APRIL 5

El Nopal II Inc., 5205 South Laburnum Avenue – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; facility doesn’t have a verifiable employee health policy; facility doesn’t have written procedures for cleaning up vomit/diarrhea; observed a layer of pink slime buildup inside ice machine; temperature of white sauce on ice is elevated; several foods made yesterday (cooked potatoes, chili rellenos, tamales and refried beans) are not date marked; back door was left open; screen door has large holes big enough for pests to enter through; chip bin, taco salad tortillas and tostados are uncovered on the cook line; two buckets of food incorrectly stored directly on the walk-in cooler floor; racks in dry storage are rusty; wall is damaged to the right of the dish machine; water is pooling on the floor near the ice machine; floors behind the cook line and under equipment are greasy and dirty; wall behind hand sink is dirty; wet mops are not being hung to air dry.

Mexico Restaurant, 11621 West Broad Street – No violations reported during a follow-up inspection.

APRIL 6

Ciao Capri, 10478 Ridgefield Parkway – The following violations were reported during a routine inspection: no sanitizer was detected at kitchen or bar dish machine; tomatoes and mozzarella are not date marked; observed heavy grease and dust buildup on sides of fryer and cook line equipment; there is grease and food debris on cook line shelving; observed food residue on walls at cook line and on floor under fryer; ceiling vents are dusty.

New Asian Panda, 10430 Ridgefield Parkway – The following violations were reported during a routine inspection: observed an employee handle cooked pork with bare hands; temperatures of egg rolls and chicken wings in silver reach-in are elevated; General Tso chicken has been cooling for 1.5 hours on the counter and the temperature is still elevated; boxes of fortune cookies and boxes of takeout containers incorrectly stored directly on the floor; observed food debris on cart and shelves; walk-in fan grates are dusty.

Fat Boy’s Bar + Grill, 2 West Williamsburg Road – The following violation was reported during a follow-up inspection: rack near dish machine is rusty.

Papa John’s Pizza, 9853 Brook Road – The following violations were reported during a routine inspection: employee restroom doesn’t have a handwash sign posted; temperatures of tomatoes, steak and cheese in top part of pizza prep unit are elevated; small reach-in and two-door reach-in have torn gaskets; small reach-in is not maintaining foods at the proper temperature; observed dirt and food debris in multiple containers holding clean utensils; racks in walk-in have buildup.

Jimmy John’s, 4420 South Laburnum Avenue – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit/diarrhea; quat sanitizer level is at 0 ppm at three-compartment sink; one prep unit is not working properly (facility isn’t keeping food in it); facility couldn’t provide test strips for sanitizer.

APRIL 7

Hardee’s, 6302 Lakeside Avenue – The following violations were reported during a routine inspection: observed small flies in the mop sink area; there is ice buildup in walk-in and reach-in freezers; exterior of ice machine, other units and some racks are rusty; several areas need cleaning (walk-in racks, sides of cooking equipment, outside of units and shelf holding lines for soda); trash can in women’s restroom doesn’t have a lid; observed grease, dirt and old food under equipment; observed grime and dust buildup on floors, walls and ceilings of the entire facility; there is low grouting in the dish area; floor tiles and grouting throughout kitchen are cracked; hand sink is pulling away from the wall causing it to drain slow; some ceiling tiles in the back storage area are missing.

APRIL 10

Pizza Hut, 5100 Williamsburg Road – The following violations were reported during a routine inspection: temperatures of diced tomatoes and pasta are elevated; walk-in cooler’s ambient temperature is elevated; observed food debris and a leak inside lowboy; rolling racks are greasy; outside of walk-in freezer door is greasy and dirty; floor between pizza prep and dough cooler and floor outside freezer are dirty.

Greek Cuisine, 11214 Patterson Avenue – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; styrofoam takeout containers are stored unprotected on hand sink basin; observed person in charge place dirty spoon in hand sink (hand sinks are for handwashing only); there is slime buildup in ice chute at soda station; observed bits of gyro meat on clean dishes on shelves; person in charge states that knife and gyro shaving utensil are washed and sanitized every evening (this must be done every four hours); temperature of stuffed grape leaves in refrigerator is elevated because they are stored in a deep covered container; observed mouse droppings in dry storage/mop sink room; boxes of cucumbers, tomatoes and a pot of hot soup incorrectly stored directly on floor under three-compartment sink; gyro cooker is not turning; observed grease and food residue on shelves in food prep area; hand sink is very slow to drain; mop is not hung to air dry; ceiling vents are dusty; mop sink is soiled.

