Sheetz, 7035 West Broad Street – The following violations were reported during a routine inspection: observed food debris on four scoops stored clean on front line; there is 0 ppm quaternary ammonium sanitizer at three-compartment sink; temperature of cut lettuce in prep top unit is elevated because it is stored above the air curtain; drinks and a box of water bottles incorrectly stored directly on the floor; wooden cutting boards on front line are heavily scratched; reach-in unit at coffee station on retail side is not in working condition; fan guards in both walk-in coolers and ceiling vents in kitchen are dusty; there is grease under fryers on cook line and in between gaskets of lowboy unit; observed buildup/liquid under soda machine on retail side; cold water pressure is low at hand sink near back kitchen and office; observed debris/grease/buildup on walls behind equipment on cook line; there is buildup on floor under ice machine in kitchen; there is low grouting on floors under both hand sinks on front line.

Portabella’s Restaurant, 2627 New Market Road – The following violations were reported during a follow-up inspection: facility doesn’t have a verifiable employee health policy; facility doesn’t have written procedures for cleaning up vomit/diarrhea; flour and parmesan cheese on the cook line are unlabeled; to-go containers are not stored inverted; there is a leak at the three-compartment sink; ceiling tiles in kitchen are stained.

Ironclad Coffee Roasters, Richmond International Airport, Concourse A and B – No violations reported at either location during a routine inspection.

True Taste 1, 1090 Virginia Center Parkway – The following violations were reported during a routine inspection: observed an employee go into the restroom and then re-enter the kitchen to continue food prep without washing hands; raw meat incorrectly stored over egg rolls in refrigerator; raw meat incorrectly stored next to cabbage and cooked pork in walk-in; observed mold-like buildup in ice machine; cooked noodles, pork and eggs do not have date marks; noodles, cut cabbage and chicken are on a time control policy but no times were written down; pack of cigarettes incorrectly stored on shelf over hot holding units; orange peels and an open can of mushrooms incorrectly stored with nails/tools and other hardware; frozen spare ribs are improperly thawing in a bag on the shelf; several non-descript white substances on the cook line are unlabeled; knives incorrectly stored in between prep surfaces; single-service items incorrectly stored directly on dining room floor; plastic single-use grocery bags should not be used to store food in reach-in and walk-in; several items/surfaces need cleaning (inside and outside of units, racks, small hanging fans, top part of rice hot holding units, underside of main cooking station, walls across from fryers, under cooking equipment, under three-compartment sink and hood filters).

Mellow Mushroom, 12171 West Broad Street – The following violations were reported during a routine inspection: in-use dishwasher is dispensing 0 ppm chlorine; observed dishes on the drying rack with old date labels, food particles and greasy residue; garlic oil was left out at room temperature; temperature of food inside Delfield reach-in cooler is elevated; this unit is not functioning properly; improper cooling methods observed for cooked beef in walk-in; observed small flies in kitchen and at bar; sauce brushes on rack have very frayed ends; cutting board at hoagie station and colored boards at prep station are heavily scratched; dishwasher is coated in grime; drying rack has some residue buildup; observed food particles and dust on blender base; mop sink doesn’t have a dual check valve installed; ceiling vents in kitchen are dusty, especially near GF station; wall at three-compartment sink is dirty; dish hood opening is coated in a black slimy substance.

Pizza Hut, 11385 Nuckols Road – The following violations were reported during a routine inspection: observed dust and grease residue on utensils and containers stored on dry rack; observed dried food debris and grease buildup on surfaces of wire storage racks and carts, hood system and fan grates in walk-in coolers; ceiling panel/vents and wall near fryers are dusty; food debris found on floors under fryers and sauce station.

West Coast Provisions, 301 Maltby Boulevard – The following violations were reported during a routine inspection: open cup of water found on back dish rack; observed old tape and some food particles on clean dishes; one soda gun nozzle at the bar has heavy buildup; unsalted herb butter has been held at room temperature for 24 hours; wasabi and yuzu aioli in the walk-in are past their expiration dates; sushi rice is on time control but there is no written procedure for this; many ingredient squeeze bottles are unlabeled; ice scoop at bar is incorrectly stored with handle touching the ice; cutting boards are heavily worn; dish rack and walk-in rack are rusty; one gasket on the line is torn; two lights are out under the hood; fridge doors, dishwasher flaps, storage lunch box in sushi area and back plate of soda dispenser are dirty; utensil storage bins have debris in them; top of expo shelving has a greasy film; dark buildup found on walk-in fan guards; rack in wine cooler at bar is grimy; wall in dish area is mildewy; floor under cook line has grease deposits; floors under tables/racks are darkened from soil; hood filters are dark and greasy.

