JAN. 9

Westwood Club, 6200 West Club Lane – The following violations were reported during a routine inspection: observed a worker eating food while prepping food; it appears that no sanitizer is hooked up to the bar dish machine; garlic in oil double-nested in make table found at an improper temperature; cream cheese and goat cheese are past their expiration dates; no date marks found on potatoes, green beans or goulash; stainless steel cleaner in bottle is unlabeled; observed numerous dead and live roaches (facility was bombed last week); food is stored directly on the walk-in floor; torn film on outside of convection oven; cutting board at bar has split; faucet at two-compartment sink doesn’t stop dripping; vacuum breaker is leaking at dish machine; hand sink in service area is loose; beer lines are moldy; floor is dirty in dish room and broom closet; linoleum in service area is loose; floor and wall in broom closet is not smooth and easily cleanable; tile on dish room floor is broken; diamond plate floor panel in beer walk-in is loose.

Pho Thien Phat, 6403 Rigsby Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; person in charge unable to verify that employees have been trained in regards to employee health; cutting board is dirty; eggs were left out at room temperature; temperatures of sprouts and the display case are elevated; no date marks found on sliced chicken or cooked noodles; time is not being tracked for lettuce or cooked pork; improper cooling methods observed for beef, pho and spring roll mix; there is no thermometer in milk refrigerator or outside unit; top of grease trap, shelf under chargrill and legs to the cook line are dirty; mop sink is dripping; vacuum breaker is broken; wall panel to the left of walk-in door is loose; ceiling panels and floor beside grease trap are dirty.

U Play Indoor Soccer, 7501 Ranco Road – The following violations were reported during a routine inspection: observed food items stored directly on the floor; facility couldn’t provide test strips for three-compartment sink.

Bernie’s Sub & Pizza Shop, 415 North Ridge Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; the employee health policy is unverifiable; hand sink doesn’t have hand soap; no date marks found on any food; time is not being tracked for tomatoes or lettuce; time control policy is missing; no test strips for sanitizer tabs; exterior of spice containers and shelves are soiled; floors are extremely dirty; hand sink is dirty.

Sandston Bistro & Shake, 18 East Williamsburg Road – The following violations were reported during a routine inspection: certified food protection manager certificate is expired (person in charge took exam yesterday); lettuce residue found on slicer blade (slicer hasn’t been used today); some foods in the walk-in (tomatoes, hot dog, pork) have marginal temperatures; foods in top rightside of make table (bologna, hot dog, cheese) have elevated temperatures; wiping cloth used for wiping shake mixer attachment isn’t stored in a sanitizing solution; no sanitizer residual detected on wiping cloth; wooden spoon in tea appears worn; walk-in racks are rusty; walk-in refrigerator is running high; mops are not hung to air dry.

Arianna’s Italian Grill, 5107-09 Lakeside Avenue – The following violations were reported during a routine inspection: raw shell eggs and raw shrimp are stored over ready-to-eat foods; several bulk containers of white powder are unlabeled; cutting boards throughout kitchen are scratched and stained; cooler door gaskets throughout kitchen are dirty; wire storage racks on hot line are dirty/dusty; sides of cooking equipment on hot line have grease buildup on them; there are food stains on floors and walls throughout kitchen; observed grease and food on floor under and behind deep fryers; observed dust buildup on ceiling; floor tile in back of kitchen is missing.

Firehouse Subs, 9069 Staples Mill Road – The following violations were reported during a routine inspection: prep and single cutting board are heavily scratched and stained; observed food debris inside toaster and on wire storage racks in back storage area.

O’Charley’s Restaurant, 9990 Brook Road – The following violations were reported during a routine inspection: observed mildew on soda nozzle and shoot of ice machine; temperature of cheese in reach-in is elevated; salmon is thawing in reduced oxygen packaging; racks on units are rusty; gaskets on cook line are torn; observed heavy ice buildup on backs of units across cook line; cutting boards on cook line are stained; observed heavy grease buildup on backs, sides, wheels and handles of and around fryers; grease and food debris found under grill; observed food and other debris in units; there is buildup on racks in dry storage; observed food and grease buildup under equipment on cook line; mold/mildew found on walls behind three-compartment sink and dish machine; ceiling and hanging computers are dusty; hood filters on cook line and over pots are greasy and dusty.

Taziki’s Mediterranean Café, 4024-C Cox Road – The following violations were reported during a routine inspection: can opener blade is soiled; temperatures of cut melon, cucumber tomato salad and chicken roll up in top of make table are elevated; make table isn’t maintaining foods at the proper temperature; clean containers are stacked while still wet.

Subway Sandwiches & Salads, 4014 Glenside Drive – The following violation was reported during a routine inspection: cutting boards on make tables are heavily scratched and stained.

Pizza Hut, 9189-B Staples Mill Road – The following violations were reported during a routine inspection: metal wires on fryer baskets are bending; observed heavy buildup of grime and food debris inside dish machine; observed buildup of grease and dust on surfaces of green wire racks and carts, measuring cups and oil spray bottles stored on clean rack.

Edwards’ Catering, 8308 Fredonia Road – The following violations were reported during a routine inspection: dried food debris found inside microwave; observed dried food stains on wall behind stove.

Big Chile, 10184 West Broad Street – The following violations were reported during a routine inspection: dented cans found on racks in dry storage room; temperature of two large pans of refried beans in walk-in cooler is elevated (they have exceeded the two-hour cooling timeframe); to-go containers are stored unprotected on a dusty surface; metal wires on several fryer baskets are bending; prep unit and single cutting boards are heavily scratched and stained.
