Sushi Masa, 8087 West Broad Street – The following violations were reported during a routine inspection: open drink cup found on prep surface with clean plates; employee failed to sanitize equipment after washing and before storing on drying rack; temperatures of two pans of tempura shrimp cooked two hours prior are elevated; raw tuna improperly thawing in reduced oxygen packaging; in-use knife improperly stored in dirty crevice between prep unit and table; rice scoops stored in ambient temperature water.

Jimmy John’s Gourmet Sandwiches, 7005 Three Chopt Road – No violations reported during a follow-up inspection.

Mosaic Restaurant, 6229-A River Road – The following violations were reported during a follow-up inspection: facility couldn’t provide parasite destruction information from vendor; slicer is dirty; disclosure is missing from the menu; chicken breasts found improperly cooling in a covered container; dead roaches observed in the dish area; fryer cabinets are greasy; there is a leak in the corner of the spray sink; spray hose is broken at three-compartment sink; hand sink at bar isn’t working; observed buildup on floor and wall behind/under dish machine.

La Victoria Latino Products, 4309 Nine Mile Road – The following violations were reported during a routine inspection: employee restroom in kitchen doesn’t have handwash signage posted; half-way covered container of salsa incorrectly stored under raw chicken in three-door reach-in cooler; dried food debris found inside microwave; shelves and wire racks throughout kitchen are lined with aluminum foil; lowboy cooler’s rubber door gasket is torn; observed grease buildup on sides of hot line equipment; bottom interior of reach-in freezer is dirty.

Top’s China, 68 South Airport Drive – The following violations were reported during a routine inspection: observed staff washing hands for less than 10-15 seconds; main kitchen hand sink doesn’t have paper towels; plastic scoop found in hand sink (hand sinks are for handwashing only); chicken wings and General Tso’s chicken are sitting out at room temperature and not on time control; multiple foods in walk-in cooler have elevated temperatures; can of Raid found in kitchen; salt, sugar and MSG on hot line are unlabeled; food incorrectly stored directly on floor of walk-in freezer and cooler; cardboard boxes are being reused throughout kitchen; cardboard is also being reused as storage for food; wire racks throughout kitchen and inside walk-in cooler are rusty; cutting boards throughout kitchen are heavily scratched and stained; walk-in cooler fan guards and wire racks and back reach-in freezer doors are dirty; observed dried food debris and grime on shelves and inside refrigeration units throughout the kitchen; there is grease buildup between hot line equipment; dumpster door and lid were left open; ceiling tiles throughout kitchen are stained; hoods are greasy.

Tropical Smoothie Café, 11321 Nuckols Road – The following violations were reported during a routine inspection: facility couldn’t provide certified food protection manager certificate; person in charge unable to verify that employees have been trained on their illness reporting requirements; two knives in prep area that haven’t been used today are dirty; person in charge says knife and pizza cutter used for quesadillas are washed at the end of the day (should be washed and sanitized every four hours); observed food debris and metal shavings on can opener blade; temperatures of sweet potatoes and corn in small fridge are elevated; roasted red onions, sweet potatoes and corn are past their expiration dates; tomatoes, chicken, spinach and pickled red onions are not date marked; chemicals incorrectly stored on shelf above single-service items; bins with powders/supplements have old labels that are not legible; containers storing seeds and powders don’t have lids; takeout containers are not stored in a way to prevent contamination; single-service smoothie cups have been splashed with what appears to be smoothie residue; white cutting board is scratched and discolored; small refrigerator is not maintaining food at the proper temperature; food containers are cracked; large soiled towel found on clean dish drain board; “menumaster” equipment is covered in food debris; storage racks at the smoothie blender are soiled; mop sink doesn’t have a dual check valve installed; mop is not being hung to air dry; there is food splatter on the ceiling; observed grime on wall by prep sink, behind panini press and inside prep sink.

Five Guys Burgers and Fries, 1601 Willow Lawn Drive – The following violation was reported during a routine inspection: there isn’t a backflow preventer installed on hose with pressure device.

La Michoacana Crave & Munchies, 8129 Staples Mill Road – No violations reported during a routine inspection.

Marco’s Pizza, 9681 West Broad Street – The following violations were reported during a routine inspection: employee restroom doesn’t have paper towels; temperature of ha in the make table is elevated; wrapped pizza boxes incorrectly stored directly on floor in back room; mop sink doesn’t have a dual check valve installed; wet mop isn’t hung to air dry.

Thai Diner, 8059 West Broad Street – The following violations were reported during a routine inspection: observed moldy old labels on containers on wire rack; mice droppings found on prep shelf where food is stored; dead mouse found in sticky trap under reach-in cooler; egg cartons are being reused; prep cutting boards are heavily scratched and stained; fryer baskets are in disrepair; observed grease dripping from wire racks above three-compartment sink; observed food debris and grease on prep shelves and counters, inside reach-in freezer and on oil collection buckets; there is a buildup of grease and food debris on floors, walls, under/behind equipment and tables, and on hood filters and suppression nozzles; grease is dripping from system; one filter is damaged.

Taco Bell, 9420 West Broad Street – No violations reported during a routine inspection.

Taco Bell, 5811 West Broad Street – The following violations were reported during a routine inspection: time isn’t being tracked for lettuce, tomatoes and cheese on both prep lines; prep cutting boards are heavily scratched and stained; observed dents in metal pans and lids; tortilla pans are heavily stained; observed dust and grease on surfaces of wire storage racks and inside wrapper holders.
