McDonald's, 11218 West Broad Street – The following violations were reported during a routine inspection: observed an employee touch pants with gloved hands and then fail to change gloves and wash hands before handling food; inside the reach-in freezers need cleaning; there is heavy grease accumulation on the floor under the grill.

Ukrop’s Market Hall, 7250 Patterson Avenue – The following violations were reported during a routine inspection: temperature of thawed chicken in two-door unit is elevated because of improper thawing process; there is no date mark on open container of cream cheese; time is not being tracked for used breading; potatoes are being cut before washing (regulations require vegetables to be washed prior to cutting); cardboard box cannot be used to store cut potatoes.

Rare Olde Times, 10602 Patterson Avenue – The following violations were reported during a routine inspection: facility couldn’t provide parasite destruction documentation from salmon supplier; pickled red onions in the walk-in are past their expiration date.

Shawarma Bistro, 12422 Gayton Road – The following violations were reported during a routine inspection: person in charge couldn’t produce his certified food protection manager certificate; facility doesn’t have written procedures for cleaning up vomit/diarrhea; okra soup in the refrigerator is past its expiration date; observed small flies by mop sink and soda dispenser; utensils for falafel incorrectly stored in ambient temperature standing water; observed grease buildup and food debris on floor under fryer; mop isn’t hung to air dry.

Roslyn Conference & Retreat Center, 8727 River Road – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit/diarrhea; pink slime found inside ice machine; can opener, can opener holder and drip trays to equipment are dirty; cooked sausage is past its expiration date; no date mark found on open ham in the walk-in; croutons and cranberries on the buffet are not under a sneeze guard; there are no heat tapes or holding thermometer to monitor dish temps; dish machine is leaking; floor and lower wall in dish area are dirty.

Maggiano’s Little Italy, 11800 West Broad Street – The following violations were reported during a routine inspection: bar hand sink doesn’t have paper towels; no sanitizer in glass washer; temperature of mashed potatoes in alto-sham is too low; shrimp, cheese and pasta in top and in drawers at the line are elevated; spray bottle in chemical closet is unlabeled; some cutting boards are worn at the corners and have deep scratches; several items need cleaning (inside refrigerators, fan covers, hood filters, beer walk-in racks); some ceiling vents are dusty.

Daddio’s Grille, 12385 Gayton Road – The following violations were reported during a routine inspection: shell eggs incorrectly stored over ready-to-eat foods in the walk-in; menu is missing disclosure for foods that may be served undercooked; observed several small flies at bar on non-smoking side; gasket is torn on chest freezer at bar; heavy ice buildup observed in chest freezer at bar; soda gun at bar is leaking onto the floor; observed grease buildup on sides of oven; refrigerators at the bar need cleaning; walls and floor at bar are soiled; wall and pipes under three-comparment sink are soiled.

Weinstein JCC, 5403 Monument Avenue – The following violations were reported during a routine inspection: hand sink in warming kitchen doesn’t have paper towels; eggs incorrectly stored over peppers; several items/areas are dirty (can opener, drip trays to all stoves, interior of open warmer, hand sink, interior of microwave and microwave counter); hot water faucet isn’t working in warming kitchen; carpet should be removed from warming kitchen; there is a hole in the wall under the three-compartment sink.

Baskin-Robbins, 5510 West Broad Street – The following violations were reported during a routine inspection: observed a live pest crawl out from under the small Pepsi cooler and then crawl back underneath (cooler has dried food debris, crumbs and sprinkles underneath); product incorrectly stored directly on freezer floor; observed food stains more than 24 hours old on exterior of electric table mixer; observed food debris and crumbs in reach-in coolers and take-home ice cream coolers; cabinet shelves throughout kitchen need cleaning; white reach-in cooler and walk-in freezer gaskets are dirty; walk-in freezer racks and door handle are dirty; food debris found on floors underneath counters and equipment throughout kitchen; there are food stains/splatters on fan guards and backs of take-home ice cream coolers; floor tile in back room/office/dry storage area is cracked; floor tile in dining area is loose.

Mosaic Restaurant, 6229-A River Road – The following violation was reported during a follow-up inspection: the attempt to use ice in mid unit has failed because temperature of edamame stored there is elevated (mid unit to be replaced).

Whisk, 8308 Staples Mill Road – The following violations were reported during a routine inspection: open bag of candy and drink cup found on prep table in kitchen; a single-use container is being used as an ice scoop; observed dust and flour buildup on surfaces of ovens, all wire storage shelves, prep table shelves and portable fans; floors under and behind equipment and tables are dirty; ceiling vents are dusty; several ceiling panels are stained.

Chipotle Mexican Grill, 4930 West Broad Street – No violations reported during a routine inspection.
