Dunkin Donuts, 3125 Lauderdale Drive – The following violations were reported during a routine inspection: squeeze water bottles along the line are unlabeled; observed dark buildup and debris on hash brown freezer gasket; other fridge gaskets also have dark patches of buildup; freezer floor has trash.

The Brass Tap, 4901 Libbie Mill East Boulevard – The following violations were reported during a routine inspection: raw chicken incorrectly stored over ready-to-eat foods; slicer has food particles on it; queso has been reheating for approximately 2-3 hours in the steam table but its temperature is still too low; temperatures of all foods in the grill drawers (steak, pulled pork, sauces, ham, cheese, etc.) are elevated because the unit was off (a breaker had tripped); old date labels were not removed from pans before the pans were refilled; squeeze bottle of water in the kitchen is unlabeled; sanibuckets incorrectly stored directly on the floor.

Plentiful Foods, 1145 Gaskins Road – The following violations were reported during a routine inspection: didn’t observe a consumer advisory statement on menu for roast beef tenderloin cooked medium rare, per manager; cutting boards on shelf in kitchen are heavily scratched; observed slight buildup on hood filters on cook line and on racks in kitchen; observed carpet in dry storage room in back of facility where beers and wine are stored as well as other equipment for catering (floors should be smooth, non-absorbent and easily cleanable).

Mariscos El Barco, 6409 Rigsby Road – The following violations were reported during a routine inspection: no dates found on prepped empanadas; the consumer advisory statement on the new menu is missing disclosure for steaks; cleaner incorrectly stored beside pineapple juice; spices are unlabeled; spice bucket incorrectly stored directly on floor; lower shelf under flat grill and top of grease trap are rusty; there is loose coving under the dish machine.

Mexico Restaurant, 5213 Williamsburg Road – The following violations were reported during a routine inspection: facility couldn’t provide a verifiable employee health policy; facility doesn’t have written procedures for cleaning up vomit/diarrhea; observed food particles in the bottom of the hand sink just outside the kitchen (hand sinks are for handwashing only); raw shrimp incorrectly stored over ready-to-eat vegetables in reach-in cooler; raw chicken incorrectly stored over raw chorizo and steak in drawers under cook top; queso in the steam well is not hot enough; temperatures of salsa and white sauce are elevated because they are sitting out; no date marks found on cooked spinach, tamales and carnitas in lowboy; spray bottle of cleaner incorrectly stored next to wine on a shelf; salt and sugar on the cook line are unlabeled; to-go containers are not stored inverted; facility couldn’t provide test strips for sanitizer; water is pooling near the dish area; floor tiles near ice machine are broken.

New Dragon City, 2220 John Rolfe Parkway – The following violation was reported during a routine inspection: raw shell eggs are stored out on the cook line.

Chicken Fiesta, 8211 Brook Road – The following violations were reported during a routine inspection: open drinks found on prep shelves; raw salmon incorrectly stored over flan in reach-in; raw chicken incorrectly stored with ready-to-eat vegetables in walk-in; raw meat incorrectly stored over cooked chicken and veggies in meat cooler; temperatures of beef and pinto beans made last night are elevated; temperature of smoked ribs is elevated because they were left in the cambro from the night before; no date marks found on shredded chicken, cooked chicken, beef, beans or salsas; chicken improperly thawing at room temperature; observed an employee wiping down poles that held raw chicken and placing cloth over side of open ice bin (needs to be placed in a bucket or in a way to avoid cross contamination); taco shells on top of prep are uncovered; flan in walk-in and shredded chicken in meat walk-in are uncovered; there is 0 ppm sanitizer in wiping cloth bucket; observed multiple wet wiping cloths and visibly soiled wiping cloths hanging from employees’ aprons (wiping cloths should be stored in sanibuckets and not used to wipe hands).

El Torito Mexican Restaurant and Grill, 5600 Williamsburg Road – The following violations were reported during a follow-up inspection: raw fish incorrectly stored over ready-to-eat food in reach-in cooler; raw eggs incorrectly stored over sauce in walk-in cooler.

