Sedona Taphouse, 5312 Wyndham Forest Drive – The following violations were reported during a routine inspection: hand sink near dish machine doesn’t have paper towels; temperatures of pasta, raw chicken and chicken strips in reach-in cooler are elevated; insect sticky tape shouldn’t be installed above prep tables and food; forks on server prep station are not stored inverted; monitors and cables on service line are dusty; observed food debris and grime on wire storage racks in walk-in cooler.

Ginza Spot, 3402 Pump Road – The following violations were reported during a routine inspection: uncovered employee drink found on back prep table; shumai dumplings are double panned and stacked above the air curtain to cool; hand sanitizer incorrectly stored next to poke toppings; bean sprouts are soaked in hot water and then put into the make table to cool (it is suggested to keep sprouts raw/cold and let them soak in soup broths when ordered); bulk bins of white powders and bottles of oil are unlabeled; many food containers incorrectly stored directly on the freezer floor; rice scoops incorrectly stored in standing water; to-go containers on top expo shelf are not stored inverted or covered; FRP finish at three-compartment sink is peeling.

Les Crepes, 1308 Gaskins Road – The following violations were reported during a routine inspection: observed an employee drinking water from an open cup on the cook line; employee drinks should be stored in a separate, designated area; employee threw away crepes in trashcan and didn’t wash hands or change gloves before handling food; two hand sinks are missing paper towels; strainer found inside hand sink (hand sinks are for handwashing only); neither employee restroom or hand sink in dish room have hot water; facility couldn’t provide written agreement about salmon being farm raised and pellet fed or parasite destruction documentation; dish machines in dish room and dry storage room have 0 ppm chlorine sanitizer; observed mold on plastic panel of ice machine in dry storage room; temperature of crepes made last night in walk-in cooler are elevated because they were not cooled properly; temperatures of spinach, spring mix, cheese, salmon and raw chicken in rightmost sandwich unit are elevated; air temperature of this unit is elevated; temperature of milk in lowboy near bar area is elevated; incorrect date marked on garlic and oil; cheese prepped four days ago has no date mark; wine boxes, water bottles and boxes of to-go lids and cups incorrectly stored directly on floor of dry storage room; observed an employee’s apron come into contact with a crepe on the cook line; ice scoop handle is stored touching ice in ice bin; scoops above sandwich unit are not stored inverted; reach-in freezer near cook line has a torn gasket; racks in dish room are rusty; facility couldn’t provide chlorine test strips; observed debris on hood filters on cook line, inside hand sink in dry storage room, in bottom of beer lowboy units at bar, on racks throughout facility, outside of equipment, on shelf in kitchen, on top of dish machine in dish room, outside of fruit squeezer at bar and on outlets on cook line; fan guard in walk-in cooler is dusty; observed debris/buildup on floors at bar and in dry storage room and on floors and walls in dish room; ceiling and walls of cook line are stained; there is rust on wall under dish machine; light shield above dish machine is cracked.

Marco’s Pizza, 7000 Forest Avenue – The following violations were reported during a routine inspection: temperature of spinach in make table is elevated; no date marks found on two containers of cooked wings; spray hose attachment and three-compartment sink faucet are dirty; wall under mop sink and hot water faucet have mildew.

Little Caesars, 4007 Mechanicsville Turnpike – The following violations were reported during a routine inspection: observed food debris on can opener blade, pizza sauce machine and on various pieces of equipment in prep area; observed dried food debris/grime/dust/mold on speed racks throughout kitchen, pizza oven, walk-in cooler (fan guards, racks and walls), wire racks near prep table, near pizza oven, in prep and dish areas, on gaskets of Pepsi beverage cooler, prep table, exterior and interior of freezer and on walk-in cooler door; observed dried food debris/grime buildup/mold on walls (especially along prep line and three-compartment sink), floors throughout kitchen and drain of three-compartment sink; ceilings especially near ventilation are dusty.

Brasil Steakhouse LLC, 9035 Quioccasin Road – The following violations were reported during a routine inspection: person in charge couldn’t verify that workers have been told about employee health information; eggs incorrectly stored over cheese; raw pork incorrectly stored over cabbage; lower metal shelves are rusty; door to women’s restroom is not self-closing.

