Rusty Taco, 11525 West Broad Street – The following violations were reported during a routine inspection: Brillo pads, tea bags and sanibuckets found in multiple hand sinks (hand sinks are for handwashing only); observed food debris on clean dishes on drying rack; ice machine doors are dirty and the interior is moldy; can opener blade has dark buildup on it; temperature of queso from last night is elevated; foods on the steam table are below the required temperature (appears that the steam table is being used to reheat foods and it was not designed to do this); temperature of grab-n-go salsa cups is elevated; observed small flies at the bar; single-service paper boats are not stored inverted or covered on multiple spots along the line; several areas are dirty (surfaces of cook line equipment, dishwasher, walk-in racks and fan guards, shelf where InstaPot is stored, inside of undercounter freezer and inside some fridges along the line); hood filters over the prep unit are extremely dusty; ceiling around the vent over the steam table area and ceiling vents over the dish area are dusty; splatter residue found on wall at three-compartment sink; doors have black scuffing and dirt buildup on them; observed trash and debris under equipment and on storage racks.

New Top’s China 1, 7111 Staples Mill Road – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit/diarrhea incidents; observed an employee handle raw chicken and then handle cooked chicken without washing hands; bowl of cooked chicken incorrectly stored on top a container of raw chicken in reach-in cooler; raw vegetables are stored in a moldy box in walk-in cooler; a moldy gallon jug is being used to scoop cooked rice; temperatures of noodles, cut cabbage, cooked shrimp and chicken pieces are elevated; no date marks found on eggrolls, chicken and other prepared items held past 24 hours; several cleaning spray bottles are not labeled with the common name of the chemical; container of meat and other food boxes incorrectly stored on floor in walk-in cooler and freezers; to-go containers are not stored inverted; observed broken metal pieces on all fryer baskets; broken containers are being used to store food; wire racks are rusty; prep unit lid is broken; prep cutting board is heavily scratched; soiled cardboard should not be lining prep storage surfaces; boxes incorrectly storing condiments and spices; observed dried food debris buildup and grease on surfaces of prep tables, counters, walk-in cooler shelves, crates storing food, reach-in cooler interior, sauce buckets, fryers and wok station; observed buildup of food debris, grease and grime on floors throughout the kitchen and storage areas; items that are unnecessary to the operation or maintenance of the establishment shouldn’t be stored on the premises; lighting in the walk-in cooler and dry storage area is insufficient (rooms are very dark even with lights on).

Pho Que Huong, 10073 Brook Road – The following violations were reported during a routine inspection: sponge and hose found in dish area hand sink (hand sinks are for handwashing only); dishes on the cook line are dirty; observed buildup on the slicer; temperatures of pho broth, bean sprouts, lettuce, pickled vegetables and egg rolls are elevated; no date marks found on prepared items (cooked meats, pho broth and soups); the walk-in freezer is stored so tightly with food from floor to ceiling that it is hard to add or remove any items; spoons and forks are not stored inverted; single-service items incorrectly stored on floor in room by bar; the walk-in is not holding foods at the proper temperature and one of the fans is in disrepair; small walk-in freezer is not working; drain board for dirty dishes is being held up by small sauce containers; observed heavy ice buildup on walk-in freezer (fans are blowing out ice); the discharge pipe from the ice machine is not in line with the drain causing water to pool on the floor; surfaces under ice machine and along the cook line need cleaning.

Starbucks Coffee Company, 9700 Gayton Road – The following violations were reported during a routine inspection: facility couldn’t provide a certified food protection manager certificate; whisk found in drive-thru hand sink (hand sinks are for handwashing only); hot water at drive-thru hand sink doesn’t reach the required temperature; temperatures of creamer and whipped cream are elevated; temperature of sweet cream is elevated because of improper cooling methods (deep container with lid on top); boxes of coffee, water bottles, oat milk and boxes of cups and lids incorrectly stored on floor in back of facility; leftmost lowboy unit has a torn gasket; cutting board on front line counter is scratched; facility couldn’t provide high temperature test strips; observed debris on racks where syrups and coffees are stored in back of facility; observed debris/stains on shelves in cabinets and on bottoms of lowboy units on front line; observed debris/buildup on floor under reach-in units, under ice machine in back of facility and under counters and equipment on front line.

Vampire Penguin, 11345 Nuckols Road – No violations reported during a routine inspection.

Glenn’s Forest Café, 7202 Glen Forest Drive – The following violations were reported during a routine inspection: water is pooling inside make table; vents inside make table are dirty; observed food splashes on exterior of cabinets; wall behind cook line is soiled.

Frida’s Mexican Restaurant, 910 East Parham Road – The following violations were reported during a routine inspection: strainer found in hand sink (hand sinks are for handwashing only); raw shrimp and scallops incorrectly stored over ready-to-eat uncovered vegetables in walk-in cooler; observed dried food residue on clean containers and knives stored on the wall; observed buildup on slicer; there is 0 ppm sanitizer in the dish machine; temperature of rice and beans in hot holding units is too low; whipped cream was left sitting out on top of prep unit; no date marks found on cooked beef, beans, chicken or potato/cheese mix; food incorrectly stored on floor of walk-in; to-go containers are not stored inverted on top of prep units; observed low grouting near drink machine and in dry storage area; a light is broken in dry storage; some ceiling tiles are stained.