Sakura Japanese Restaurant, 9008 West Broad Street – The following violations were reported during a routine inspection: employee drinks found stored near food and food contact surfaces; bowl found inside hand sink next to prep sink and food bits found inside hand sink next to employee restroom (hand sinks are for handwashing only); facility couldn’t provide parasite destruction letter for fish used in sushi; didn’t observe written agreement from salmon provider that salmon is farm raised and pellet fed; raw chicken incorrectly stored over bamboo in prep top unit; raw beef incorrectly stored next to pineapple and over a box of potatoes in walk-in cooler; raw eggs incorrectly stored next to box of tomatoes, raw chicken incorrectly stored over zucchini, and raw shrimp incorrectly stored over salad in walk-in cooler; observed food debris greater than one day old on knives and slicer; there is pink mold on plastic panel of ice machine in kitchen; temperatures of cooked rice, unsalted butter, raw shell eggs, cooked broccoli, and raw shrimp are elevated; didn’t observe written times for sushi rice, breading on cook line, tempura breading at sushi station and on tofu stored out at room temperature; temperatures of salad, lettuce and sprouts prepped earlier are elevated because of improper cooling methods; escolar is listed as albacore on the menu; white fish is not specified on the menu; observed foods incorrectly stored directly on the floor throughout the facility, including walk-in cooler and freezer; hibachi carts and tempura breading should not be stored next to hand sinks; observed multiple dirty wiping cloths stored in water on floor under prep sink; multiple utensils and rice scoop incorrectly stored in standing water at wait station; whisks and utensils on cook line shelf are not stored inverted; to-go containers and lids on kitchen shelves and wait station area are not stored inverted; multiple units in kitchen and sushi station have torn gaskets; racks in walk-in cooler are rusty; cutting boards throughout facility are heavily scratched; observed debris inside microwave; observed buildup/debris/grease in between and under equipment, under fryers, on hood filters on cook line and at hibachi station, on hibachi carts, outside of sauce containers, outside of microwave, on racks and shelves throughout facility, and on floors and walls throughout facility; fan guards in walk-in cooler are dusty; observed debris in between gaskets throughout facility, on top of dish machine and on bottom of reach-in freezer; there is not hot or cold water at hand sink in front of employee restroom; faucet handle at prep sink is leaking; ceiling tiles throughout kitchen are stained; ceiling tile is missing above walk-in cooler; observed low grouting on floors throughout facility; light shield is cracked above walk-in cooler; light shield is missing at wait station where boba is prepared; personal items and employee clothes incorrectly stored next to items in dry storage shelf.

Brick Road Coffee Co., 8055 Shrader Road – The following violations were reported during a routine inspection: no supervisors or managers are certified food protection managers; observed in-use utensils used with milk products stored in water that is not at least 135 degrees.

Chick-fil-A—Willow Lawn, 4920 West Broad Street – The following violations were reported during a routine inspection: slicer for chicken has not been switched out with a clean slicer after four hours; the veggie wash is above the recommended range when tested.

Carytown Burgers & Fries Catering and Café, 5404 Lakeside Avenue – The following violations were reported during a routine inspection: person in charge couldn’t verify their food manager certification; person in charge couldn’t provide verification that employees have been trained on their illness reporting requirements; facility doesn’t have written procedures for cleaning up vomit/diarrhea; hand sink doesn’t have a sign reminding employees to wash hands; ice machine has pink slime; can opener blade is soiled; temperatures of chili and queso in the steam table are too low; onion straws made this morning are at an incorrect temperature; spray bottle of cleaner is unlabeled; filter powder in shaker container is unlabeled and uncovered; quat sanitizer level at three-compartment sink is too high; white powder in souffle cup is unlabeled; handles of the walk-in refrigerator and small reach-in freezer are broken; racks in the walk-in refrigerator are soiled with food debris; observed heavy grease on grease receptacle outside; there is food debris on the floors of the walk-in units; observed grease and food debris on floor and walls at cook line.

Hyatt Place Richmond/Innsbrook, 4100 Cox Road – The following violations were reported during a routine inspection: no supervisors or managers are certified food protection managers; front hand sink is blocked by a cooler; hand sink doesn’t have paper towels or hand soap; there isn’t a chemical sanitizing test kit present to ensure the chemical sanitizing solution used is at the proper concentration; observed dried food debris inside reach-in coolers, utensil bins and on surfaces of oven; observed dried food debris and grime on floors, inside floor drains, and under and behind equipment.
