Chuy’s, 11229 West Broad Street – The following violations were reported during a routine inspection: hand sink in prep area is blocked and items were found stored in it (hand sinks are for handwashing only); front server station hand sink doesn’t have hand soap; observed dried food debris on containers and utensils stored clean; parts of chicken breast were below the required temperature once taken off the grill; spray bottles of sanitizer at the bar are unlabeled; observed a tube of ant pesticide stored with tubes of lubricant for the drink machines (tubes were of similar color and size); chicken and burritos improperly covered during cooling process; observed a food prep worker wearing a dangling bracelet during prep; concentration in sanitizer buckets is below the required amount; raw strawberries were not washed thoroughly in water to remove soil and other contaminants before being cut and served to a customer; single-service containers are stored on dirty prep surfaces unprotected; containers are cracked; utensils are in disrepair; all the prep cutting boards are heavily scratched and stained; observed dried food debris and dust buildup on portable fans, fan grates in walk-in cooler, wire storage racks, oil filter machine, storage and counters; observed grease buildup on wheels of fryers; observed food debris buildup on walls and on floors under three-compartment sink and fryers; ceiling panels/vents in the dishwash area are dusty.

Lehja Restaurant, 11800 West Broad Street – The following violations were reported during a routine inspection: three dented cans found in dry storage; raw fish incorrectly stored above open sauce containers and ginger in the walk-in; observed food debris on dishes on expo shelving and in lowboy freezer; there is heavy mold buildup in the ice machine; can opener and blade are dirty; temperature of chocolate samosas prepared last night in the walk-in is elevated; temperature of paneer in the make table is elevated because it is overfilled; onion sauce is sitting out on the stove top but no burner is on; temperature of chicken curry in make table is elevated; cake slices opened two days ago are not date marked; three containers of sauces are improperly cooling in large, deep containers while covered in the walk-in; dry goods are in containers without lids on dry storage rack; damp wiping cloth is sitting out on a cutting board and should be stored in sanitizer between uses; large spoons incorrectly stored in standing water; to-go containers are not stored inverted or covered; small cutting board on cook line is dark and worn; walk-in racks are rusty; one light is out under the hood; ice machine is covered with peeling tape; dish crates are very worn; some plastic food containers in the walk-in are cracked; wire racks throughout the kitchen are dirty; dishwasher hood vent, walk-in fan opening and ice machine air vent are dusty; gaskets, speed racks, bins storing silverware and can rack have food debris on them; exteriors of cook line equipment and fridges have sauce or grease splatters; floor mixer has green buildup; blender bases and spice grinder are heavily soiled; top expo shelves feel gritty and sticky; bar hand sink faucet is leaking; employee restroom and ceiling over mop sink have water damage (peeling paint, bubbled walls); one floor tile is cracked in prep area; kitchen walls have dark buildup or food stains on them; ceiling has splatters on it in some sections; walk-in cooler floor corners are littered with food and dirt; cook line floor drains are grimy; ceiling light over cook line is missing a shield.

Waffle House, 5414 Williamsburg Road – The following violations were reported during a routine inspection: high temp dish machine is not reaching the required temperature; ice machine has a layer of slime and dirt; temperature of sliced American cheese in refrigerated drawer on cook line is elevated; this drawer refrigeration unit is not keeping foods at the proper temperature.

Christian’s Pizza, 7003-A Three Chopt Road – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit/diarrhea; a finished pizza was being handled by an employee with bare hands; interior of ice machine has slime; temperature of pizza sauce made in-house is too low; fresh mozzarella, spinach and lettuce don’t have date marks; cleaner incorrectly stored beside oil; while sauce was being prepared, containers were placed onto the floor and stacked (nothing should be placed on the floor); ice receptacle is unprotected; several surfaces are dirty (flat grill, pizza oven exterior, inside freezer in back, soda carbonator cabinet, floor by ice machine and flat grill, suppression lines, hood filters, floor and floor drain in back, walk-in floor); cold water faucet at hand sink isn’t working; floor drain in back has terrible odor; light shields are missing above pizza prep area.

Awful Arthur’s Seafood Company, 10498 Ridgefield Parkway – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; facility doesn’t have written procedures for cleaning up vomit/diarrhea; bar hand sink doesn’t have paper towels; date of shell stock last served is missing from tags; observed buildup inside soda gun nozzles at the bar; the disclosure on the menu is inadequate (must be disclosed that the menu items with an asterisk may contain raw or undercooked ingredients); tuna and salmon are incorrectly thawing in reduced oxygen packaging; cutting boards are heavily scratched; heavy grease on floor under fryers.

