Nick’s Roman Terrace, 8051-A West Broad Street – The following violations were reported during a routine inspection: mouse droppings found in the back prep/storage areas; prep unit cutting board is heavily stained and scratched; observed grease and food debris on sides of fryers and on shelves; portable fans are dusty; floors in the back prep/storage area are no longer smooth and easily cleanable.

McCormack’s Big Whisky Grill, 1420 North Parham Road – The following violations were reported during a routine inspection: toilet paper, tape and tickets found in hand sink near dish area; wings and ribs delivered from another restaurant are not at the proper temperature (take temperatures upon arrival and refuse food found in the danger zone); ice machine interior has a lot of black mildew; pimento cheese is past its expiration date; a disclosure is missing from menus; sprayers are unlabeled; clean utensils are not stored inverted to prevent contamination; lights are burned out in the hood; several areas are dirty and need to be cleaned (fryer cabinet, drip trays on the cook line, ice machine, door track and interior of oven); there is no cold water at the hand sink.

Burger King, 3720 Mechanicsville Turnpike – The following violations were reported during a routine inspection: black mold found inside ice machine; box of tomatoes incorrectly stored directly on floor of walk-in cooler; observed grease and food debris buildup on top of fry station hoods and inside ventilation system; monitors throughout kitchen are dusty and greasy; iced coffee cooler gaskets are moldy and dirty; hood filters over the broiler are dirty; outdoor dumpsters do not have doors; lights are out by the ice machine; floor tile by front end is cracked; walls throughout kitchen have food debris; observed mold/mildew along walls of three-compartment sink; observed food debris and dust buildup on walk-in cooler walls, ceilings and fan guards.

Garden Café at Lewis Ginter Botanical Garden, 1800 Lakeside Avenue – The following violations were reported during a routine inspection: observed debris on slicer and on plates stored clean; temperature of brisket made yesterday in the walk-in is elevated; temperatures of sandwiches in one-door reach-in are elevated; did not see written procedure for items held on time not temperature (salad bar, mac and cheese and pork barbecue); cakes from a different facility are being packaged at the café without proper labels; multiple lights are out under the hood and throughout the kitchen.

Muse Paintbar, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit/diarrhea; neither restroom has a handwashing sign; chemical sprayer is unlabeled; facility doesn’t have a metal stemmed thermometer; pretzel salt is unlabeled; a “no smoking” sign isn’t posted.

JULY 7

Shake Shack, 5400 West Broad Street – The following violations were reported during a routine inspection: temperature of chicken breast prepped this morning in the walk-in cooler is elevated because of improper cooling methods (pans are tightly sealed and double stacked); lids and side doors to dumpsters are open; observed an excessive amount of trash in the dumpster area.

Marco’s Pizza, 2284 John Rolfe Parkway – The following violations were reported during a routine inspection: temperatures of tomatoes, fajita chicken, steak and ham in the top of the pizza make table are elevated; observed small flies at several sinks; cutting board at sandwich unit and individual cutting boards are heavily scratched and discolored; many plastic lids in-use and in storage are cracked; mop sink doesn’t have a dual check valve installed; spray hose at three-compartment sink is broken and it hangs below the flood rim of the sink; wet mop is not hung to air dry; observed dust on ceiling around vents and lights and on wall at pizza station; mop sink is soiled.

Arby’s—Tuckernuck Square, 9080 West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit/diarrhea; temperatures of turkey, gyro meat and brisket are elevated; cooked chicken is past its expiration date; shelf on side of counter, freezer area and behind lowboy around condensor are dirty; floor around water heater and pipes are dirty.

Jersey Mike’s Subs, 11321-A Nuckols Road – The following violation was reported during a routine inspection: sanitizer concentration in two buckets is at 0 ppm.

Domino’s Pizza, 5526 Lakeside Avenue – The following violations were reported during a routine inspection: there is 0 ppm sanitizer in sanibucket; clean containers are stacked while still wet; there is food splatter on walls; under equipment needs cleaning; floor tiles are cracked; coving tiles are pulling away from the wall; observed low grouting near pizza station.

