Pho Thien Phat 2, 9031 West Broad Street – The following violations were reported during a follow-up inspection: facility couldn’t provide a certified food protection manager certificate; food bits found in kitchen hand sink and an employee was rinsing wiping cloths at hand sink (hand sinks are for handwashing only); raw chicken incorrectly stored over raw beef in walk-in cooler; observed live and dead roaches on kitchen floor; racks in walk-in cooler are rusty; lowboy unit in front bar area has torn gaskets; air temperature of reach-in unit in kitchen is elevated; cutting boards in back kitchen are heavily scratched; observed debris/grease/buildup in between equipment, under fryer and grill, on cart, hood filters, light bulb covers on cook line, outside of equipment on shelf under prep table, and on racks and shelves throughout facility; walk-in cooler fan guards are dusty; ceiling tiles are not easily cleanable in dry storage room and above ice machine in kitchen storage room; observed debris/buildup/grease/stains on floors, walls and ceilings throughout facility, especially on cook and service line in kitchen.

Perlas Pizza, 201 Towne Center West Boulevard – The following violations were reported during a routine inspection: “Hot Shot” bug killer spray found under register (DIY insecticides may not be used in a restaurant); soiled cardboard is lining shelves and sitting on the oven; some lights are out in the kitchen; observed residues on fridge door handles and gaskets; large floor mixer and storage shelving on walls feel sticky; observed buildup on walk-in racks; soda bag lines are dusty; walls at the prep table, three-compartment sink and at back electric panel are dirty; wall in the walk-in has some chipped paint.

The Place at Innsbrook, 4036-C Cox Road – The following violations were reported during a routine inspection: clean dishes have a white residue on them; can opener blade has residue on it; older metal surfaces (walk-in, grease trap, some pipes) in the kitchen are rusty; fridge gaskets on the line have buildup; dish rack is dusty; cooler/freezer fan encasements have areas of dark buildup; walls, doors and floors/grout in kitchen have staining and dirt; ceiling vents are dusty; dish wall is mildewy.

The Pit and the Peel, 11800 West Broad Street – The following violations were reported during a routine inspection: front hand sink doesn’t have paper towels; one spray bottle of cleaner is unlabeled; grab-n-go lunch box labels are missing some info (business address and net weight – also remember to include allergen info if spreads/sauces contain egg, peanut, etc. and preservatives); utensils incorrectly stored in standing water; all cutting boards are heavily worn; syrup rack and walk-in cooler fan grate are dusty; box fan near office has heavy dust buildup; observed debris stuck in rubber drain mats; observed buildup in walk-in floor corners; kitchen floor has worn patches.

Glen Allen Bistro, 10209 Staples Mill Road – The following violations were reported during a routine inspection: cutting boards are heavily stained; some wire racks are rusted; observed food debris and dust on wire racks, tops of ovens, portable fans, containers, walls, ceiling panels and ceiling vents.

The Lilly Pad, 9680 Osborne Turnpike – The following violations were reported during a routine inspection: observed moldy tomatoes and mozzarella in pizza prep unit and mold on hummus in the salad prep unit; several foods are past their expiration dates (cooked shrimp, cooked chicken, feta, braised cabbage, mushrooms, carmelized onions, hot pickled veggies, roasted garlic aioli, cooked lentils, four containers of black bean burgers); menu is missing an advisory statement regarding burgers and brunch burger with sunny side up egg, both of which are cooked to order; two metal skimmer baskets are broken; didn’t observe proper test strips for quaternary ammonium sanitizer used at three-compartment sink.

PITSA, 11321-B Nuckols Road – The following violations were reported during a routine inspection: observed dried food in both hand sinks (hand sinks are for handwashing only); cleaning spray bottles are unlabeled; several bottles of oil are unlabeled; observed dust and food debris buildup on prep counters storing clean containers, single-service items and spices.

Zara Grill, 10865 West Broad Street – The following violations were reported during a routine inspection: temperature of rice on the stove is too low because the burner was turned off; temperatures of sauces in the make table are elevated because they are piled up high; rice puddings and cake slices are not date marked; frozen raw meat incorrectly stored in “thank you” bags (do not use bags with ink printing on them); dirty cardboard is lining a storage rack; gasket on lowboy freezer needs repairing; door frames and walls in the kitchen are dirty.

Funny Bone Comedy Club, 11800 West Broad Street – The following violations were reported during a routine inspection: prep area hand sink is blocked by a piece of equipment; hand sink also had drain plugs in it (hand sinks are for handwashing only); raw shell eggs incorrectly stored over produce in walk-in; observed heavy mold buildup inside ice machine; spray bottle of sanitizer is unlabeled; butane incorrectly stored next to dry ingredients; glass cleaner and sanitizer tablets incorrectly stored inside bar fridge; bulk flour bin is unlabeled; cutting board at make table is worn and stained; didn’t observe chlorine test strips; observed buildup on bottom of drying rack near three-compartment sink and walk-in racks; floor under and behind ice machine is heavily soiled; walls in both dish areas need cleaning.

Marco’s Pizza, 1070 Virginia Center Parkway – No violations reported during a routine inspection.

The Juicy Crab, 4306 South Laburnum Avenue – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit/diarrhea incidents; raw fish and chicken incorrectly stored over ready-to-eat boiled eggs and crab legs; temperature of chicken wings made yesterday are elevated; salt in back of kitchen is unlabeled; wet wipe cloth is stored in a sanitizer bucket with only 10 ppm bleach solution; single-service to-go boxes are not stored inverted on the cook line; sanitizer dispenser at three-compartment sink isn’t working.

Firehouse Subs, 8191-A Brook Road – The following violations were reported during a routine inspection: temperature of tomatoes prepped five hours earlier is elevated; temperatures of lettuce, prime rib, onions, ham, turkey, steak and tuna salad are elevated; gaskets are torn on lowboy and three-door reach-in.

Ironclad Coffee Roasters, 2100 John Rolfe Parkway – The following violations were reported during a routine inspection: back hand sink doesn’t have hand soap or paper towels; observed some pink/black residue inside the ice machine; raw shell eggs were left out on the counter and the person in charge doesn’t know why; temperature of honey butter is elevated; unsalted butter is out on the counter; chemical spray bottles are unlabeled; bulk flour and sugar bins are unlabeled; boxes of single-service items (cups, spoons, deli containers) incorrectly stored on the floor; cardboard box shouldn’t be used to permanently store single-service cups; wood at pass-thru window above three-compartment sink appears unsealed and moldy; wood under coffee makers is unsealed; wet mop incorrectly stored in mop bucket instead of being hung to air dry.

Subway, 11234 Patterson Avenue – The following violations were reported during a routine inspection: person in charge was unaware of the “big six” reportable foodborne illnesses and the associated symptoms; this information couldn’t be located during the inspection; facility doesn’t have written procedures for cleaning up vomit/diarrhea incidents; temperature of tuna salad prepped 5.5 hours ago is elevated because containers are covered and stacked; temperatures of rotisserie chicken, chicken strip, cheese, teriyaki chicken, lettuce, tomatoes and veggie patty are elevated; back-up refrigerator on the line is not maintaining food at the proper temperature.
