McDonald’s, 9751 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide proof that employees have received training on the employee health policy; pink buildup found on ice deflector inside ice machine; food debris found on sheet pan; observed old date label stickers on a few containers in walk-in cooler and on clean dish shelf; fryer cleaner and grill cleaner incorrectly stored on shelf with clean equipment; stirring spoons incorrectly stored in standing water; some pans and food storage containers are stacked while still wet on shelf in dish area; temperature of small refrigerator at front beverage station is a little high; mop sink basin is backed up with water.

Panera Bread, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: observed buildup on bakery screens; can opener holder and support and rolling racks in bakery are dirty; observed dirt and mildew on spray sink faucet and areas around three-compartment sink faucet; there is a leak from drain line at three-compartment sink; several areas are dirty (mop sink, wall at mop sink, floor by CO2 tank, wall by bag-in-a-box); there is mildew along three-compartment sink caulking.

US Best Wings, 3716 Mechanicsville Turnpike – The following violations were reported during a routine inspection: a mop bucket is blocking the hand sink; women’s restroom doesn’t have paper towels; raw chicken incorrectly stored over seafood; raw beef incorrectly stored over produce and noodles; temperature of rice in rice cooker is too low; temperatures of raw chicken and cooked wings in walk-in cooler are elevated; walk-in cooler fan has heavy ice buildup and is leaking water; temperature of three pans of cooked pasta made last night are elevated because of improper cooling methods; wire racks in walk-in cooler are rusty; facility doesn’t have sanitizer test strips; gaskets of reach-in freezer are dirty.

JULY 12

Plaza Azteca Mexican Restaurant, 10099 Brook Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; raw chicken incorrectly stored next to raw beef; there is 0 ppm sanitizer in the dish machine; temperature of cheese in hot holding unit is too low; temperatures of pico, cooked veggies, lettuce, wings and salsa are elevated; observed large flies near cook line and dish area; racks in walk-in cooler are rusty; walk-in cooler curtains are ripped; there is standing water in units; hose at prep station is leaking; there is low grouting throughout; floor and coving tiles are missing/cracked; a few ceiling tiles are missing.

Intermission Beer Company, LLC, 10089-A Brook Road – The following violation was reported during a routine inspection: there is 0 ppm sanitizer in dish machine.

JULY 13

China House, 4703 Nine Mile Road – The following violations were reported during a routine inspection: several foods are incorrectly stored (raw chicken next to cooked shrimp, raw chicken above produce, shelled eggs above produce, produce in raw chicken box); temperature of fried chicken wings is elevated; items in lowboy unit have elevated temperatures; container of egg rolls in walk-in cooler is not date marked; facility needs a thermometer for both hot and cold temperatures; bulk containers of flour and rice are unlabeled; egg cartons cannot be reused; walk-in cooler racks are rusty; fry baskets are in disrepair; observed grease buildup on sides of fryers and wok station, on refrigerator shelves and on walk-in fan grates; there are multiple cracked, stained and/or missing ceiling tiles throughout the facility.

Starbucks Coffee Company, 11716 West Broad Street – The following violations were reported during a routine inspection: plastic containers are stacked while still wet in dry storage area; floor and drain under three-compartment sink are dirty.

Nineveh Bakery & Special Food, 8026-C West Broad Street – The following violations were reported during a routine inspection: temperatures of hummus and tomato in the prep unit are elevated; small prep unit up front is not holding foods at the proper temperature; single cutting boards are scratched and stained; observed flour and food debris buildup on fryers, grill, bread makers/mixers, wire storage racks, inside reach-in cooler and on floors and walls.

Jus Crab Bar & Grill LLC, 2204 East Laburnum Avenue – The following violations were reported during a routine inspection: clean dishes found stored in front hand sink (hand sinks are for handwashing only); several foods are incorrectly stored (raw burgers over French fries, raw chicken over seafood, shell eggs over produce); can opener blade is dirty; lowboy reach-in cooler and walk-in cooler door have torn gaskets; three fry baskets are in disrepair; wire racks in both walk-in coolers are rusty; facility couldn’t provide sanitizer test strips; hand sink in front service area is not draining water properly; floor tile in dry storage area is cracked; observed food stains on walls near flat top grill; walk-in cooler fan guards are dusty; observed dried food debris on walk-in cooler door.

Halal Fried Chicken & Gyro, 2601-B Tuckernuck Drive – The following violations were reported during a routine inspection: facility couldn’t provide certified food protection manager certificate; person in charge was unaware of the “big six” reportable foodborne illnesses and the associated symptoms; observed dried food debris on dicer; two chemical spray bottles and a jar of white powder are unlabeled; bulk containers of food are unlabeled; a single-service cup and container incorrectly used as scoops in bulk food; mop sink doesn’t have a backflow prevention device installed.

Rose Marie Inn, 8923 Patterson Avenue – The following violations were reported during a routine inspection: hand sink in kitchen doesn’t have hand soap or paper towels; silver equipment found in bar hand sink (hand sinks are for handwashing only); there is 0 ppm sanitizer in three-compartment sink; menu is missing disclosure for items that can be served undercooked; observed mold/mildew buildup inside prep unit; observed buildup of grease and debris on sides of cooking equipment; restrooms are missing covered trash cans.

Chen’s Chinese Restaurant, 11361 Nuckols Road – The following violations were reported during a routine inspection: observed an employee wipe face and nose and not wash hands before continuing to handle food; an employee is wearing bracelets; temperatures of tofu and cabbage in small prep unit are elevated; this unit is not maintaining foods at the correct temperature; observed improper cooling methods for chicken, noodles and wings (covered and in deep containers); a personal soda can is incorrectly stored inside the ice machine that is used for beverages; employees are using soiled wiping cloths on hands and food contact surfaces; prep unit cutting boards are heavily scratched and stained; observed grease and food debris on all wire storage racks, inside reach-in coolers and freezers, on fan system in walk-in cooler, bulk flour bins, fryers, prep units, floors, walls and ceiling panels.

Lulabelle’s Café & Catering, 2012 Staples Mill Road – The following violations were reported during a routine inspection: person in charge couldn’t provide certified food protection manager certificate; there is food debris inside the microwave; bulk containers of white powdery substances are unlabeled; multiple cutting boards in the kitchen are scratched and stained; dried food debris found on exterior of microwave and on lids of bulk containers; observed mold on wall near three-compartment sink; ceiling tiles throughout kitchen are stained, warped and/or broken.

JULY 17

Yen Ching, 9031-35 West Broad Street – The following violations were reported during a routine inspection: person in charge is not ensuring that employees are properly trained in food safety, proper handwashing procedures, preventing cross contamination and bare hand contact with ready-to-eat foods, proper cold holding temperatures, and cooling methods; facility couldn’t provide certified food protection manager certificate; observed employees fail to wash hands after handling raw meat and before continuing food prep; observed an employee rinse their hands at the prep sink instead of hand sink and they didn’t use soap; observed an employee handling wontons bare-handed; some cans in storage room are dented; temperatures of noodles, garlic in oil and pork in prep units are elevated; containers of eggrolls, chicken and noodles in walk-in cooler are not date marked; observed improper cooling methods for chicken and noodles (cooled at room temperature in deep bowls); prep and single cutting boards are heavily scratched and stained; observed grease and food debris on wire racks, rolling carts, prep shelves, walk-in cooler fan grates, dishwash racks and inside reach-in coolers; observed food debris and grime on floor under wok, inside walk-in freezer and in storage room.