Fresk Wok Restaurant, 5146 Nine Mile Road – The following violations were reported during a routine inspection: observed person in charge grabbing a handful of salad greens without gloves; dirty dishes found in both hand sinks (hand sinks are for handwashing only); facility couldn’t provide parasite destruction letter from the supplier; raw salmon and dumpling filling incorrectly stored over ready-to-eat foods; drink machine spouts and can opener blade and base are dirty; temperature of cooked chicken wings is too low; sushi rice is being held at the incorrect temperature; temperatures of food in the reach-in cooler are elevated; noodles, egg rolls and raw meats are not date marked; dine-in menu doesn’t have a consumer advisory for raw or undercooked foods (takeout menu does); fish is incorrectly thawing in reduced oxygen packaging; several knives incorrectly stored between prep table and cooler on hot line; several deep fryer baskets are in disrepair; wire racks in walk-in cooler are covered in food debris, mold and rust; mesh shelf liners shouldn’t be used as a liner for tempura crumbles; lights are out in the hood system; observed grease buildup and food debris on floors and walls.

APRIL 11

Boychik’s Deli, 4024-B Cox Road – The following violations were reported during a routine inspection: observed two open drinks stored on equipment; deli counter hand sink had been used to store a bucket (hand sinks are for handwashing only); raw philly steak incorrectly stored next to chicken salad in walk-in; can opener is dirty; observed debris on clean dishes; temperature of lettuce in make table is elevated because it is overfilled; liquid eggs and frozen French fries were left out on counters; sliced deli meats and wet salads are incorrectly covered while cooling; one live roach found on floor behind deli case; buckets of food incorrectly stored directly on walk-in cooler floor; knife incorrectly stored in crevice of make table; several areas are in need of cleaning (interior and exterior of coolers and cook line equipment, expo shelving, soda shelving across from three-compartment sink, speed racks, waffle iron wire rack, reach-in cooler fan guards, and wire rack over three-compartment sink); walls throughout kitchen need cleaning, especially at the cook line; floors under equipment and the corners of the walk-in cooler have dark buildup; ceiling vents are dusty, rusted and worn; many floor tiles in the kitchen are cracked; corner guard on the wall near the dish area is cracked and chipping; some ceiling tiles are dark from age; hood system is greasy.

Bella Luna, 1212 Concord Avenue – The following violations were reported during a routine inspection: temperatures of cheese in homestyle fridge and milk and sauce in one-door reach-in are elevated; box of medicine incorrectly stored with bread; bottle of stomach relief medicine incorrectly stored on front counter with drinks; multiple units have torn gaskets that are repaired with tape; racks in multiple units are rusty; observed heavy ice buildup in freezers.

Bistro 804, 443 North Ridge Road – The following violations were reported during a routine inspection: no handwash sign is posted at hand sink downstairs; sanitizer level in three-compartment sink is too low; raw chicken was not cooked to the proper temperature; temperatures of salmon, crab and whipped butter in left make table are elevated; temperature of corned beef is elevated; soap in a squeeze bottle is unlabeled; breadings, spices and dusting flours are uncovered; dirty wet wiping cloth found on counter instead of being stored in sanitizer between uses; a light is out above the three-compartment sink downstairs; lower shelves downstairs are dirty; mop sink faucet doesn’t have a dual check valve installed; floor behind chargrill is greasy; observed food debris on electrical outlets; wall downstairs by three-compartment sink has holes; wall at bottom of stairs is in poor repair.

Domino’s Pizza, 11210 Patterson Avenue – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; facility doesn’t have written procedures for cleaning up vomit/diarrhea; temperatures of sliced cheeses in top of make table are elevated; water is leaking from a pipe under the three-compartment sink; observed moldy/missing caulking at three-compartment sink and wall.
