MOD Pizza, 8671 Quioccasin Road – The following violations were reported during a follow-up inspection: temperatures of salami and blue cheese in both stations are elevated; floor edges still need to be cleaned thoroughly.
Davis Elementary School, 8801 Nesslewood Drive – The following violations were reported during a routine inspection: dumpster is located on grass (place on asphalt/concrete); wall in restroom has peeling paint.
Dumbarton Elementary School, 9000 Hungary Spring Road – The following violations were reported during a routine inspection: the middle window A/C unit is dripping condensate water onto prep tables, containers and equipment stored underneath; ceiling panels in the kitchen are not smooth, durable, easily cleanable and nonabsorbant.
Glen Allen High School, 10700 Staples Mill Road – The following violation was reported during a routine inspection: observed dust buildup on surfaces of ceiling panels and vents.

Crafted, 4900 Libbie Mill East Boulevard – The following violations were reported during a routine inspection: observed mold buildup inside upper area of ice machine; dried food debris found on can opener blade; dish machine isn’t dispensing sanitizer; temperatures of tomatoes, cheese, cut romaine and leafy greens in lowboy unit are elevated; in-use utensils improperly stored in stagnant room temperature water; multiple cutting boards are heavily scratched and stained; walk-in cooler fan guards are dusty; observed dust/mold/dried food debris on wire racks in walk-in cooler.
Chipotle Mexican Grill, 10501 West Broad Street – The following violations were reported during a routine inspection: temperatures of carne asada and chicken in back line are too low; temperature of seasoned steak cubes on front serving line is too low.
2 MPS Mediterranean Market Deli, 7103 Staples Mill Road – The following violation was reported during a routine inspection: no supervisors or managers are certified food protection managers.
Laburnum Elementary School, 500 Meriwether Avenue – The following violations were reported during a routine inspection: microwave interior and can opener blade are dirty; multiple foods in reach-in cooler are elevated; several wire pan liner racks used in kitchen are rusty; observed dried food stains and debris on stainless steel wall underneath serving line.
Quioccasin Middle School, 9400 Quioccasin Road – The following violations were reported during a routine inspection: laundry detergent container is unlabeled; dead insects found on the premises; single-service items are stored unprotected from contamination; light on hood is burned out; line to spray hose near washing machine is leaking; tile wall in storeroom has a hole; ceiling tiles are not smooth and easily cleanable; dish room floor, restroom ceiling vent and mop room floor are dirty.
Douglas Wilder Middle School, 6900 Wilkinson Road – The following violations were reported during a routine inspection: observed food stains on green painted walls under serving line and on ceiling/wall of dry storage area; light bulb in ventilation system is burned out.
Pemberton Elementary School, 1400 Pemberton Road – The following violations were reported during a routine inspection: there is less than 150 ppm quat sanitizer in cleaning solution; vent in restroom is dusty.

Hungary Creek Middle School, 4909 Francistown Road – The following violations were reported during a routine inspection: temperatures of three salads in small reach-in cooler on the service line are elevated; single cutting boards are heavily scratched; observed dust buildup on portable fans and on fan system grates in walk-in cooler.
Johnson Elementary School, 5600 Bethlehem Road – The following violations were reported during a routine inspection: stored food is not protected from contamination; kitchen equipment in box incorrectly stored directly on the floor; toilet room doors are being kept open.