Original Mexican Restaurant, 6406 Horsepen Road – The following violations were reported during a routine inspection: person in charge couldn’t verify that workers have been told about employee health; temperature of queso is too low; canned corn was placed into the make table before cooling to the proper temperature; stock made today was not cooled properly; margarita mixes and sugar at the bar are unlabeled; boxes of chicken incorrectly stored on walk-in floor; container of ice cream in walk-in isn’t covered; extra flat top is dirty; grease barrel found open; storeroom floor corners have buildup.

Halal Fried Chicken & Gyro, 2601 Tuckernuck Drive – The following violations were reported during a routine inspection: hand sink in kitchen doesn’t have paper towels; raw chicken incorrectly stored next to cooked chicken in walk-in cooler; observed food debris inside metal container and on multiple utensils in container next to prep table; temperature of lettuce prepped this morning is elevated because of improper cooling methods; no date marks found on cheese curd and milk dessert in reach-in unit; multiple powders throughout kitchen are unlabeled; food incorrectly stored directly on floor in walk-in freezer and near back exit door; to-go trays on the cook line are not stored inverted; walk-in cooler has a torn gasket; there is ice accumulation along pipes, around door and on floor of walk-in freezer; observed discolored quaternary ammonium sanitizer test strips in kitchen; observed debris/grease/buildup on hood filters, under grill, on racks throughout and in between gaskets on lowboy unit; there is buildup on walls; ceiling vents throughout facility are dusty; paint on walls and ceiling tiles is chipping; light shield near cook line is cracked.

Einstein Brothers Bagels, 3320 Pump Road – The following violations were reported during a routine inspection: temperature of onion and chive shmear is elevated; spray bottles of water are unlabeled; compressor in make table is broken and not maintaining foods at the proper temperature; observed a bucket full of coffee under the dump sink (no active leak observed when water at the sink was on but there is a leak somewhere).

Little Caesars, 6040 Brook Road – The following violations were reported during a routine inspection: the certified food protection manager certificate has expired; containers on dry storage rack are dirty and sticky; no date marks found on nacho cheese or sausage; didn’t observe a marked time on pizzas left out on rack; observed buildup of dirt and grime inside hot holding units, on racks next to pizza oven, on rolling racks for pizza and on clean dish rack; caulk is moldy around three-compartment sink; coving tiles and floor tiles under three-compartment sink are heavily stained and peeling away.

Potbelly Sandwich Shop, 10921-E West Broad Street – The following violations were reported during a routine inspection: open drink without lid found on back prep table; front sanibuckets do not have enough quat sanitizer; cutting boards at make tables are heavily scratched; knife holder on wall has brown stains on it; observed buildup on fridge handles, some gaskets and other exterior and interior surfaces; area behind the ice chute at the soda dispenser has black buildup; floors under equipment are dark and have food particles; walls behind prep tables have sauce spills; both hood systems are very dusty.

Rise Biscuits & Donuts, 11561 West Broad Street – The following violations were reported during a routine inspection: open employee drink without lid found on mixer table; cleaning supplies found in the dish area hand sink (hand sinks are for handwashing only); raw shell eggs and a bottle of beaten eggs were stored out at room temperature on the cook line; cans of Raid found on chemical storage shelf (Raid cannot be used in a food establishment); facility only has quat test strips available but the sanitizer being used is Sink and Surface; observed dark buildup on walk-in cooler fan guard; bottom tier of walk-in cooler rack has a lot of flour on it; gasket on lowboy freezer appears to be moldy; pink residue is built up behind the ice chute of the soda machine; biscuit warmers have darked, caked up residues along the inner drawers; walls around the cook line corner hand sink and at the mop sink are mildewy; floors under fryers have grease deposits; floor drain at the three-compartment sink is grimy; back hood filters have a coating of flour on them.

Amanecer Latino Market, 8410 Staples Mill Road – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit/diarrhea; observed dried food debris on blade of can opener; pan of chicken in water bath warmer is at the incorrect temperature; temperature of a container of slaw in reach-in cooler is elevated; salsas and cheese/bean mix are incorrectly covered and stacked during the cooling process; prep unit cutting board is heavily scratched and stained; observed dried food debris and grease on counters, inside reach-in coolers, on prep unit, on walk-in cooler fan system, inside utensil bins, in crates and under grill; concrete floor in dry storage isn’t sealed or painted; walls in back room have holes; wall in dry storage is damaged; observed food debris and grime on walk-in cooler floor and under and behind equipment.

Subway, 9127 Staples Mill Road – The following violations were reported during a routine inspection: person in charge couldn’t provide written policy or cleanup kit for vomiting and diarrheal events; open boxes of condiments incorrectly stored under hand sink; observed an employee wearing bracelets while preparing food; prep unit cutting boards are heavily scratched; light in the walk-in cooler isn’t bright enough.
