Dunkin’ Donuts, 10390 Ridgefield Parkway – The following violations were reported during a routine inspection: person in charge could not describe the “big six” reportable foodborne illnesses and the associated symptoms or locate verification that employees have received training on this; temperatures of whipped cream and almond milk are elevated because they were left out on the counter; handle of ice scoop is touching the ice; stirrers incorrectly stored in ambient temperature standing water; mop sink doesn’t have a dual check valve installed; cracked floor tile and eroding grout with pooling water observed by mop and three-compartment sinks and ice caddy.

Chanello’s Pizza, 10166 West Broad Street – The following violations were reported during a routine inspection: several pizza pans are bent; some containers and lids are cracked; observed flour and food debris buildup on wire racks, fan grates in walk-in cooler and pipes connected to grease trap; the condensate waste pipe from the walk-in cooler is improperly draining into the three-compartment sink.

Domino’s Pizza, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: cornmeal is unlabeled; grate is missing from mop sink drain; three-compartment sink faucet is dripping.

Bell Greek, 7011 Three Chopt Road – The following violations were reported during a routine inspection: person in charge is unable to verify that workers have been informed of employee health information; hand sink in back is blocked; the low-temp dish machine isn’t sanitizing; temperatures of hummus and grape leaves in make table are elevated; no date marks found on desserts; temperature of a full pan of lasagna in the freezer is elevated because it is covered in plastic wrap and not cooled properly; simple syrup incorrectly stored below hand towel dispenser; single-service items incorrectly stored directly on the floor; there is a hole in the men’s restroom ceiling.

Burger King, 8081 Brook Road – The following violations were reported during a routine inspection: hand sink by office doesn’t have paper towels; paper towel dispenser is jammed at hand sink by three-compartment sink; time is not being tracked for lettuce and tomatoes at second prep station and underneath prep surface; there is 0 ppm sanitizer in sanibucket; observed grease buildup in between equipment on cook line and on legs of prep tables; observed old food buildup in units and on gaskets; observed grime buildup on rolling racks and hot holding racks.

Boil Bay Cajun Seafood & Bar, 8161 Brook Road – The following violations were reported during a routine inspection: observed an employee eating while prepping vegetables; open drink found on rack in walk-in; front hand sink doesn’t have paper towels; tags were removed from oysters on the half shell when placed in different containers to thaw; observed today’s date being placed on all items while the inspector was there; eggs are being stored on the line without a written time or policy in place; sleeping aids, vitamins and other supplements incorrectly stored above prep units; oysters incorrectly thawing in containers of standing water; single-service items incorrectly stored directly on the floor in back storage area.

Desi Bites, 3621-C Cox Road – The following violations were reported during a routine inspection: hand sink near front entrance door doesn’t have hand soap; whisk found in hand sink near kitchen door entrance (hand sinks are for handwashing only); observed debris on can opener blade on prep table; there is 0 ppm chlorine at three-compartment sink in dish area; food debris found inside blender, on multiple knives in prep area and on multiple utensils stored clean on rack; temperatures of mashed potatoes, multiple containers of vegetable donuts, rice, curry, potatoes and lentil batter are elevated because of improper cooling methods; temperature of rice in rice warmer is too low; temperatures of paneer, cream cheese and milk in walk-in cooler are elevated; multiple powders in back dry storage area are unlabeled; multiple oil boxes and food containers are incorrectly stored directly on the floor in dry storage area; pans are stacked while still wet on clean rack in dish room; to-go containers are stored uninverted on cook line; walk-in cooler has a torn gasket; cutting boards throughout kitchen are heavily scratched; observed debris/grease/buildup on hood filters, in between equipment, on light bulb covers, under grill on cook line, on racks and shelves throughout, in between gaskets of sandwich units and outside of grinder; sticker residue found on multiple containers in dish area; there are debris and stains on walls on cook line, especially behind equipment; ceiling tiles in prep/tea station are stained; observed debris on floors and walls in dish area and in mop sink/dry storage room; there is low grouting on floors throughout the kitchen.

