El Paso Mexican Restaurant, 3417 Cox Road – The following violations were reported during a routine inspection: hand sink in kitchen doesn’t have any handwash signage; bits of food, grill scraper and top of can found inside kitchen hand sink (hand sinks are for handwashing only); raw shrimp incorrectly stored over mashed potatoes in reach-in unit; raw shrimp and raw beef incorrectly stored over plantains in walk-in cooler; chlorine level is too low at chemical dish machine in kitchen; observed an employee place multiple pans on clean rack without sanitizing the equipment properly; observed food debris inside of Moscow mule cups in beer cooler at bar area; temperature of queso on hot line is too low; temperature of salsa is elevated because it has been sitting out since 11 a.m.; temperature of cut tomatoes is elevated because they have been out at room temperature for three hours; menu doesn’t contain asterisks that link raw/undercooked meats to the consumer advisory statement; temperature of tomatoes in prep top unit on cook line are elevated because of improper cooling methods; multiple packages of salmon are improperly thawing in walk-in cooler in reduced oxygen packaging; two containers of white powders are unlabeled; multiple foods are incorrectly stored on floor of walk-in cooler; tortilla bowls are stored uncovered on cook line; multiple breads are stored uncovered in reach-in freezer; pans on clean rack are stacked while still wet; to-go trays are not being stored inverted on cook line; multiple refrigeration units have torn gaskets; racks in walk-in cooler and a clean rack in the dish room are rusty; one light bulb is out at hood system on cook line; cutting boards throughout kitchen are heavily scratched; there is heavy grease under the grill on cook line; observed debris in bottom of reach-in freezer, on clean rack and on top of chemical dish machine; observed buildup on soda gun holder at bar, shelves on top of aluminum foil on cook line and on can opener holder on prep table; there is a leak under the faucet at prep sink in kitchen; the floor is sinking where floor drains are located on cook line; there is chipping paint on floor near tortilla machine; observed buildup on wall behind equipment on cook line; ceiling vent and surrounding ceiling tiles in kitchen are dusty; observed debris on bottom of floor under grill on cook line.

Subway, 3326 Pump Road – The following violations were reported during a routine inspection: one nozzle on the soda machine in grimy; temperatures of lettuce and veggie patties on the line are elevated; facility couldn’t provide quat test strips (manager delivered some before end of inspection); interior surfaces and gaskets of the cook line lowboy are soiled; walk-in cooler racks are dirty; wall under three-compartment sink is dirty.

Sonic Drive-In Restaurant, 11250 West Broad Street – The following violations were reported during a routine inspection: rack above three-compartment sink is rusty; gasket on lower door of the reach-in freezer is torn; prep unit cutting board is heavily scratched; observed buildup on fridge and freezer gaskets; crevices of the shake dispenser are crusty; walk-in cooler fan guards are very dusty; walk-in cooler racks are dirty; ceiling over the prep unit and the drive-thru area wall are dirty.

Arilang Sushi, 2239 Old Brick Road – The following violations were reported during a routine inspection: facility couldn’t provide parasite destruction documentation from salmon and ground tuna suppliers; raw eggs incorrectly stored above avocados and cucumbers; tuna is incorrectly thawing in reduced oxygen packaging in refrigerator.

Hillbilly Reds BBQ, 353 East Williamsburg Road – The following violations were reported during a routine inspection: observed food debris greater than one day old on microwave; temperature of chicken in the crockpot is too low; no date marks found on brisket, chicken wings, baked beans, etc. in the walk-in cooler.

Desi Bites, 3621-C Cox Road – The following violations were reported during a routine inspection: employee did not replace her gloves and wash hands after touching kitchen door and before handling food; observed a waitress touch food as she was handing it off to a customer; hand sink in dish room doesn’t have hot water (faucet for hot water is not in working order); hot water doesn’t reach the required temperature at the hand sink next to sandwich unit; this hand sink is also missing paper towels; all hand sinks are missing required handwash signage; raw eggs incorrectly stored in between mashed potatoes and curry paneer in lowboy unit; chlorine level is at 0 ppm at three-compartment sink; observed food debris inside multiple blenders, scoop and utensil stored clean; observed Ibli, onion sauce, tomatoes, cabbage, fried vegetables, cooked eggs, cooked chicken, cooked noodles, fried chicken, pasta, fried fritters and fried pepper at incorrect temperatures because of improper cooling methods; temperature of chili oil is elevated because it was held out at room temperature since yesterday; temperatures of mashed potatoes, beef curry, cooked vegetables, curried goat, yogurt and hard boiled eggs are elevated; temperatures of curry, vegetable and mashed potato curry, garlic and oil, ginger, cooked chicken, onion bread, chili chicken and rice are too low; no date marks found on mashed curry potatoes and paneer curry made three days prior in lowboy unit; did not observe a time control policy on multiple TCS food items left out at room temperature throughout kitchen and did not observe a written time on any of food items; white powders on the cook line are unlabeled; there are dead flies on fly tapes hanging from the kitchen ceiling; observed multiple small flies on wall in prep area; observed one live roach crawling behind sandwich unit and one live roach crawling on floor in dry storage room; multiple foods incorrectly stored on floor of walk-in cooler; in-use utensils incorrectly stored in standing water; pans on clean rack are stacked while still wet; single-service fork box incorrectly stored on floor; single-service to-go trays in back kitchen are not stored inverted; air temperatures of rightmost sandwich unit on cook line and display case in front of house are elevated; cutting board on sandwich unit is heavily scratched and stained; facility couldn’t provide chlorine test strips; observed debris in between gaskets on lowboy units throughout kitchen, on racks throughout facility, on bottom of freezer reach-in unit, on shelves behind cook line, on top of freezer, outside of equipment in prep area, in between equipment and under grill on cook line and on shelves throughout kitchen; there is debris on floors and stains on walls throughout kitchen, especially in dish area; floors throughout kitchen have low grouting.

