Bonefish Grill, 11251 West Broad Street – The following violations were reported during a routine inspection: temperatures of diced tomato and sauces in prep unit are elevated; employee is wearing bracelets on arm while preparing food; to-go containers and plates are stored unprotected/uninverted on dirty shelves; fryers are leaking underneath (heavy buildup of oil under and wiping cloths are being used to soak up the oil); wire shelves inside reach-in cooler are rusted; single cutting boards are heavily scratched and stained; observed dried food debris and dust buildup on monitors, cables, wire racks, walk-in cooler fan grate, bulk flour bins, dry storage racks and inside reach-in coolers and prep units; observed heavy buildup of grease and food debris on surfaces of toaster ovens, microwaves, appliance cords, fryers and grills; there is no hot water at hand sinks (hot water is not available in sufficient quantities to meat the peak hot water demands throughout the food establishment); observed food debris, grease and grime on floors; ceiling panels and walls are dusty.

Waffle House, 7109 West Broad Street – The following violations were reported during a routine inspection: employee cracked eggs and failed to wash hands and change gloves before handling bread and clean plates; temperatures of ham and sliced tomato in prep unit are elevated; ice bucket is cracked and taped together; wire racks inside reach-in coolers are rusted; observed dried food debris and grease buildup on surfaces of waffle irons, coffee makers, walk-in cooler wire racks and fan system, under grills and inside storage drawers; ceiling vents and panels are dusty.

America’s Best Wings, 5650 Brook Road – The following violations were reported during a routine inspection: employee failed to wash hands when entering kitchen and before putting on gloves; temperature of wings in hot holding unit is too low; temperatures of raw shrimp and chicken sitting out on the counter are elevated; live roaches found on cutting board of prep unit and on wheels of prep unit; cutting boards on prep unit are scratched and stained; observed grease buildup on sides of cooking equipment; debris found on shelves of dry storage and over prep units; there is heavy buildup of sauces on racks holding sauce containers; observed black/old sauce buildup on underside of lip of sauce containers; coving tiles are pulling away from the wall under three-compartment sink; there are holes and wall bloating in hallway where single-service items are stored; floors and walls throughout kitchen need cleaning; observed grease dripping from hood.

Apple Spice Junction, 1504 East Parham Road – The following violations were reported during a routine inspection: no handwash signage posted at hand sink; temperatures of tomatoes and cheese in prep unit are elevated; prep and reach-in units have torn gaskets; racks in walk-in are rusty; facility couldn’t provide test strips for bleach sanitizer in dish area.

Pho Thien Phat, 6403 Rigsby Road – The following violations were reported during a follow-up inspection: facility doesn’t have a certified food protection manager; top of grease trap and floor beside grease trap are greasy; there is a loose wall panel to the left of the walk-in cooler door.

Bowl America—Short Pump, 4400 Pouncey Tract Road – The following violations were reported during a routine inspection: employee failed to wash hands after entering kitchen from the front rental counter and before handling food and clean equipment; observed dried food debris on can opener blade; temperature of cheese sauce in dispenser is too low; metal wires on fryer baskets are bent; observed food debris and grime on surfaces of wire racks, walk-in cooler fan system and under front grill.

Thai Top Ten, 9986 Brook Road – The following violations were reported during a routine inspection: employee touched raw meat and failed to wash hands before touching ready-to-eat food; employee put limes on plates with bare hands; back hand sink and bar hand sink don’t have hand soap; front hand sink is blocked by mops and boxes; observed pink mildew on ice machine shield; temperatures of chicken, tomatoes and lettuce in the make table are elevated; knife stored in between prep tables; rice scoops stored in water; hood filters need cleaning.

Bryan Park Bar & Grill, 5516 Lakeside Avenue – The following violation was reported during a routine inspection: tuna is thawing in reduced oxygen packaging in reach-in.

Al Modina, 10158 West Broad Street – The following violations were reported during a routine inspection: temperature of veggie samosa in small warming cabinet is too low; fan grates and storage racks in walk-in cooler are dirty; floor in back storage area is no longer smooth and easily cleanable.

Q Barbeque, 1070 Virginia Center Parkway – The following violations were reported during a routine inspection: temperatures of potato salad, tomatoes and lettuce in prep table and ribs, beans and collard greens in walk-in cooler are elevated; containers on clean dish rack are not in good condition (plastic burned, stained, etc.).

Chiki’s Pancake, 1404 North Parham Road – The following violation was reported during a routine inspection: there is a leak under the three-compartment sink.

The Wine Whisperer, LLC, 11535 West Broad Street – The following violations were reported during a routine inspection: raw fish is stored over puff pastry; menu doesn’t disclose that caviar is raw; temperature of pimento cheese mix is elevated (wasn’t cooled before being placed into make table); cardboard is lining shelves; mop sink doesn’t have a dual check valve installed; there is a leak under the three-compartment sink; a “no smoking” sign isn’t posted.

E-Loteria Botanas & Ice Cream, 9022 West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have a person in charge working; facility couldn’t provide verification that employees have been trained in health; facility doesn’t have written procedures for cleaning up vomit/diarrhea; observed an open cup in the kitchen; employee didn’t wear gloves when cutting vegetables for pico; raw eggs stored over cheese dip; temperatures of refried beans and rice inside warmer are too low; temperature of beef birria in walk-in cooler is elevated because it wasn’t cooled properly; temperature of horchata in dispenser is elevated; a blue liquid is unlabeled; the back door isn’t self-closing; food containers stored directly on walk-in floor; drip tray and walk-in threshold are dirty; chest freezer is full of ice; a “no smoking” sign isn’t posted.

Little Angela’s Ristorante and Pizzeria, 10246 Staples Mill Road – The following violations were reported during a routine inspection: back hand sink doesn’t have paper towels; date when last shellstock (clams and mussels) were sold wasn’t recorded on tags; shellstock tags are not kept in chronological order; the identification tag wasn’t in the container of mussels in the walk-in cooler; observed dried food debris on can opener and slicer; door gaskets on pizza prep unit and reach-in freezer are torn; racks in walk-in cooler are rusted; prep unit cutting boards are heavily scratched and stained; observed dust buildup on wire racks, ceiling panels and vents; sides of grill are greasy.

Taziki’s Mediterranean Café, 4024-C Cox Road – No violations reported during a follow-up inspection.