Hibachi Grill Buffet, 4338 South Laburnum Avenue – The following violations were reported during a routine inspection: an employee failed to wash hands after rubbing his nose and before continuing to cut vegetables; several cans are badly dented at the seams; person in charge couldn’t provide parasite destruction letters from salmon provider; facility has not been keeping shellstock tags for 90 days and not recording the last date sold of each batch; chemical dish machine isn’t sanitizing properly; temperature of sushi rice is too low; sushi rolls on the buffet are not being held at the correct temperature and are not on time control; menu doesn’t have a consumer advisory and disclosure for salmon sushi rolls that may be served raw and/or undercooked; buffet card says flounder but swai fish is being served; several fryer baskets are in disrepair.

Chum Chum Onigri, 9073 Staples Mill Road – The following violations were reported during a routine inspection: old date marking labels are not being removed or changed once the item is taken from the freezer to thaw; observed grease stain on wall behind oven; discontinue use of the convection oven; facility doesn’t have a hood system.

Bryan Park Bar & Grill, 5516 Lakeside Avenue – The following violations were reported during a routine inspection: temperatures of onions and peppers on the grill are too low; fries and sweet potato fries on rolling rack need to be placed on time control or kept cold; menu doesn’t have a disclosure for items cooked to order; cutting board on small prep surface is heavily stained; observed grease and food buildup under and around equipment on cook line.

Topgolf Richmond, 2308 Westwood Avenue – The following violations were reported during a routine inspection: a staff member touched mask with gloved hands and returned to work without washing hands and changing gloves; dirty knife stored on clean knife strip; there is food debris on the meat slicer; temperature of queso dip in warmer is too low; mesh wire strainers and fry basket are in disrepair; cast iron pans in main kitchen are rusted; cutting boards throughout kitchen are scratched and stained; observed food debris and grease buildup on surfaces throughout kitchen (shelves underneath prep tables, expo heated shelves, hood filters at main kitchen fry stations); wire racks in walk-in coolers are moldy; observed grease and food debris along floor and walls of hot line in prep and main kitchens, on ceiling tiles in main kitchen and in main kitchen walk-in cooler.

Akoya Kitchen, 10921-F West Broad Street – The following violations were reported during a routine inspection: temperature of fried rice on the steam table is too low; unsalted butter incorrectly stored at room temperature next to the grill.

MAY 8

JJ’s Grille, 10298 Staples Mill Road – The following violations were reported during a routine inspection: no supervisors or managers are certified food protection managers; temperatures of tuna and coleslaw in middle prep unit are elevated; chemicals incorrectly stored on cart with food; prep unit is not maintaining foods at the proper temperature and water is pooling inside it; observed grease and food buildup under grill, on wheels of fryers, dry rack, air curtains in walk-in cooler and on counter surface storing clean plates.

Antonio’s Pizza, 63 South Airport Drive – The following violations were reported during a routine inspection: meat slicer and floor mixer have food debris; to-go boxes and single-use items are not stored inverted under prep table; racks throughout kitchen are rusty, as well as walk-in cooler and prep area storage shelves; observed food debris on walk-in cooler racks, aluminum lining and non-aluminum liner, can opener base, reach-in cooler gaskets and shelving inside reach-in cooler; observed grease buildup and food debris along floors, walls and ceilings of walk-in cooler, hot line and pizza oven area.

Trak’s Gyro & Burgers, 9115 Quioccasin Road – The following violations were reported during a routine inspection: shell eggs incorrectly stored over cheese; slicer has food debris; temperature of cheese sauce is too low; temperature of feta inside True unit is elevated; there is no date mark on open box of gyro meat or prepared pasta; remove paper lining tray where clean glassware is stored; lower shelf under flat top and cart beside fryer are dirty; freezer interior is dirty and has ice buildup; dish room floor and floor behind ice machine need cleaning; ceiling fan in prep/passage area is dusty; wall panel by entry to kitchen is loose.
