MPM Tiki Bar, 2451 Old Brick Road – The following violations were reported during a routine inspection: person in charge unable to verify that employees have been trained on their illness reporting requirements; facility doesn’t have written procedures for cleaning up vomit/diarrhea; prep area hand sink doesn’t have paper towels; raw tuna and salmon incorrectly stored over edamame in lowboy; raw scallops incorrectly stored next to lemons and raw calamari incorrectly stored next to pineapple; observed mold-like substance in ice machine; employee seen washing dishes without sanitizing; temperature of raw chicken on ice on line is elevated; temperatures of squash and tomatoes in make table are elevated; crab rangoons are past their expiration date; there is no time control policy for reconstituted noodles out on the line; menu has inadequate disclosure for cheeseburger and poke that may be served undercooked/raw; dishes are stacked while still wet; facility couldn’t provide test kit for dish machine; observed grease buildup on equipment and floor at cook line; trash can isn’t covered in women’s restroom.

NES Cornhole Lounge, 2443 Old Brick Road – The following violation was reported during a routine inspection: wet mop isn’t being hung to air dry.

Starbucks Coffee Company, 3820-A Gaskins Road – The following violations were reported during a routine inspection: facility couldn’t provide temperature measuring device for hot water dish machine; observed food debris and trash on floor under cabinets and sinks; floor tiles are pitted and damaged on service line.

Terraza, 6115 Staples Mill Road – The following violations were reported during a routine inspection: observed in-use open drink mug stored on prep cutting board where food is being prepared; temperature of deep pan of rice is elevated; temperature of black beans in steam table is too low; temperature of two bowls of papusa mix (beans and cheese) is elevated; to-go containers are not stored inverted to prevent contamination and the surface they are stored on is dirty; single cutting boards are heavily scratched and stained; small salsa reach-in cooler has torn gasket; observed dust and food buildup on surfaces of receipt printers, shelves storing bowls and plates, on top of ovens, and fan system in walk-in cooler.

Hunan Café, 9117 Staples Mill Road – The following violations were reported during a routine inspection: in-use open drinking containers are stored in a manner that may contaminate food, food contact surfaces or utensils; hand sink is being used as a dump sink (hand sinks are for handwashing only); bowl of raw shrimp incorrectly stored on top of raw broccoli near wok station; observed moldy old labels on container storing raw onions in walk-in cooler; two containers of egg rolls are not date marked; containers of salt, sugar, etc. on spice cart are unlabeled; single-service articles are stored on a dirty surface unprotected and uninverted to prevent contamination; single and prep cutting boards are heavily scratched and stained; observed dried food debris, grime and dust on surfaces of all wire storage racks, fan system in walk-in cooler and inside reach-in cooler/freezer; observed food debris and grime on floors under and behind equipment.

Zaxby’s, 5816 West Broad Street – The following violations were reported during a routine inspection: observed a kitchen staff member wash hands for less than 10-15 seconds before putting on new gloves; front hand sink and breading table hand sink are out of paper towels; dish room hand sink’s paper towel dispenser isn’t working; ventilation system is missing a hood filter; observed dust, flour and grease buildup on surfaces of fryers, grills, cables, monitors, dry storage racks, dish drying racks, and sides/in between hot line equipment; observed grime buildup on exterior of cooler doors throughout the kitchen; rubber door gaskets throughout kitchen, walk-in cooler doors and walk-in cooler racks are dirty; there is grease and food buildup on floors and walls behind tea urn and behind drive-thru drink machine; ceiling panels and vents throughout restaurant are dusty; ceiling tile above breading table is stained.

Chuck E. Cheese, 9030 West Broad Street – The following violations were reported during a routine inspection: two-door and three-door units have a torn gasket; observed leak at faucet at hand sink and three-compartment sink.

Sandston Moose Family Center, 4505 Oakleys Lane – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager on staff; back bar hand sink doesn’t have hand soap or paper towels; there is no handwash signgage posted in the men’s room; dried food debris found inside microwave; cutting boards used in the kitchen are scratched; facility doesn’t have sanitizer test strips; observed heavy grease residue on sides of and in between hot line equipment (deep fryers, flat top grill and grill); trash can in women’s restroom doesn’t have a lid; floor under deep fryers and wall behind hot line are greasy.

Firehouse Subs, 4501 South Laburnum Avenue – The following violations were reported during a routine inspection: temperatures of potato soup and meatballs are too low; observed employees wearing bracelets while preparing food; employee is working in the food service area without proper hair restraints; soiled, open box of to-go lids found under far hand sink; observed heavy buildup of grease on inside rollers of toaster; observed trash and food debris on floors in back prep area and on service line.

Panera Bread, 4483 South Laburnum Avenue – The following violations were reported during a routine inspection: towel dispenser at dishwash area hand sink isn’t working; temperature of lettuce in bottom pan of two-door cooler is elevated (drawer doesn’t shut all the way); employee with long, bushy beard isn’t wearing a beard guard; single and small prep cutting boards are heavily scratched; some wire racks are rusted; bottom door on salad reach-in cooler doesn’t close; observed carbon and food residue buildup on baking sheet pans; food debris buildup found on floors.

Qdoba, 4501 South Laburnum Avenue – The following violations were reported during a routine inspection: hand soap dispenser at hand sink near register is broken; door gaskets are torn on service line reach-in cooler; some wire racks are rusted; observed grease residue and food debris on surfaces of all wire storage racks, portable fan and prep table shelves.

McDonald’s, 11311 Nuckols Road – The following violations were reported during a routine inspection: pink slime found on ice chute at soda station; temperature of fries in hot holding is too low; no sanitizer detected in solution in wiping cloth bucket; one of the fryers is not in service; shake machine is out of order and the temperature of the unused product inside the unit is elevated; observed food residue in freezer; mop sink doesn’t have a dual check valve installed; there appears to be a leak in the line/equipment of the soda dispensing system (floor is covered in liquid waste when cabinets are opened); observed buildup of grease and food residue under cook line equipment; there is heavy ice buildup in the freezer; observed grime and leaking liquid under soda machine.

Tropical Smoothie Café, 11321-C Nuckols Road – The following violations were reported during a follow-up inspection: observed buildup and metal filings on can opener blade; freezer and refrigerator are still waiting to be repaired or replaced (currently not in use).
