Torchy’s Tacos, 11120 West Broad Street – The following violations were reported during a routine inspection: observed employees changing gloves without washing hands in between; temperature of chicken on the steam table is too low; temperature of beaten eggs in the make table is elevated because it was double panned; spinach on time control has an incorrect discard time/date; prepped foods were left sitting out for extended times after employees were finished portioning.

Panda Express, 11286 West Broad Street – The following violations were reported during a routine inspection: nozzles at drive-thru appear to have buildup along the sidewalls; gasket and bottom of freezer on the cook have a lot of crumbs; observed grease buildup inside fryer cabinets; top shelf above back prep area feels sticky; floor tile near floor drain along the cook line is cracked.

Panera Bread, 8800 Staples Mill Road – The following violations were reported during a routine inspection: single-use items are stored uninverted on a dirty shelf; single cutting boards are heavily scratched; observed dried food debris buildup on surfaces of storage racks, reach-in cooler interiors, walk-in fan system, prep shelves, monitors, microwave handles, cooler handles, coffee makers and lid holders; observed encrusted carbon deposits and/or other soil deposits on baking sheets; observed food debris on floors under prep tables, racks and coffee stations; walls and inside drain under dish machine have food splatter.

TGI Friday’s, 7023 West Broad Street – The following violations were reported during a routine inspection: women’s employee restroom doesn’t have paper towels; there is food debris on the salad spinner on the prep table and on unused plates on the cook line shelf; final rinse temperature at high temp dish machine doesn’t reach the required temperature; observed pink mold on ice chute of both ice machines and on sides of smaller ice machine near bar area in back of kitchen; temperature of lettuce prepped this morning is elevated; temperatures of pasta, parmesan cheese and tomatoes in prep top unit are elevated; temperature of rice prepped this morning is elevated because of improper cooling methods; label maker is printing out wrong prep times; there is 0 ppm quaternary ammonium sanitizer in bucket on cook line; forks and to-go trays are being stored uninverted; door to lowboy on cook line is not closing properly; racks in walk-in cooler and fan in meat walk-in cooler are rusty; walk-in cooler door gasket is torn; there are gaps in between hood filters on hood system on cook line; cutting boards in kitchen are moldy and heavily scratched; observed debris/buildup/grease on hood filters, in between equipment, under grill and fryers, on light bulb covers, in between gaskets for refrigeration units, on pipes on cook line, and on shelves and racks throughout facility; fan guards in walk-in cooler are dusty; faucets at prep sink are leaking; hand sink is slow to drain at wait station in kitchen; dry storage area wall is deteriorating; there are cracked floor tiles and low grouting on floors throughout facility; ceiling light shield near three-compartment sink is cracked; there is debris/grease/buildup on walls and floors under and behind equipment on cook line and in the dish room, liquor room and unused bar area; there are stains on ceiling tiles in dish room and unused bar area; ceiling vents in unused bar area are dusty; mop sink and walls at mop sink are soiled; there is standing water on floor of liquor room and buildup on floor corners.

Chicken Fiesta, 7925 West Broad Street – The following violations were reported during a routine inspection: multiple employee drinks stored next to food/food service items; no hand sinks in the kitchen have handwash signage posted; hot water doesn’t reach the required temperature at hand sink next to reach-in unit and hand sink next to back exit door; metal container with soap and metal scrub brush found in hand sink (hand sinks are for handwashing only); boxes of raw beef incorrectly stored below boxes of raw chicken in leftmost walk-in cooler; raw ground beef incorrectly stored next to flan in reach-in unit; there is 0 ppm quaternary ammonium sanitizer at three-compartment sink; temperatures of cut lettuce, beans, pico de gallo, shredded chicken, tamales, flan, salad and rotisserie chicken are elevated because of improper cooling methods; temperature of chicken double panned on the hot line is too low; temperatures of raw marinated chicken, veggies, unsalted butter, salad, shredded cheese, shredded chicken, sliced cheese and pico de gallo are elevated; air temperature of two units is elevated; laminated menus are missing an asterisk next to steak; menu posted at front register is missing a disclaimer/reminder about foods cooked to order; two dead roaches found on floor under soda boxes in back of facility; multiple dead flies are hanging from fly tape above ceiling in kitchen; tongs improperly stored in bucket of dirty water on cook line; ice scoop handles are touching ice in ice machine in back of facility and in ice bin near front of house; metal pans are stacked while still wet on clean rack; to-go plates in kitchen are not stored inverted; cutting boards throughout kitchen are heavily scratched; cutting board on hot line is also heavily stained and black; multiple refrigeration units have torn gaskets; racks in rightmost walk-in cooler are rusty; observed grease/debris/buildup on light bulb covers, hood filters, in between and under equipment, under fryers on cook line, on shelves and racks throughout, on bottoms of reach-in freezer and outside of multiple equipment on prep table near dish area; fan guards in walk-in cooler are dusty; there are cracked floor tiles in leftmost walk-in cooler; floors throughout kitchen have low grouting; observed debris/grease/buildup/stains on floors and walls behind and under equipment on cook line and in front service area; ceiling tiles in kitchen have stains; ceiling vents in kitchen are dusty.

