of many white plastic containers are dingy and gray; some ceiling panels in the kitchen are no longer smooth and easily cleanable.

Carrabba’s Italian Grill, 11237 West Broad Street – The following violations were reported during a routine inspection: food employee failed to wash hands after leaving kitchen and handling cell phone and before engaging in food prep; prep line hand sink doesn’t have hand soap; cook line and prep line hand sinks don’t have paper towels; meat slicer and vegetable dicer are dirty; oil mixture containing garlic and butter is being held at room temperature without proper time control label; empty hand sanitizer jug is stored on a shelf next to clean dishes; cooling and cold-holding records are not being completed; olive oil found on floor under hand sink at bar; forks are not stored inverted; single-service containers are stored unprotected on a dirty shelf; fryer baskets are bent and in disrepair; top edge of plastic salad spinner is broken and chipped; several cutting boards throughout kitchen are scratched and stained; observed dried food debris, dust and grease residue on surfaces of storage racks throughout kitchen and walk-in cooler, broiler and ovens, fan grates in walk-in cooler, exteriors of fryers, dish racks, tupperware in pantry, storage shelves for spices and bread slicer; cold water handle isn’t working at bar hand sink; walk-in freezer floor, floors throughout kitchen and ceiling tiles above cook line are dirty and need cleaning; observed areas of low grout; there are loose floor tiles throughout kitchen.

Arilang Sushi, 2239 Old Brick Road – The following violation was reported during a routine inspection: utensils in bin on prep line are not stored inverted.

Las Palmas Mexican Restaurant, 8415 West Broad Street – The following violations were reported during a routine inspection: hot line hand sink doesn’t have working hot water; sanitizer bucket found in hand sink; dried food debris found inside both microwaves; flour used for breading seafood isn’t labeled with a discard time; two spray cleaner bottles of Windex are unlabeled; tuna is thawing in reduced oxygen packaging in walk-in cooler; knives are stored in gaps between wall and prep units; several cutting boards throughout kitchen are scratched; observed grease buildup on sides of hot line cooking equipment; bar area cooler door gaskets are dirty.

West Lake Chinese Restaurant, 11256 Patterson Avenue – The following violations were reported during a routine inspection: raw food is stored over ready-to-eat food in walk-in cooler; rice, egg rolls and raw meat are uncovered; various containers of food throughout kitchen are undated; white substances on cook line are unlabeled; band-aids, cream and supplements are stored alongside food in dry storage; a raw chicken box is being reused for fried wontons; plastic bags and egg cartons are being reused for food storage; walk-in cooler racks and door are rusty; observed heavy ice buildup in freezer and small standing freezer; facility couldn’t provide test strips for chlorine sanitizer; observed grease and old food on the cook line; gaskets and racks throughout need cleaning; there is buildup around hot holding units; front hand sink doesn’t have cold water and there’s a leak; underneath cookline and the hood are greasy; observed dust and other debris under dry storage racks and other equipment.

McDonald’s, 8975 Staples Mill Road – The following violations were reported during a routine inspection: metal wires on fryer baskets are bent and in disrepair; cement floor pad under water heater/washer isn’t sealed.

Zaxby’s, 9110 Staples Mill Road – The following violations were reported during a routine inspection: hand sink near breading table doesn’t have hand soap; temperature of chicken wings on make line is too low; metal wires on all fryer baskets are bent and broken; reach-in unit on make line has water pooling inside which is dripping onto the floor and rusting the unit; observed dried food debris and grease residue on surfaces of all thaw racks, monitors, bread racks, wire storage racks and under grill; observed heavy buildup of grime on floors throughout the kitchen and walk-in cooler; there are food splatters and grime on walls in dish wash area; floor in front of fryers is damaged and badly pitted.

Burma Sushi & Salad To Go, 11216 Patterson Avenue – The following violations were reported during a routine inspection: facility couldn’t provide parasite destruction letter from salmon provider; temperatures of boiled eggs, tomatoes and miso paste are elevated; menu doesn’t contain asterisks linking foods to the consumer advisory statement.
