Hurley’s Tavern / Devil’s Lettuce, 4028-J Cox Road – The following violations were reported during a routine inspection: in-use knives stored in dirty crevice between prep unit and table; metal wires are broken and bent on all fryer baskets; observed dust and food debris buildup on wire storage rack and inside reach-in cooler near fryers; walk-in cooler floor is pitted.

Noorani Kabab House, 2757 Hungary Spring Road – The following violations were reported during a routine inspection: raw eggs stored over potatoes and raw meat stored over cooked food in walk-in; dish machine has 0 ppm sanitizer; dishwasher is washing dishes without sanitizing (sanitizer won’t pull from container); temperature of biryani made last night is elevated because of improper cooling methods; temperature of rice in warmer is too low; two small roaches found in prep units; spoons are stored utensil side up at the buffet; to-go containers are not stored inverted on shelf; walk-in racks and spice rack are rusted; equipment not in operation should be removed from facility; observed food debris in bottom of units and on gaskets; walk-in fans are dusty; sides of cooking equipment have grease buildup.

Jackie’s on Laburnum, 1241 North Laburnum Avenue – The following violations were reported during a routine inspection: bar area hand sink is blocked by various objects and dishes; observed dried food debris inside microwave and on can opener blade; menu lists catfish but the facility is serving swai; bulk containers (sugar, rice, etc.) throughout kitchen are unlabeled; single and prep unit cutting boards are heavily scratched and stained; facility couldn’t provide sanitizer test strips; there is dust buildup and food debris on floor fans in kitchen; sides of hot line equipment have grease buildup; cooler door gaskets throughout kitchen, exterior and interior of three-door reach-in freezer and exterior of ice machine are dirty; ceiling tiles are dusty and stained, especially above hot line; there are food stains and dried food debris on walls throughout kitchen; floor under deep fryers is greasy.

India K’ Raja Restaurant, 9051-55 West Broad Street – The following violations were reported during a routine inspection: gasket on small reach-in cooler is torn; observed dust and food debris buildup on wire storage racks, prep counters, blue dish racks and inside reach-in coolers; there is a hole in the wall at the end of the prep line; ceiling panels in dry storage are damaged and stained.

Pho Thien Phat 2, 9031 West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; food employee failed to wash hands after handling raw meat and before engaging in food preparation; dish wash employee handled soiled utensils and cups and didn’t change gloves before touching clean equipment; food and other items found inside both kitchen hand sinks; hand sink at drink bar is being used as a dump sink; observed dried food debris on slicer; temperatures of sprouts in reach-in cooler and brisket and chicken in prep unit are elevated; improper cooling methods observed for pork chops and large bowls of noodles tightly covered and sitting out at room temperature; dead roaches found on floor in dry storage areas; rice scoops are stored in ambient temperature water; prep and single cutting boards are heavily scratched and stained; wooden pallets are being used to store broth buckets in walk-in cooler; observed dust and food debris on surfaces of portable fan, shelves storing clean plates, single-service items, coffee maker/grinder, sugar on storage shelves and spice carts; wire racks in walk-in cooler have grime and food debris; floor tiles and wall corners are damaged; there are holes in walls; ceiling panels are broken and/or missing; walk-in freezer door is rusted and damaged; wall around door is rusted; there is heavy ice buildup around door and inside freezer; observed heavy staining on walls and ceiling panels; food debris and dirt found on floor under equipment.

Dunkin Donuts, 4050 Spring Oak Drive – No violations reported during a routine inspection.

Five Guys Burgers and Fries, 10841 West Broad Street – The following violation was reported during a routine inspection: time isn’t being tracked for blanched fries.

Hyatt House Richmond/Short Pump, 11800 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t verify that all food employees have received training on illness reporting responsibilities; high temp dish machine isn’t reaching the required surface temperature; temperatures of sausage and hashbrowns under heat lamp and sausage and hashbrowns on serving line are too low; temperatures of half-and-half, waffle batter and cream cheese on serving line are elevated; quat sanitizer solution in wiping cloth bucket is below required amount; quat sanitizer test strips expired in 2010; other test strips by the dish machine measure pH (not the right kind for a quat sanitizer); there is a leak under the hand sink (container is being used to catch dripping water); mops are not hung to air dry; observed grease and food debris on floor under fryer; wall behind dish machine is soiled; caulking at prewash sink is moldy.

Send-A-Chef, 3073 Mechanicsville Turnpike – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager on staff; employee restroom doesn’t have paper towels; observed dried food debris on can opener blade and in both microwaves; temperatures of oxtails, chili and mashed potatoes in walk-in cooler are elevated; time isn’t being tracked for cooked vegetables and breader; cleaning products are stored under flat top grill next to food; tilapia is thawing in reduced oxygen packaging; several bulk containers in kitchen are unlabeled; interior, exterior and rubber door gaskets of both homestyle reach-in coolers are dirty; observed heavy grease buildup on exteriors and sides of hot line equipment, in between equipment and underneath equipment; chest freezer gaskets are dirty; walls behind and floors under hot line equipment are greasy; there are dried food stains and debris on walls throughout the kitchen; hood filters in ventilation system are greasy.

My Empanada, 1421 Blue Jay Lane – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; person in charge couldn’t provide written procedures; an employee failed to sanitize blenders and utensils after washing; temperatures of empanadas in front warmer case are too low; coca cola reach-in cooler has a torn door gasket.

Bojangles, 5441 South Laburnum Avenue – The following violations were reported during a routine inspection: temperatures of cheese, coleslaw and shredded cheese in small prep unit are elevated; this unit is not maintaining foods at the proper temperature; food employee is wearing jewelry on her arms while preparing food; cutting board on small prep unit is heavily scratched and stained; metal wires are bent on fryer baskets; observed dried food debris, grease and flour buildup on surfaces of wire racks, fryers, equipment at biscuit making area, rolling carts, rice bin and shelves storing to-go containers, and inside reach-in freezer, wrapper holders and hand wash sink near office; observed food debris and grime buildup on floors; ceiling vents and panels are dusty.

Cook Out Restaurant, 4300 South Laburnum Avenue – The following violations were reported during a routine inspection: upper portion of ice machine has yellow mold; soda/ice dispensers throughout kitchen have moldy, sticky soda residue on them; two spray cleaner bottles are unlabeled; temperatures of hot dogs, raw chicken breast and shredded chicken in upper portion of prep table are elevated; items that are cooling (shredded chicken) shouldn’t be stored in upper portion of prep table; several cutting boards are scratched and stained; walk-in cooler and freezer fan guards are dusty; observed grease residue on sides and in between hot line equipment and on undersides of deep fryers; there is a leak in plumbing under prep sink which is leaving puddles of water on the floor; wall behind file cabinet and ice machine is dusty; light fixture and surrounding ceiling tile are dusty; ceiling tile has light food stains; observed grease buildup on fire suppression dispensers in hood system.

Hardee’s, 4841 South Laburnum Avenue – The following violations were reported during a routine inspection: temperatures of sausage/cheese omelet blend in ice bath and ham in small reach-in unit are elevated; improper cooling methods observed for trays of ham (wrapped and stacked) and tomatoes (covered and stacked) in walk-in cooler; observed grease and food debris buildup on surfaces of fryers, rolling carts, warmer cabinets, floor mats in freezer, monitors, cables and D/T soda drain, and inside reach-in coolers/freezers and cup holders.
