JAN. 3

Tony’s Italian Restaurant, 3409 Cox Road – The following violations were reported during a routine inspection: observed an employee fail to change gloves after wiping his face with his shirt; several foods in the walk-in cooler (pasta, ham, tomato and salami) have elevated temperatures; observed ready-to-eat fries draining on a used cardboard box on the prep table; fryer baskets are in disrepair (metal wire is broken off); wire racks in walk-in cooler are heavily rusted; gaskets on prep unit doors are torn; several pizza dough pans are bent and not easily cleanable; observed dried food debris buildup on surfaces of wire racks near prep/cook areas, fan grates in walk-in cooler and fryer baskets.

McDonald’s, 8060 West Broad Street – The following violations were reported during a routine inspection: fryer baskets are in disrepair (metal wires are bent and broken); sheet pans are pitted; grease traps at burger grill aren’t working properly; observed heavy buildup of grease under grills and grease traps.

Chicken Fiesta, 7925 West Broad Street – The following violations were reported during a routine inspection: menu doesn’t contain an asterisk linking steak to the footnote statement that it may be served raw and/or undercooked; heavily soiled cloths are being used to wipe cutting boards, knives and employee hands; to-go containers are stored unprotected on dirty and dusty shelves; all prep and single cutting boards are heavily scratched and stained; all wire shelves are lined with dirty cardboard; metal wires are broken off or bent on several fryer baskets; wall near back door is damaged; floor tiles in walk-in are broken.

Jefferson Lakeside Club, 1700 Lakeside Avenue – The following violations were reported during a routine inspection: hot water at hand sinks doesn’t reach the required temperature; raw burgers stored next to lettuce; dish machine has 0 ppm sanitizer when tested; observed mold buildup on soda nozzle and can opener; roasted corn, grilled chicken and salsa are past their expiration dates; dish machine and three-compartment sink are in disrepair; observed grease and other buildup on sides of cook line equipment; overhead heaters are dusty; there is buildup on piping behind equipment, floors under equipment, near drains and under dish machine.

Bowlero Richmond, 6540 West Broad Street – The following violations were reported during a routine inspection: no handwash signage is posted in women’s restroom; red sanitizer buckets found in kitchen and bar area hand sinks (hand sinks are for handwashing only); bar area hand sink doesn’t have paper towels; dried food debris found inside microwave; spray cleaner bottles in kitchen and bar area are unlabeled; observed several fruit flies at bar area, specifically underneath and near the three-compartment sink; sides of cooking equipment on hot line have grease buildup; beer cooler fan guard is moldy; dish drying wire rack is dusty; back freezer door rubber gaskets are dirty.

Auntie Anne’s Pretzels, 11800 West Broad Street – The following violations were reported during a routine inspection: temperatures of hot dogs and cheese out for prep are elevated; spray bottles throughout are unlabeled; observed black mold/mildew on reach-in racks and fans.

Hardee’s, 3201 East Parham Road – The following violations were reported during a routine inspection: observed grease residue and old labels on containers stored in storage room; temperature of tomatoes in walk-in cooler are elevated (after prep they were tightly covered); to-go containers are stored unprotected on a greasy surface; metal wires are bent and breaking on fryer baskets; observed encrusted grease deposits and/or other soil deposits on sheet pans; observed dried food debris and grease buildup on surfaces of fryers, ovens, grills, broiler, cables, monitors, hot hold units, prep tables, rolling racks, inside reach-in freezer, inside wrapper holders, and racks and fan grates in walk-in cooler; mop sink faucet doesn’t have a dual check valve installed; there is food debris and grime buildup on floors and walls; ceiling panels and vents are dusty; trash found on floor drain under drive-thru ice bin; light bulbs above make unit are not covered by a protective shield.

Burger King, 5108 Nine Mile Road – No violations reported during a routine inspection.

Sea By Thai Restaurant, 2452 Old Brick Road – The following violations were reported during a routine inspection: ice scoops are stored in a dirty container at the bar; dirty cardboard is lining shelves in the reach-in freezer; observed dust buildup and grease residue on wire storage racks; floors and walls are not smooth and easily cleanable (paint is chipping).

Performance Pickleball RVA, 8641 Quioccasin Road – The following violations were reported during a routine inspection: facility doesn’t have an employee health policy set up; facility doesn’t have written procedures for cleaning up vomit/diarrhea; bar hand sink doesn’t have paper towels; temperatures of ham and salami in the make table are elevated (they’re piled too high); no date found on open packs of cooked chicken; single-use containers are not stored protected in order to prevent contamination; faucet at three-compartment sink doesn’t reach all sinks.

Los Panchos Mexican Restaurant, 3532 Pump Road – The following violations were reported during a routine inspection: cans of tomato sauce stored on rack are badly dented; observed buildup of black mildew inside soda gun at bar; tilapia is improperly thawing in reduced oxygen packaging; to-go containers and lids are stored unprotected on a dusty surface; metal wire is broken and bent on fryer baskets; door on upright warmer unit doesn’t close completely; prep unit and single cutting boards are heavily scratched and stained; observed dust buildup on ceiling panels and vents above cook line.

La Cabaña, 4086 Crockett Street – The following violation was reported during a routine inspection: in-use utensils are stored in ambient temperature water.

Waffle House, 3202 East Parham Road – The following violations were reported during a routine inspection: an employee failed to wash their hands after cleaning the floor and before continuing food prep; observed food debris buildup inside reach-in cooler, on shelves storing bowls, under grill, on crates above three-compartment sink storing lids, and in utensil holders.

Lebanese Bakery and Shawarma, 6215 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t verify that employees have been trained on their illness reporting responsibilities; chlorine sanitizer concentration at three-compartment sink is too strong; spoon and falafel scoop stored in ambient temperature water; refrigerator make table is not maintaining foods at the proper temperature; brush used for bread has a metal band; falafel scoop is rusty.