JULY 10

Little Caesars, 5101 South Laburnum Avenue – The following violations were reported during a routine inspection: there is no certified food protection manager on site; facility couldn’t provide a verifiable employee health policy; facility doesn’t have written procedures for cleaning up vomit/diarrhea; a soda bottle for an employee incorrectly stored in pizza prep area; two hand sinks are blocked; hand sink in dough area doesn’t have hand soap; front hand sink doesn’t have paper towels; observed an employee making dough with gloved hands and then use a cell phone with the same gloves and resumed handling pizza dough; prepped pizzas are on a time control policy but timers aren’t being used; baking soda and parmesan cheese are unlabeled; racks next to pizza prep are greasy and dirty; risers in and outside walk-in cooler are dirty; observed heavy crumbs in the pizza oven and food debris in two hand sinks; toilet in employee restroom is extremely dirty; several areas are dirty (door to employee restroom, floor in employee restroom, floor under three-compartment sink, walls throughout kitchen and floor under dough machine).

KFC, 3800 Mechanicsville Turnpike – The following violations were reported during a routine inspection: breading station hand sink doesn’t have paper towels; drink machine spouts and the underside of dispensers are in need of disassembly and cleaning; fry baskets are in disrepair; wire racks in dry storage area are rusty; observed dried food debris and grime/grease buildup on speed racks and storage racks in raw chicken cooler and racks of hot holding units; observed mold on French fry freezer gaskets and wire racks in walk-in cooler; floor drains throughout kitchen need cleaning; observed dried food debris and grime buildup on floors, walls and ceiling of chicken cooler; there is flour on walls and floors behind breading area; ceiling vents are dusty; there is mold/mildew on walls in mop sink area.

Jason’s Deli, 1700 Willow Lawn Drive – The following violations were reported during a routine inspection: dishes aren’t being sanitized in dish machine (appears to be a leak in the dispensing of chlorine sanitizer); temperature of potato salad in walk-in is elevated because of improper cooling methods (covered in a deep Lexan pan); cold holding units on both service lines are not holding foods at the proper temperature (temperatures of salami, melon, ham, turkey, chicken salad, chicken, lettuce and fresh mozzarella are elevated); temperature of cheesecake in dessert case is elevated; coffee/tea filters are not covered to prevent contamination; there is rust on bottom shelf under the drain board; lights in the hood aren’t working; one rolling oven is not working; dish racks are deteriorating; can opener holder is dirty; observed black mildew on hand sink in kitchen; observed food splashes under front line top shelves; baking pans are carbonized; observed debris buildup in crumb area to Turbo Chef rolling oven; there is trash on the floor around the water heater and on walk-in freezer floor.

Chili’s Grill & Bar, 9950 Brook Road – The following violations were reported during a routine inspection: observed mold buildup on sides of ice machine and on soda nozzle at bar; observed food buildup on containers and tops stored clean; temperatures of onions and peppers, chicken and corn in hot holding units are too low; temperatures of fatty bacon and marinara sitting out on prep unit and raw chicken on rolling racks unattended are elevated; observed multiple large flies on cook line; an employee is prepping/cooking food while wearing multiple bracelets; containers on clean drying rack are stacked while still wet; cooking pans and sheets are black and encrusted with black debris (employees don’t know the original color of pans); equipment needs cleaning (all racks in units, rolling racks, gaskets, door handles of units, rolling carts, drain boards of dish machine and sides of cooking equipment); hose needs a backflow device installed; coving tiles are coming off the wall, especially in walk-in; observed low grouting throughout kitchen; wall is drooping by hand sink; several areas need cleaning (coving tile area, under cooking equipment, dish area, all drains and walls and ceilings throughout).

Buffalo Wild Wings Grill & Bar, 1090 Virginia Center Parkway – The following violations were reported during a routine inspection: sanibuckets incorrectly stored in hand sink by dish area; mold buildup found on shoot of ice machine; both bar dish machines have 10 ppm chlorine sanitizer; cutting boards on prep unit are heavily stained; temperatures of lettuce, tomatoes and pico de gallo in prep unit are elevated; observed buildup in units on cook line and on sides of equipment; old food debris found inside shelves for sauces; old sauce buildup found on pump containers.
