California Pizza Kitchen, 11800 West Broad Street – The following violations were reported during a routine inspection: open employee drink found on cook line shelf; dishes on the drying rack have some debris and/or old stickers on them; storage containers have food debris in the bottoms; observed dark, moldy buildup inside ice machine and food particles inside Electrolux; temperature of tomato salsa on the line is elevated; salmon incorrectly thawing in reduced oxygen packaging in the walk-in; several surfaces are rusty (lowboy racks, walk-in cooler racks, soda drain rack and can opener); walk-in freezer door is not closing tight; colored cutting boards are heavily worn; Thermo labels for the high temp dish machine expired in 2019; shelving along the line feels sticky and has debris on it; sides of cooking equipment have areas of buildup; walk-in cooler fan guards are dusty; walk-in doors and some fridge gaskets have food debris inside crevices; grout throughout back kitchen is low; floor tiles are broken/missing at the walk-in freezer door; ceiling tiles in the dish area are discolored; floors under equipment on the line are soiled; walls in dish and prep areas are dirty; large metal grated floor drains are dirty.

Portabella’s Restaurant, 2627 New Market Road – The following violations were reported during a routine inspection: facility doesn’t have a verifiable employee health policy; facility doesn’t have written procedures for cleaning up vomit/diarrhea; raw eggs incorrectly stored over ready-to-eat sausage in walk-in cooler; hard-boiled eggs, sliced turkey, sliced ham, sliced tomatoes and cooked spaghetti in prep unit and reach-in cooler do not have date marks; lasagna is cooling uncovered in the walk-in cooler; flour and parmesan cheese on cook line are unlabeled; plastic containers are stacked while still wet; to-go containers are not stored inverted; there is a leak at the three-compartment sink; trash receptacle in back restroom doesn’t have a cover; ceiling tiles in kitchen are stained.

Noodles & Company, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: employee handled cooked chicken without a utensil or wearing gloves; cold rail wasn’t turned on so temperatures of spinach and broth are elevated; chemical spray bottle is unlabeled; no residual found inside wiping cloth bucket; there is rust on lower shelf beside hand sink; dish racks have begun to deteriorate; pasta baskets have mineral buildup; faucet at spray sink, pipes under sinks and red bucket protecting drain are dirty; floor around oven/flat top and rails under both hot wells are dirty; there is no light above the mop area.

Outback Steakhouse, 7917 West Broad Street – The following violations were reported during a routine inspection: hand sink in dish room doesn’t have paper towels; raw shrimp incorrectly stored over bacon bits in lowboy unit; sanitizer is at an improper level at mop sink; temperature of cooked mushrooms in steam unit is too low; temperatures of shredded cheese, raw shrimp and ribs are elevated; chili is incorrectly cooling in deep containers in the walk-in freezer; scoops are stored in water near wait station in kitchen; containers are stacked while still wet on rack in prep area; cutting boards throughout kitchen are heavily scratched; facility couldn’t provide high temperature test strips; observed buildup/debris on hood filters, under grill, on light bulb covers on cook line, on shelves throughout kitchen, and in soda gun holders at bar; paint is chipping on walls throughout kitchen, especially in dish room and prep area; observed low grouting on floor in walk-in cooler; there is buildup on walls behind equipment on cook line and behind fryers at fry station; some ceiling tiles are stained in prep area; there is litter on floor at bar area; ceiling vents in mop sink area are dusty; walls in dish room are moldy.

Mission BBQ Richmond, 5440 Glenside Drive – The following violations were reported during a routine inspection: several pans of green beans and collards are incorrectly covered and stacked while cooling in the walk-in cooler; observed food debris and dust on wire storage racks, racks in warmers, and on counter and storage surfaces; cables and monitor on service line are dusty; old labels/tape are still on clean containers; tile coving behind floor mixer is damaged; observed dust buildup on ceiling vents in dishwash area.

Tropical Smoothie Café, 4501 South Laburnum Avenue – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit/diarrhea; ice buckets in storage are not stored inverted; several ceiling vents in the kitchen are dusty.

