Cook Out Store, 1501 Eastridge Road – The following violations were reported during a routine inspection: no evidence of handwashing (both hand sinks are dry); temperature of cooked bacon on flat top pan is too low; temperatures of cheesecake and eggnog are elevated; time is not being tracked for chicken/cheese mix on the line; cardboard incorrectly lining several shelves; fryer cabinets, flat grill, back side of soda machine and wall under three-compartment sink are dirty; floors and floor drains are dirty; water is pooling in the storeroom and by the three-compartment sink.

Panera Bread, 9960 Brook Road – The following violations were reported during a routine inspection: hand sink in the baking area doesn’t have hot water; temperatures of steak and chicken in drawer of lowboy are elevated; pepperoni, chicken and bacon in the prep unit are all past their expiration dates; observed fraying bristle brushes, rusted scissors and worn stained brushes; interior of units is filled with buildup of grime and food debris; observed mold/mildew buildup in gaskets of units; racks throughout the kitchen need cleaning; observed food splatter on plastic where blenders are placed; water heater is leaking; several areas need cleaning (floors under equipment, walls in back prep area, dish area, and around the mop sink).

Golden Skillet, 345 East Williamsburg Road – The following violations were reported during a routine inspection: no date mark found on coleslaw made yesterday; time is not being tracked for fried gizzards and potato wedges on time control; sanitizer incorrectly stored next to cans of tomato sauce; four containers of fried chicken incorrectly sitting out at room temperature to cool; drinks incorrectly stored on floor in back of dry storage hallway; flour is uncovered in dry storage.

Arby’s, 4250 Pouncey Tract Road – The following violations were reported during a routine inspection: can opener is dirty; time is not marked on cheese on time control; many foods in the walk-in are improperly cooling while covered and packed into paper boats which impedes air flow; utensils near the soup warmer incorrectly stored in standing water; gaskets are torn in the prepped veg lowboy and in the under-counter freezer; front counter shelving is dusty.

Chianti Italian Restaurant & Pizzeria, 1304 Gaskins Road – The following violations were reported during a routine inspection: did not observe a certified food protection manager certificate; an employee didn’t wash hands after handling raw chicken; no written agreement from supplier about salmon being farm raised and pellet fed; heavy whipped cream incorrectly stored between raw breaded veal in lowboy unit on cook line; raw chicken incorrectly stored over raw veal in lowboy unit; raw calamari incorrectly stored between raw chicken in reach-in unit; raw shrimp incorrectly stored next to raw chicken in walk-in cooler; quaternary ammonium sanitizer levels at three-compartment sink is too low; angel hair pasta prepped this morning incorrectly cooling in a deep container with saran wrap; chicken and unsalted butter on cook line are at improper temperatures; temperatures of sausage, pasta salad, ham, cheese, tomatoes, whipped cream, peppers, marinara, lasagna, soup, eggplant, pasta, raw chicken and raw shrimp are elevated; no dates found on items made two or more days ago in reach-in unit and walk-in cooler (marinara, lasagna, salted butter, soups, etc.); did not observe written times for breading used for chicken and veal on cook line; improper cooling methods observed for pasta, sliced ham, lettuce and salads; wine, drink and food boxes incorrectly stored on the floor at bar, front service area and in walk-in cooler; in-use utensils incorrectly stored in standing water on cook line; spoons in front service area and to-go trays on cook line are not stored inverted; multiple units have torn gaskets; water is pooling in lowboy units on cook line and in bar area; air temperature of walk-in cooler is elevated; ice machine at front service area doesn’t close properly; cutting boards on cook line have black mold and are heavily scratched; observed buildup/grease/debris on hood filters, in between and outside of equipment, in between gaskets on cook line, and on racks and shelves throughout the facility; fan guards in walk-in cooler are dusty; observed buildup/grease/debris/stains on floors and walls throughout facility; ceiling in dish area has holes; some floor tiles are cracked; there is low grouting on cook line and in prep area; wall is deteriorating in dish area.

Hong Kong Kitchen, 2907 Williamsburg Road – The following violations were reported during a routine inspection: three-compartment sink sanitizer has 0 ppm bleach; temperature of General Tso’s chicken in front glass door reach-in is elevated; time is not being tracked for fried chicken on time control.

Arianna’s Italian Grill, 5107-5109 Lakeside Avenue – The following violations were reported during a routine inspection: temperatures of chicken, tomatoes and deli meat in lowboy are elevated; wet wiping cloths are sitting out on prep surfaces instead of being stored in a sanibucket; racks in between cooking equipment are greasy and dusty; observed dust and grease on top of pizza oven.

Longhorn Steakhouse, 4409 South Laburnum Avenue – The following violations were reported during a routine inspection: bar hand sink doesn’t have hand soap; facility couldn’t provide a parasite destruction letter from the salmon provider; microwaves on the cook line are dirty; temperature of sweet potatoes in hot holding drawer is too low; one of the hood vents above the flat top grill is missing; facility couldn’t provide multi-quat test strips; floors are dirty, especially under equipment.