China King Chinese Restaurant, 4362 South Laburnum Avenue – The following violations were reported during a follow-up inspection: facility doesn’t have a certified food protection manager; back reach-in freezer has torn gasket and broken handle; ceiling tiles are stained.

Chiki’s Pancake, 1404 North Parham Road – The following violations were reported during a routine inspection: no date mark on banana pudding; time is not being tracked for pancake batter; chlorine sanitizer level is too high in sink and in wiping cloth bucket.

Melody’s Sugar Shack at Glen Allen Softball Complex, 2203 Mountain Road – The following violations were reported during a routine inspection: racks in reach-in units are rusted; freezer has a torn gasket.

Melody’s Sugar Shack at RF&P Park, 3600 Mountain Road – The following violation was reported during a routine inspection: BBQ, chili and cheese from last night incorrectly stored in deep containers.

APRIL 5

Revel, 6223 Lakeside Avenue – The following violations were reported during a routine inspection: open drink found over prep table; bar hand sink and cook line hand sink don’t have paper towels; hand sink by dish machine and bar hand sink don’t have hand soap; tags for mussels incorrectly removed from container before sold; raw meat incorrectly stored over ready-to-eat food in lowboy; raw shell eggs incorrectly stored over veggies and cake incorrectly stored with seafood in walk-in; chicken is being par-cooked but written procedures have not been submitted to the health department; temperatures of rabbit, spinach and pickled items in walk-in are elevated; temperature of prawns in prep top is elevated because they are overstacked; temperature of unsalted butter is elevated because it was left out by cook line; menu doesn’t have a disclosure that beef tartar and tuna can be served raw/undercooked; observed raw chicken being vacuum sealed without a HACCP plan; packaged items in the market do not list ingredients or net weight; spoons incorrectly stored in sanitizer on cook line; spoons and knives incorrectly stored in water at bar; ice scoop handle is touching ice; oyster shells are being cleaned and reused on site.

Umi Sushi Restaurant, 11645 West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; plastic storage containers have old stickers on them; temperature of raw tuna in make table is elevated; ingredient bottles are unlabeled; damp cloths incorrectly stored out on counter instead of in sanitizer; walk-in racks are rusty; two lights are out under the hood; several equipment surfaces have grease and other residue buildup (sides of oven, knife holder, sushi sliding doors and shelving at dry storage); cook line floor is greasy; floor drains have dark buildup.

Roy’s Big Burger, 5200 Lakeside Avenue – The following violations were reported during a routine inspection: person in charge states there is no written employee health policy; facility doesn’t have written procedures for cleaning up vomit/diarrhea; shell eggs incorrectly stored with raw onions in reach-in cooler; onions over grill are at an improper temperature; cardboard boxes should not be used as shelf lining in drink refrigerator; observed grease and dust buildup on fry basket and heat lamps; floor tile is cracked; flooring in dry storage area/under soda machine is unsealed.

Jersey Mike’s Sub Shop, 11444 West Broad Street – The following violations were reported during a routine inspection: did not observe written procedures for cleaning up vomit; front hand sink is blocked by boxes; clean dishes have a greasy feeling residue on them; temperatures of lettuce and tomatoes on the line and many items in the deli case are elevated; tuna salad in the walk-in isn’t date marked; foods wrapped in plastic are cooling in the walk-in; backup stock of lettuce and tomatoes behind the line are piled high and placed directly into the make top to cool; prep unit handle is broken off; walk-in freezer and cooler fan guards are very dusty; surfaces and crevices of the deli case have debris; sides of the flat top are greasy; wire rack in the walk-in cooler is dirty; floors and walls throughout kitchen are grimy, especially at the oven and and dish area walls; ceiling tiles are discolored; mop is not hung to air dry.