Wingstop, 4501 South Laburnum Avenue – The following violations were reported during a routine inspection: vent outside ice machine is dusty; mop sink doesn’t have a dual check valve installed; floor in walk-in cooler has food debris greater than one day old and liquid pooling.

Starbucks Coffee Company, 11800 West Broad Street – The following violations were reported during a routine inspection: front hand sink doesn’t have paper towels; observed moldy buildup inside ice machine; gaskets in nitro lowboy and back two-door reach-in are torn; facility doesn’t have thermo labels or thermo disc available for the dishwasher.

Burger King, 4240 Pouncey Tract Road – The following violations were reported during a routine inspection: ice machine has buildup in interior corner; back of ice machine door turned an alcohol swab brown when wiped; squeeze bottle of water is unlabeled; single-service boxes are not stored inverted; walk-in rack on bottom tier is beginning to rust; equipment surfaces throughout kitchen have buildup and/or crumbs (warmers, freezer gasket, sides of fryers, dry storage racks, walk-in fan guards and bread rolling carts); floor under fryers has greae and food debris buildup; floors under storage racks and ice machine are darkened from soil.

Skrimp Shack, 4760 Finlay Street – The following violations were reported during a routine inspection: spray bottle of cleaner is unlabeled; container of white chemical powder for grease is not labeled with the common name; bulk container of sugar and a container of powdered sugar are unlabeled; the handle of the scoop in the bulk sugar container is stored in the product; two lights above three-compartment sink and one light under the hood are out.

China King Chinese Restaurant, 4362 South Laburnum Avenue – The following violations were reported during a routine inspection: no certified food protection manager is onsite; facility doesn’t have written procedures for cleaning up vomit/diarrhea; microwave is dirty with food debris; temperatures of pork, chicken and fried shrimp in the glass door reach-in cooler are elevated; this unit is not holding foods at the proper temperature; fried rice is kept out at room temperature on time control but no times are recorded; three white powders on the cook line are not labeled with the common name; buckets of chicken, broccoli and other foods incorrectly stored directly on the floor; bulk spice containers contained reused single-serve plastic containers as scoops; to-go containers are not stored inverted; reach-in freezer and chest freezer have buildup and torn gaskets; reach-in freezer handle is broken; freezer door in front area formerly used for dining is broken; shelves of glass door reach-in cooler, chest freezer outside walk-in cooler and shelf near the fryer shouldn’t be lined with greasy cardboard; round cutting board is heavily scratched; facility is reusing single-service items; reach-in freezer is dirty; chest freezer in kitchen has mold growing on the inside of the lid; ceiling tiles are stained; back door is dirty.

Popeye’s, 8044 West Broad Street – The following violations were reported during a routine inspection: an employee failed to wash their hands after touching glasses and before engaging in food preparation; paper towel dispenser isn’t working at handwash station near three-compartment sink; observed dried food debris and grease residue on dishes stored clean; temperatures of fried chicken breast and thighs are too low; temperature of bags of macaroni and cheese in hot holding unit is too low; pans of rice cooling in the walk-in cooler are incorrectly double-stacked and covered; observed mold and rust on wire racks in both walk-in coolers; observed dust and flour buildup on surfaces of warming cabinets and wire racks in walk-in coolers and storage area; food debris found inside reach-in coolers; observed buildup of food debris and standing water on floors in walk-in coolers; there is low grouting on kitchen floor; floor tiles under far left fryer are cracked; walls near back door have chipped paint.

Azzurro Restaurant, 6221 River Road – The following violations were reported during a routine inspection: facility couldn’t provide a verifiable employee health policy; facility doesn’t have written procedures for cleaning up vomit/diarrhea; three-door lowboy has elevated air temperature and temperature of foods inside is elevated (pasta, soup, goat cheese); consumer advisory on the menu is not clear as to what may be requested/served undercooked; degreaser is unlabeled; gasket on salad unit door is loose; wood shelves in dish room and storeroom are unsealed; remove linen under bar shakers; facility couldn’t provide heat tapes or irreversible thermometer for dish machine monitoring; exterior of mixer is dirty; faucet at three-compartment sink is loose; wall and floor under/behind booster heater has mildew; floor under fryer and chargrill is dirty.