Peter Chang, 11408 West Broad Street – The following violations were reported during a routine inspection: two open drinks found in food prep areas; back corner hand sink is covered with a sheet pan; raw meats incorrectly stored over cooked noodles and chicken in the walk-in; raw ducks incorrectly stored above prepared sauces; can opener and ice machine interior are dirty; noodles were found cooling on sheet pans stacked directly on top of one another, with bottom of pans in contact with the noodles below it; meats in the walk-in freezer are stored in direct contact with bags that have printing on them; knives incorrectly stored in equipment crevices along the cook line; ice scoop handle is touching ice; cutting boards along the cook line have dark deep scratches; two refrigerators have torn gaskets; one is being held together with tape; bottom tiers of work tables are rusted; there is some rust on walk-in racks; observed buildup on refrigerator and cook line surfaces; some fridge gaskets have food residues along the edges; walk-in cooler racks and fan guards are heavily soiled; cook line floor under and behind cooking equipment is greasy; wall and floor behind ice machine have soil buildup; walls in the kitchen have stains/dirty splotches in various areas; hood filters are very greasy.

Mosaic Restaurant, 6229-A River Road – The following violations were reported during a routine inspection: observed open drinks on front cook line and back area; observed an employee enter the back door and begin to handle equipment before washing hands; bar hand sink doesn’t have hand soap; there is inadequate record keeping for molluscan shellfish when removed from their tagged or labeled container; slicer is dirty; incorrect temperatures observed for salmon on rack, gouda cream, tuna, beef, gouda dip, black rice on rack, greens on ice, gouda spread, and squash puree; raw egg whites is used for a drink at the bar but the menu doesn’t disclose the use of raw egg nor does it remind of the risk; soap incorrectly stored on food shelf beside kitchen hand sink; grill cleaner incorrectly stored on food shelf; lights in dish room are not working; a plastic container is cracked; linen should not be used to line shelf above stove; an irreversible registering temperature indicator or heat tapes were not provided for the hot water mechanical warewashing operation; prep sink is dripping; dish machine is leaking; employee restroom door was left open; bare wooden block found under dish machine; there is exposed foam inside the beer walk-in; ceiling panels are missing above the chemical shelves; floor under fryers is dirty.

Baskin-Robbins, 5510 West Broad Street – The following violations were reported during a routine inspection: observed a can of Raid in the back storage area (Raid may not be used in a food establishment); shelves under point-of-sale and storage drawers have residues in/on them; observed soil and debris under service line equipment.

Zaxby’s, 5816 West Broad Street – The following violations were reported during a routine inspection: single-service and single-use items stored on a dusty counter; a knife with chipped blade is being used to cut chicken; single cutting boards are heavily scratched; observed dust/flour/grease buildup on surfaces of fryers, grills, cables, monitors, dry storage racks, fan system in walk-in cooler and on prep counters; observed grease and food buildup on floors and walls; ceiling panels and vents in back room are dusty.

McDonald’s, 5520 West Broad Street – The following violations were reported during a routine inspection: quat sanitizer level at three-compartment sink is too high; ice scoop handle is touching ice; plastic lids on top of the hot holding unit are not covered or inverted; milk dispensing tube is not cut at an angle; milk racks are rusty; walk-in freezer and cooler fan guards are dusty; gaskets in undercounter fridge have buildup; storage compartments under the toppings table has debris; surfaces of the ice cream machine are dirty, including the cone holsters; trash cans in the women’s restroom need lids; a few floor tiles near the walk-in are cracked; metal plate on the back door is loose and allowing air through the weather stripping; a cluster of ceiling tiles over the line are discolored; observed grease buildup under fryers; trash and liquid found under the drive-thru area; walls throughout the kitchen are stained.

DQ Grill & Chill, 807 East Parham Road – The following violations were reported during a routine inspection: hand sink in back kitchen doesn’t have paper towels; observed a buildup of pink slime on ice machine shield; observed a buildup of slime on ice machine at drink machines and on drip trays; temperatures of ice cream mix and milk in the lowboy are elevated; did not observe dates on prepped salads, cut tomatoes or gravy; containers and utensils are heavily cracked, which is causing staining or pitting; racks in storage area are rusty; units on front counter and back freezers have torn gaskets; fans in the lowboy are leaking into a drip pan; observed fuzzy mold on racks in walk-in cooler, gaskets of ice cream machine and inside lowboy on front line; several areas need cleaning (inside of units, gaskets, doors on units, standing fan and equipment on cook line); observed pest droppings and other dirt in cabinets under drive-thru; mop sink is leaking; nozzle is hanging into the three-compartment sink; there is low grouting in front counter, around the ice machine and in dish area; coving tiles around wall near drive-thru are missing; caulking is worn around the front hand sink; observed food splatter on walls at drive-thru; fans and ceiling of walk-in are dusty; there is grime/dirt buildup under equipment throughout kitchen.
