APRIL 18

Wendy’s, 2200 East Laburnum Avenue – The following violations were reported during a routine inspection: cups found in front hand sink (hand sinks are for handwashing only); temperature of diced chicken in warmer drawer at drive-thru is too low; temperature of vanilla frosty mix in the dispenser is elevated; observed heavy buildup of grease/dust/food debris on fryers, wire storage racks, condiment bins, lemonade dispenser, cup holders at drive-thru, monitors and cables, and fan grate in walk-in cooler; paint is peeling on all the hood systems; floor in the outdoor walk-in cooler/storage unit is heavily rusted and in disrepair; observed grease and food debris on floors, walls, hood systems and filters; ceiling panels are heavily stained and dusty.

Arby’s, 11298 Patterson Avenue – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit/diarrhea; can opener blade has food debris on it; temperature of rotisserie chickens is too low; thigh meat is undercooked; fried onion straws are past their use-by time; observed several large roaches, both dead and alive, in kitchen area; hood filter over oven is in disrepair; there is low grouting on floors; freezer door doesn’t close tightly; caulking at hand sink is starting to peel away.

Charles’ Kitchen, 10835 West Broad Street – The following violations were reported during a routine inspection: Barz Moss seamoss products were not able to be verified as an inspected and approved business today; open pack of raw bacon incorrectly stored on top shelf in reach-in cooler; hand can opener has buildup around the blade; dishes stored on drying rack have debris on them; temperature of curry goat cooling for over two hours is elevated; temperature of chicken patties in hot case is too low; cakes in the grab ‘n go fridge have no labeling on them; utensils on prep table incorrectly stored in standing water; single-use plastic food containers are being reused to store prepared foods; ceiling vent over expo line is dusty; one light shield is missing in the kitchen.

APRIL 19

Olive Garden Italian Restaurant, 11829 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t verify that employees have received training on illness reporting requirements; observed two employees eating at the cook line and dish pit; handwash signage is missing at two hand sinks; multiple knives are stored dirty; temperature of bag of marinara sauce is too low; temperature of lettuce in salad unit is elevated; this unit appears to be running high and the inside of the door is damaged; monitoring records for April 7 are incomplete; an employee with long facial hair is not wearing a beard restraint; observed tomatoes taken directly from cardboard produce box and sliced without washing first; to-go lids are stored uninverted; facility couldn’t provide test strips for quat sanitizer; there isn’t an irreversible temperature registering device for hot water sanitizing at the dish machine; two lights are out in the hood system; more lighting is needed on the cook line.

Habanero Mexican Grill, 9101-A Quioccasin Road – The following violations were reported during a routine inspection: person in charge couldn’t verify that employees have received training on illness reporting requirements; facility doesn’t have written procedures for cleaning up vomit/diarrhea; open drinks found in the kitchen; an employee used a hand sink to rinse out cloths (hand sinks are for handwashing only); observed live roaches on front counter and prep table; front refrigerator is leaking water.

Pizza Hut, 3113 Mechanicsville Turnpike – The following violations were reported during a routine inspection: food bits found inside hand sink next to fryers (hand sinks are for handwashing only); chlorine level is too low at chemical dish machine; observed food debris greater than four hours old on pizza cutters and wooden pizza pan; temperature of pasta prepped within the past hour is elevated because of improper cooling methods; metal pans are stacked while still wet on clean shelf; two light bulbs are out where pizza convection oven is located; there is ice accumulation on pipes of walk-in freezer; observed debris/grease/buildup on pipes behind fryers, wheels under fryers, racks throughout, hood filters at fry station and on top of dish machine; walk-in cooler fan guards are dusty; there is soot on bottoms of pizza pans stored clean; observed debris inside of container where dough pan lids are placed; walls in dry storage area have chipped paint; floors in dry storage area are in disrepair; there is low grouting on floor under dish machine; floor tiles in front of sandwich unit are cracked; ceiling lights aren’t working in dish area; ceiling vents and surrounding ceiling tiles are dusty; mop sink is dirty; there are stains and debris on walls near dish machine; walk-in freezer floor has debris.

McDonald’s, 8975 Staples Mill Road – The following violations were reported during a routine inspection: fryer baskets are in disrepair; observed grease and food debris on wire racks near three-compartment sink, inside reach-in freezer, on wheels of fryers, on fan grates in walk-in cooler and in cup holders; observed trash and grime on floor under drive-thru beverage machine.

APRIL 21

Dave & Buster’s, 4001 Brownstone Boulevard – The following violations were reported during a routine inspection: no sanitizer detected at the bar dish machine; pink slime found inside the ice machine; slicer has food debris on it; temperature of fries under heat lamp is too low; food temperatures (cajun butter, alfredo, tomatoes) are elevated in the top of the refrigerator unit next to the steamer; utensils at the grill incorrectly stored in standing water; handle of ice scoop is in contact with ice at the bar; glasses stored in contact with water at the bottom of the refrigerator unit at the bar; cutting boards are heavily scratched; observed mold/food debris on racks in the walk-in refrigerator; floor and floor drains at dish machine and bar are soiled.

Maple Street Biscuit Company, 11657 West Broad Street – The following violations were reported during a follow-up inspection: clean dishes on the drying rack still have food debris on them; observed dried batter splatters on the metal panel behind the cook line; floors under soda bags and at the back door are soiled; hood filters are greasy and/or have flour buildup.

APRIL 24

Papa John’s Pizza, 4744 South Laburnum Avenue – The following violations were reported during a routine inspection: front hand sink doesn’t have hand soap; clean aprons incorrectly stored in the restroom.

Wendy’s, 5212 Brook Road – The following violations were reported during a routine inspection: hand sink in back kitchen doesn’t have hand soap; hand sink by cashier doesn’t have paper towels; temperatures of shredded cheese and other sliced cheese in make top are elevated; no times are marked for lettuce and tomatoes in make table; freezer has heavy ice buildup; freezer and small one-door freezer have torn gaskets; front make table is in disrepair; observed grease and dirt buildup on sides of equipment and inside units, racks and gaskets; walk-in fans are dusty.

Dunkin Donuts, 7300 Staples Mill Road – No violations reported during a routine inspection.

Chipotle Mexican Grill, 8955 Staples Mill Road – The following violations were reported during a routine inspection: thicker pieces of chicken coming off the grill are not cooked to the proper temperature; dish machine and sanitizer dispenser at three-compartment sink aren’t working; observed mildew buildup on wall near three-compartment sink.

Mama Cucina, 4028-O Cox Road – The following violations were reported during a follow-up inspection: observed buildup on sides of reach-in cooler; paint is worn and there are some holes on kitchen walls; ceiling tiles and their lining are dark and discolored from age.