Vinny’s Italian Grill & Pizzeria, 3434 Lauderdale Drive – The following violations were reported during a routine inspection: raw hamburger incorrectly stored next to raw steak in lowboy; chemical dishwasher is not dispensing chlorine even after multiple priming attempts; observed buildup on can opener blade and ice machine; temperature of sliced provolone is elevated because it was left out on the counter; pizza sauce is being held on time control but was not on the policy; buckets of food and drink cannisters incorrectly stored on the walk-in floor; cutting boards at make tables are dark and worn; gaskets on line lowboy and reach-in freeezer are torn; hood filters are dented; walk-in racks are rusty; oven hood, soda bag lines, beer cooler vents and ceiling vents on cook line are dusty; observed dust and grease buildup on pipes behind the cook line; cabinet surfaces in the beverage area are dirty; top of dishwasher is covered in dust; ceiling tiles are discolored; observed soils on floors under equipment; walls and door frames in kitchen are stained; observed buildup inside floor drains; back kitchen light is out and has no protective shield.

El Paraiso Restaurante, 4034 Crockett Street – The following violations were reported during a routine inspection: black residue buildup found inside ice machine; temperatures of raw chicken and ham in the reach-in cooler and raw beef and cheese in the prep unit are elevated; these two units are not keeping foods at the proper temperature; concentration of sanitizer in three-compartment sink basin is too high; reach-in cooler wire storage racks are rusty; gasket is missing on prep unit; gasket is torn on back reach-in cooler; observed food debris and grease on surfaces of fryers, grill and wire storage racks above three-compartment sink; observed grease and food buildup on floor under cook line equipment and on walls.

Acacia Midtown, 2363 Roux Street – The following violations were reported during a routine inspection: didn’t observe a written agreement from supplier that salmon is farm raised and pellet fed; didn’t observe the last date shell stock was sold on shell stock tags in kitchen; chemical dish machine initially registered at 10 ppm chlorine.

Saladworks, 7000 Forest Avenue – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager (this is a repeat violation); temperatures in the display case are inconsistent; cutting boards are scratched and stained.

Waffle House, 5102 Williamsburg Road – The following violations were reported during a routine inspection: layer of slime found inside ice machine; temperatures of cheese slices, cubed ham, sliced country ham and milk in the walk-in cooler are elevated; temperature of raw shell eggs was elevated at the time they were put on time control.

Papa John’s Pizza, 3067 Lauderdale Drive – The following violations were reported during a routine inspection: hand sink on line doesn’t have paper towels; can opener and blade are dirty; temperature of chicken bites in the pizza lowboy is elevated; pizza dough trays are being used to elevate dough in the walk-in (use proper storage equipment); cutting board on the can cart is discolored; plastic tray over three-compartment sink is cracked/broken; gasket on reach-in is torn; dry storage rack is rusty; front corner warmer is coated in a brown film; area near front corner warmer is extremely dusty; walk-in cooler fan guards are dusty; rolling can cart slots and rolling dough trays have buildup on them; skinny sections of ceiling tiles are missing over the poster wall; ceiling over the oven is stained and dusty.

Subway, 9621 West Broad Street – The following violations were reported during a routine inspection: hot water at hand sink in dish area and at hand sink on front line doesn’t reach the required temperature (facility had no hot water during inspection); chlorine sanitizer level is too low in sanibucket for knives on front line; observed black mold on ice chute at soda machine in dining area; temperature of lettuce in lowboy is elevated; quaternary ammonium sanitizer level is too high at three-compartment sink; to-go bowls above convection oven on front line are not stored inverted; walk-in freezer racks are rusty; paint is chipping on walls in dry storage area in back of facility; ceiling vents on front line and above dough proofer are dusty; there is heavy ice accumulation on floor of walk-in freezer.

O’Charley’s Restaurant, 6291 West Broad Street – The following violations were reported during a routine inspection: the dish washer did not wash hands when going from dirty to clean dishes in dish room; food bits found in hand sink on cook line and in hand sink in prep area (hand sinks are for handwashing only); observed pink mold on metal panel of ice machine in back kitchen; observed food debris on tongs on cook line and on utensils stored clean on rack; temperatures of mashed potatoes and gravy are too low; temperature of salad at salad bar on cook line is elevated; temperatures of rice, whipped cream, cheesecake, parmesan cheese and cajun butter are elevated; use by dates are incorrect for cut lettuce containers and mac and cheese in walk-in cooler; there isn’t a time stamp for all breading and buttermilk on cook line; multiple white sugar packages in bulk container are unlabeled; dirty ice bags are stored directly on top of cut lettuce on cook line; forks in silverware container are not stored inverted; plastic pans on shelf are stacked while still wet; air temperature of reach-in unit at end of cook line is elevated; multiple refrigeration units have multiple torn gaskets; racks in dish area are rusty; four light bulbs are out at hood system; cutting boards throughout cook line and prep area are heavily scratched; observed buildup/debris/grease on hood filters, under fryers, on sides of convection oven, in between equipment on cook line and kettle station, on shelves and racks throughout facility, on hose pipe and outside of best oil storage system near cook line, on floors and walls under and behind equipment on cook line and on floor in dry storage room; faucet at prep sink is leaking; drain lines from soda machine at bar are placed directly on the floor and not into the drain; ceiling vents in wait station in kitchen are dusty; there is low grouting on floors throughout kitchen; ceiling tiles in disrepair on leftmost corner of dry storage room and in middle of cook line; ceiling light not working properly in dry storage room.