Shula’s Steak House, 12042 West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit; hand sink in dish area is missing paper towels; raw shell eggs incorrectly stored over bread in reach-in cooler; no date mark found on Bernaise reduction; facility is not maintaining the logs required as part of the HACCP plan for vacuum packaged foods; the current procedures are also different from what was originally approved; dishes are stacked while still wet; chemical rack is rusty; gasket on lowboy across from the stove is torn; two lights are out under the hood system; cutting boards at make tables and mini board at the bar are worn; floor under bar equipment is soiled.

La Cabaña, 4086 Crockett Street – The following violations were reported during a routine inspection: observed food in the hand sink (hand sinks are for handwashing only); sanitizer concentration is at 0 ppm in the three-compartment sink; stewed meat improperly cooling inside a deep container with cover; prep and single cutting boards are heavily scratched and stained.

Waffle House, 3202 East Parham Road – The following violations were reported during a routine inspection: temperature of a pan of sliced tomatoes in make unit is elevated; cutting board on prep line is heavily scratched and stained; rolling carts are rusty; observed grease and food debris buildup on surfaces of grills, stove crevices, reach-in unit interiors, wire racks and fan grate system in walk-in cooler; coffee makers, juice dispensers, ceiling vents and counters storing clean dishes are dusty; observed food debris on floors and walls, under and behind equipment.

West Lake Chinese Restaurant, 11256 Patterson Avenue – The following violations were reported during a routine inspection: observed an employee rinse hands without using soap after handling raw foods; observed an employee wash hands at prep sink and three-compartment sink (use hand sink); raw shell eggs incorrectly stored above cooked noodles and celery in reach-in unit; no date marks found on cooked noodles and chicken; time is not being tracked for cut cabbage, garlic in oil, egg rolls and sprouts (no written policy available); white powders are unlabeled; food incorrectly stored on floor of walk-in; bags of food incorrectly stored on floor in back kitchen; MSG is stored uncovered; faucet at front hand sink leaks; mops are not being hung to air dry; observed heavy grease on hood filters.

Glory Days Grill of Gleneagles, 10466 Ridgefield Parkway – The following violations were reported during a routine inspection: observed mold inside ice machine; sanitizer at bar dish machine is not moving through the line; temperature of cut tomatoes prepared more than four hours ago in the walk-in is elevated; temperature of milk in the bar fridge is elevated; foods in two make tables and one reach-in unit on the cook line have elevated temperatures (sliced tomatoes, raw salmon and raw chicken).

Tokyo Sushi Japanese, 10274 Staples Mill Road – The following violations were reported during a routine inspection: water had been shut off at the hand sink in the kitchen during hours of operation; observed black mildew buildup inside ice machine; single-use containers are not stored inverted and the shelf they are on is dusty; prep unit, sushi bar and single cutting boards are heavily scratched and stained; observed greasy cardboard lining prep table shelves under stove, grill and storage areas; there is heavy buildup of grime and limescale inside dish machine; wire storage racks are dusty; observed grease and food buildup between and under stove, on fryers and grill, and on suppression nozzles above stove; wire shelves inside coke cooler have food debris; walls throughout kitchen have grease and food splatter.

Twin Hickory Tavern, 11351 Nuckols Road – The following violations were reported during a routine inspection: no supervisors or managers are certified food protection managers; an employee failed to change gloves after touching their hair/face and before continuing food prep; container of bleach in dry storage room is unlabeled; several bottles of sauces and oils are unlabeled; single-use containers are not stored inverted and the shelf they’re on is dusty; prep and single cutting boards are heavily scratched and stained; wire racks and shelves above prep units are dusty; there is food and grime buildup on wire racks, fan grate and beer lines in walk-in coolers; walls have dried food splatter; observed food and grime inside floor drains under three-compartment sinks.
