Kabuto Japanese House of Steaks, 8052 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide certified food protection manager certificate; open drink found on dry storage rack above clean plates; observed heavy mildew buildup inside ice machine; dish machine isn’t reaching the required surface temperature on the plate simulator after running four times; menu doesn’t contain a consumer advisory for ribeye, filet mignon and tuna that may be served raw and/or undercooked; boxes of food incorrectly stored directly on walk-in freezer floor; rice scoop incorrectly stored in standing water; wire storage racks in walk-in cooler are heavily rusted; cutting boards are heavily scratched and stained brown; facility couldn’t provide thermometer to test hot water dish machine; observed dust and grease buildup on wire storage racks, prep counters, rolling carts, fryers and utensil bins; there are leaks under three-compartment sink and hand sink in women’s restroom; floor tiles in dry storage room are broken; observed food debris and grease on floors throughout kitchen and inside walk-in coolers.

International House of Pancakes, 9820 West Broad Street – The following violations were reported during a routine inspection: raw chicken incorrectly stored next to shredded cheese and on top of pancake batter in walk-in cooler; temperature of cheese sauce in steam well is too low; temperatures of hashbrowns in lowboy and turkey bacon and mac and cheese in prep top unit are elevated; incorrect discard times found on liquid and shell eggs on cook line; improper cooling methods observed for bacon (cooling at room temperature with saran wrap); metal pans are stacked while still wet on clean rack; cutting boards throughout kitchen are heavily scratched; observed debris/grease/buildup on racks and shelves throughout, on floors, walls and ceilings throughout, under fryers, in between equipment, under grills, outside of waffle makers/equipment, in between gaskets and on light bulb covers on the cook line; mop sink has standing water in back of house; no water is coming out of left hand sink in men’s restroom; faucet at mop sink in back kitchen is leaking; ceiling vents in kitchen are dusty; there is low grouting on floors throughout kitchen; floor tile on cook line is missing causing standing water; ceiling lights in dry storage room are not in working order.

Pop’s Dogs & Ma’s Burgers, 7301 Brook Road – The following violations were reported during a routine inspection: observed a container and food debris in the hand sink (hand sinks are for handwashing only); temperatures of chili and cabbage in small one-door reach-in are elevated; there is mold buildup on racks and inside of small reach-in; observed grease buildup around fryers, grill and prep table; observed food and grease buildup under equipment.

Panda Express, 4501 South Laburnum Avenue – The following violations were reported during a routine inspection: temperature of orange chicken on the hot holding front line is too low; salt on the cook line is unlabeled.

Moe’s Southwest Grill, 10839 West Broad Street – The following violations were reported during a routine inspection: green wire dry storage cage and two black portable fans are dusty; mop sink doesn’t have a dual check valve installed; spray hose on the three-compartment sink hangs below the flood rim of the sink; the walk-in cooler floor is dirty.

Plaza Azteca Mexican Restaurant, 12020 West Broad Street – The following violations were reported during a routine inspection: pans of raw chicken incorrectly stored above beef in the walk-in; observed heavy dark buildup on soda gun nozzle at the bar; can opener is dirty; in-use dishwasher is not dispensing any chlorine; temperatures of chicken wings, chicken strips, French fries, salsa and other foods in the line’s large make table are elevated because the fridge wasn’t plugged in; a can of Raid was found in the dish area (Raid cannot be used in a food establishment); raw fish incorrectly thawing in reduced oxygen packaging in the walk-in; dry storage rack is rusty; gaskets on the line lowboy and reach-in freezer are torn; fan guard in the walk-in is very dusty; food processing equipment on prep table has buildup; top of expo shelving is greasy; cove tiles in the dish area are broken.