Anant Cuisine of India, 4348 Pouncey Tract Road – The following violations were reported during a routine inspection: employees at this facility have continuously failed to demonstrate knowledge and abilities to operate a food establishment; certified food protection manager credentials are expired; two open drinks belonging to employees found; observed an employee handling and cutting ready-to-serve naan with bare hands; observed uncovered foods in a make table that are totally overrun with roaches; dishes stored clean are still discolored from soil and/or had food debris on them; can opener and ice machine interior are dirty; soda gun nozzles at the bar have buildup; temperature of rice in warmer is too low; temperatures of onion tomato sauce, korma sauce, vegetable roll, tikka masala sauce and raitha are elevated; no foods prepared today have been date marked; tube of caulk and a can of spray paint incorrectly stored in a bin of spice bottles; all foods in the walk-in are stored in large, deep containers (improper cooling method); facility has a roach infestation that is readily visible in one cooler on the line (kitchen is dirty with a lot of exposed food and cardboard available to harbor pests); many food containers incorrectly stored directly on the walk-in freezer floor; a soiled terry cloth towel was found wrapping a ball of dough; a very large, dirty, chipped and cracked plastic utility bin is being used for onion storage in walk-in cooler; ice scoop handle at the bar is stored touching ice; cutting boards on the line and in storage are very discolored and/or scratched; fridges along the cook line have repeatedly had problems with maintaining foods at the proper temperature; rice cooker latch handle appears to be broken; hood filters are greasy; practically all non-food contact surfaces in the kitchen are dirty (soda carbonators, sides and surfaces of cook line equipment, work tables, fridge gaskets, insides of fridges, storage racks, expo shelving, serving tray stands, storage bins, fan covers in walk-in, sink handles, top of dishwasher, electrical lines, etc.); floors, walls and ceilings throughout the kitchen (including inside walk-ins) have soil, debris and food residue buildup; ceiling vents are dusty; floor drains are grimy.

Starbucks Coffee Company, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: interior of ice machine is dirty; temperature of jug of whipping cream inside reach-in is elevated; one reach-in unit is not working; cabinets up front, especially under blender station, have soil; floor drain under blender station is dirty; there is broken cove molding by the ice machine.

Chick-fil-A, 10176 Brook Road – The following violation was reported during a routine inspection: quat sanitizer level in three-compartment sink is too high.

Davinci, 4933 Nine Mile Road – The following violations were reported during a routine inspection: hand sink by three-compartment sink is missing hand soap and paper towels; a hand sink is blocked by deep freezer; dishes found in hand sink and an employee washed a pan in a hand sink (hand sinks are for handwashing only); temperatures of chicken wings and gyro meat in lowboy unit are elevated; this unit needs servicing; pizza slice in front counter warming unit is not at the correct temperature and should be placed on time control; two cans of Raid found in the kitchen (pesticide cannot be stored or used in the kitchen); Windex and drain cleaner incorrectly stored next to food; temperatures of lettuce, tomato and marinara are elevated because of improper cooling methods; bulk containers of rice, sugar and salt are unlabeled; observed fruit flies throughout the kitchen; lowboy unit is not maintaining the correct temperature; wire racks throughout kitchen are rusted; observed grease buildup on hood filters, in between equipment, under fryers, under grill on cook line, on racks and in bottom of unused reach-in unit; there is grease on the floor of the cook line; mop sink doesn’t have a dual check valve installed; walls throughout kitchen and in dish area are stained with food debris.

Melito’s, 8815 Three Chopt Road – The following violations were reported during a routine inspection: dish machine is not reaching the proper sanitization temperature; can opener is dirty; salmon was not cooked to the required temperature; mushroom gravy, mashed potatoes and green beans are not date marked; menu is missing a disclosure for burgers and fish; spray bottle of cleaner is unlabeled; cornmeal is unlabeled; food is incorrectly stored directly on the walk-in freezer floor; wet wiping cloth is sitting out on the counter instead of being stored in a sanitizer bucket; coffee filters are stored unprotected; liquor shelf is bare wood; shelving under slicer table is in poor repair; several items are dirty (hand sink, interior of keg box, can opener holder, spray hose attachment, drip trays to stove); spray sink has a leak; cove strip under freezer walk-in threshold is torn; several areas are soiled (exterior of walk-in freezer door, walk-in floor, ceiling panels in kitchen); there is rust on walk-in refrigerator wall; caulking by three-compartment sink and walk-in ceiling are mildewy.

Vallarta Mexican Grill Mariscos Bar, 5431 Glenside Drive – The following violations were reported during a routine inspection: observed a food prep employee chewing gum in the kitchen; an employee is wearing jewelry on their arms while preparing food; thawed raw tuna incorrectly stored in reduced oxygen packaging in reach-in cooler; two fryer baskets are bent and have wire pieces broken off.