Chez Max Restaurant, 10622 Patterson Avenue – The following violations were reported during a routine inspection: open employee drink found on shelf at prep table; margarine and fresh garlic mixture have been left out at room temperature; scooped ice cream on flat trays in the freezers are uncovered; in-use utensils should not be stored in sanitizer water; restroom doors are not self-closing.

McDonald’s, 10000 Brook Road – The following violations were reported during a routine inspection: observed an employee crack eggs and then put together ready-to-eat sandwiches without washing hands and changing gloves; temperatures of sausage burritos and cream next to drive-thru are elevated; did not observe time stickers on lettuce; single-service items should not be stored on the ground; observed low grouting in back dish area and near front counter; fan grates are being held up poorly by tape; hand sink by fry station is in disrepair.

Noorani Kabab House, 2757 Hungary Spring Road – The following violations were reported during a routine inspection: temperature of vegetable soup prepared last night in the lowboy is elevated because it was not cooled properly; there are no dates on prepared foods in the walk-in cooler (chickpeas, goat, chicken, cooked spinach); observed live roaches on pipe over three-compartment sink and in dish area.

Tarrants West, 11129 Three Chopt Road – The following violations were reported during a follow-up inspection: no handwash signage is posted at hand sink at end of bar; hot water at rightmost hand sink at bar doesn’t reach the required temperature; racks in both walk-in coolers and in dry storage area are rusty; multiple cutting boards throughout kitchen are heavily scratched; floor tiles in kitchen are cracked; kitchen floors have low grouting; bottom of wall where hot water heater is located is deteriorating.

Deca’s Café LLC, 391 Azalea Avenue – The following violations were reported during a routine inspection: sanitizer levels are low at three-compartment sink; temperature of pasta reheating in the hot holding cabinet is too low.

Mosaic Restaurant, 6229-A River Road – The following violations were reported during a follow-up inspection: employee is not wearing gloves while cutting peppers; kitchen and bar hand sinks are missing hand soap; cook line hand sink doesn’t have paper towels; foods were not reheated to the proper temperature before being placed in the warmer; temperatures of cole slaw, pimento cheese and salmon in the large make table are elevated; moldy cream cheese, pork belly and Canadian bacon in the walk-in are not date marked; temperatures of greens and kale are elevated; lights in the dish room aren’t working; self-closure device on the restroom is partially done; ceiling tiles are missing.

Bowlero Richmond, 6540 West Broad Street – The following violations were reported during a routine inspection: three bottles of peroxide cleaner are unlabeled; observed a horde of small flies at the soda machine; fish bowls used for drinks incorrectly stored on the bar floor; freezer gaskets have food debris; kitchen equipment surfaces feel greasy; point-of-sale shelving, ice machine vent and hood vent over the oven are dusty; bottoms of lowboy interiors have food residues; wall near pizza station is heavily soiled; floor and pipes of beer cooler are dirty; wall above ice machine is dusty; observed grease buildup behind fryers and debris under fridges on the line; wall and pipes under three-compartment sink is soiled; wall at three-compartment sink has a lot of chipped paint.

MOD Pizza, 5466 Wyndham Forest Drive – The following violations were reported during a routine inspection: containers of red sauce improperly covered and stacked in walk-in cooler; cut tomatoes on the line shouldn’t be covered during the cooling process; several wire racks are rusty; floors throughout the establishment are not smooth, durable, easily cleanable and/or nonabsorbant.

Hardee’s, 3201 East Parham Road – The following violations were reported during a routine inspection: an employee failed to wash hands after touching face and before continuing food prep; ice scoop at drive-thru is stored in a dirty container; observed grease residue and dried food debris on containers and utensils in storage room; containers on dry racks have old labels; several foods in the make line unit are below the required temperature (chili, chicken tenders, hotdogs, hamburger); this hot holding unit is not working properly; temperature of a pack of ham on the prep table is elevated; time stickers for tomato, lettuce and other items are from Jan. 27 (time is not being marked consistently); tongs and a whisk are stored on dirty surfaces on the front line; single-service and single-use items are stored on a dirty prep surface; some wire storage racks are rusty; observed heavy buildup of grease, food and grime on surfaces and wheels of grills, charbroiler, fryers, bread toaster, electrical cords and panels; observed buildup of food debris, dust and grime on portable fans, cables, touchpads, monitors, wire storage racks, fan grate in walk-in cooler, black carts and inside reach-in coolers and freezers; paint is peeling off on ceiling above drive-thru area and in dry storage room; observed heavy buildup of food debris, grease and grime on floors and walls throughout the establishment, walk-in coolers and mop sink; ceiling panels and vents above food prep areas are dusty.

Potro’s Mexican Restaurant, 9074 Staples Mill Road – The following violations were reported during a routine inspection: observed badly dented cans stored on rack; temperatures of chicken wings and nuggets in prep drawer are elevated; single cutting boards are heavily scratched; observed grease buildup on surfaces of grill and fryers; fan grate and ceiling of walk-in cooler are dusty.