Buz and Ned’s Real Barbecue, 8205 West Broad Street – The following violations were reported during a routine inspection: no certified food protection manager certificate on site; objects found in both hand sinks at bar area (hand sinks are for handwashing only); hot water doesn’t reach the required temperature at two hand sinks; multiple dented cans found on dry storage rack; raw shell eggs incorrectly stored above corn in walk-in cooler; there is 0 ppm chlorine at dish machine; temperatures of raw shrimp and bacon are elevated because they were left out on the front line; temperatures of chicken wings and corn in reach-in unit are elevated; air temperature of this unit is elevated; quat sanitizer level at three-compartment sink is too high; didn’t observe times on packaged foods in the walk-in freezer; food incorrectly stored directly on floor of walk-in freezer; utensils incorrectly stored in standing water on front line; containers are stacked while still wet on rack in dish room; there are torn gaskets throughout all refrigeration units; there is a gap greater than one inch between hood filters on cook line; light shield is missing over fry station; one refrigeration unit on front line is not in working condition; cutting boards throughout kitchen are heavily scratched and/or stained; all walk-in cooler fan guards, hood filters in prep area and surfaces in between equipment in kitchen are dusty; observed low grouting throughout kitchen; side wall in dish room has holes and is deteriorating.

Subway, 3135 Mechanicsville Turnpike – The following violations were reported during a routine inspection: observed an employee chewing gum while making sandwiches; observed an employee rinsing a cutting knife in the hand sink (hand sinks are for handwashing only); observed a food prep employee wearing bracelets; wire storage rack and water heater are heavily rusted.

Sheetz, 8711 Quioccasin Road – The following violations were reported during a routine inspection: sanitizer level is too low in three-compartment sink; temperature of white rice/black bean mix in the make table is elevated.

Bojangles, 5207 Brook Road – The following violations were reported during a routine inspection: two hand sinks don’t have paper towels; observed dried food debris on scoops and utensils on clean dish rack; observed bags of raw chicken and mac and cheese incorrectly sitting in warm water to thaw; several areas need cleaning (inside of cold holding and hot holding units, sides and tops of fryers, grill area and handles of units); observed grease buildup on hood filters.

APRIL 17

Cracker Barrel, 3620 Mayland Court – The following violations were reported during a routine inspection: cook line hand sink doesn’t have paper towels; observed food debris on bowls and plates stored clean and on two ladles and one brush hanging above chicken breading station; temperature of pancake batter on front line and in walk-in cooler is elevated because proper cooling methods weren’t followed; temperature of roast beef on hot line is too low; temperatures of shredded cheese and roast beef in prep top unit and meat loaf in lowboy unit are elevated; did not observe time stamps for raw shell eggs left out on the cook line; improper cooling methods observed for tomatoes, pot roast and macaroni; air temperature of lowboy unit on cook line is elevated; observed debris/buildup/grease on hood filters on cook line, in between gaskets at service station, on shelves and racks throughout, on bottoms of lowboy freezer units, under grill, and in between equipment on cook line and prep areas; observed buildup inside some microwaves on cook line; walk-in cooler fan guards and ceiling vents in kitchen are dusty; there is low grouting on floors throughout the facility; there is mold on walls in dish area; observed debris/grease/buildup/stains on floors and walls throughout facility, especially behind and under equipment on cook line and in prep areas.

Rollerdome, 4902 Williamsburg Road – No violations reported during a routine inspection.

Two Guys Pizza, 11636 West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; temperature of tomatoes in the pizza unit is elevated because they are overfilled; some lexan pans on the dish racks are broken, cracked and/or chipped; hand sink handles are grimy; front counter below point-of-sale is dirty; speed rack next to the oven and surfaces/crevices of the large mixer have buildup; drying racks above the three-compartment sink have a coating of film (dark residue came off when wiped with an alcohol swab); walk-in racks, walls and pipes/wires running along the walls have a thicky fluffy coating that resembles mold; walk-in fan guard is soiled; floors under equipment and along wall corners are very dark from heavy soil buildup; walls in the pizza prep area and near salad unit are dirty and worn; back door is dirty; paint is beginning to peel from ceiling tiles over the front counter; hood filters are dusty.

Chipotle Mexican Grill, 7000 Forest Avenue – No violations reported during a routine inspection.
