Mr. Wong II Chinese Restaurant, 8715 Shrader Road – The following violations were reported during a routine inspection: observed food prep employee chewing gum; prep unit and single cutting boards are heavily scratched and stained; observed mildew and food debris buildup on wire racks in walk-in cooler and on food storage crates.

Caffespresso, 1127 Gaskins Road – The following violations were reported during a routine inspection: facility couldn’t provide a certified food protection manager certificate; bits of food found inside hand sink (hand sinks are for handwashing only); facility couldn’t provide written agreement from supplier about salmon being farm raised and pellet fed; temperatures of salmon and mozzarella in lowboy unit are elevated; ham in the lowboy unit is past its expiration date; menu doesn’t contain a consumer advisory statement and asterisks for salmon (lox); temperatures of sliced turkey, eggs and tomatoes are elevated because of improper cooling methods; labels are missing components for vegetarian soup, lasagna and merengue; multiple utensils near three-compartment sink are not stored inverted; multiple refrigeration units have torn gaskets; observed debris/buildup on shelves and racks in kitchen; observed buildup in between gaskets of lowboy unit in kitchen; hand sink in kitchen is slow to drain; observed debris on floors behind and in between equipment in kitchen.

Five Guys Burgers and Fries, 4450-A South Laburnum Avenue – The following violation was reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit/diarrhea.

Panera Bread, 4483 South Laburnum Avenue – The following violation was reported during a routine inspection: lowboy unit near service window has a torn gasket.

Brecotea, 9627 West Broad Street – The following violations were reported during a routine inspection: observed employee drinks stored near food and food contact surfaces; hot water doesn’t reach the required temperature at all hand sinks throughout facility; raw shell eggs incorrectly stored between heavy cream and pasteurized eggs in walk-in cooler; hot dogs incorrectly stored over raw chicken and next to raw comminuted sausages in walk-in cooler; quaternary ammonium sanitizer level at three-compartment sink is too low; temperature of house milk in reach-in unit is elevated because of improper cooling methods; temperatures of cheese, egg yolk, hot dogs, raw eggs, marble cream cheese, and milk are elevated; did not observe written times for items in display cases in dining area and back kitchen (egg tart, cheese toast, garlic cheese, butter pork floss bun, mustard hot dog, bacon twist and red bean); improper cooling methods observed for cream cheese cake, strawberry cream cup, red bean and cheesecake; multiple powders in prep area are unlabeled; tubs of strawberry filling incorrectly stored on floor in dry storage room; a box of cups incorrectly stored on floor in kitchen; lowboy unit in prep area has torn gaskets; air temperature of display case in front service line is elevated; no hand sinks throughout facility had hot water; both hand sinks in back kitchen are slow to drain.

Marco’s Pizza, 4450 South Laburnum Avenue – The following violations were reported during a routine inspection: multi-quat sanitizer level at three-compartment sink is too low; mop sink doesn’t have a dual check valve installed; floors are dirty; wet mop is stored in the mop bucket instead of being hung to air dry.

Which Wich, 1601 Willow Lawn Drive – The following violation was reported during a routine inspection: boxes of food incorrectly stored on the freezer floor.

Five Guys Burgers and Fries, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: shelf under hanging plastic items in the back is dirty; mop sink doesn’t have a dual check valve installed.

Groovin’ Gourmets, 8501 Sanford Drive – The following violations were reported during a routine inspection: jars of salad dressing in the walk-in cooler are sunken in; several food items in the walk-in cooler are not dated; containers are cracked on storage racks; single cutting boards are heavily scratched and stained; observed food debris on prep counters, wire racks and on fan grates in walk-in cooler; floors in kitchen, storage area and walk-in coolers are not sealed; hand sink in dish room is hanging off the wall and not operational.

Garnish, 8574 Sanford Drive – The following violations were reported during a routine inspection: person in charge’s certified food protection manager certificate has expired; single cutting boards are heavily scratched; observed old labels on containers; spatulas are stained.

McDonald’s, 11280 Patterson Avenue – The following violations were reported during a routine inspection: observed fries fall on an employee's hands while bagging them for orders; drive-thru hand sink doesn’t have paper towels; a pickle slice in the back hand sink is clogging the drain (hand sinks are for handwashing only); observed caked-on shake mix debris around the top and inside of shake machine; nozzles and non-food contact surfaces at orange juice dispenser are soiled; time is not being tracked for lettuce on the line; small ice cream cones are being stored unprotected and in direct contact with a surface soiled with food debris; no sanitizer is detected in wiping cloth solutions; two-door fry freezer and beverage station refrigerator have torn gaskets; facility couldn’t provide test strips for chlorine and quat sanitizers; racks in the walk-in are moldy; there is low grout in front of beverage station and fry fryers; observed heavy grease on floor between grills.

Frank’s West, 11238 Patterson Avenue – The following violations were reported during a routine inspection: personal medications incorrectly stored on shelf with take-out containers; two spray bottles of cleaner are unlabeled.

Taco Bell, 11242 Patterson Avenue – The following violation was reported during a routine inspection: mop sink doesn’t have a dual check valve installed.

IHOP, 10151 Brook Road – The following violations were reported during a routine inspection: observed an employee using bare hands to place food in to-go containers; no handwash signage is posted in restrooms; temperatures of liquid eggs and ham in the middle station are elevated; temperatures of lettuce, tomatoes and salsa in the make table are elevated; containers are stacked while still wet on clean storage rack; observed clean utensils incorrectly stored on the wash drainboard.

Hardee’s, 1120 East Nine Mile Road – The following violations were reported during a routine inspection: employee handled raw eggs and raw sausage at the grill and then proceeded to assemble a breakfast sandwich without changing gloves and washing hands; front hand sink doesn’t have paper towels; observed mustard and food debris in back hand sink (hand sinks are for handwashing only); hot water at all three hand sinks doesn’t reach the required temperature; hand sink near raw chicken station doesn’t have hand soap; raw burgers incorrectly stored over ready-to-eat turkey and cheese in two-door reach-in; raw shell eggs incorrectly stored over ready-to-eat sliced turkey in lowboy unit; ice machine has a layer of pink slime on inside surface; several wire racks are stored in dry storage with grease and food debris; observed chili in hot holding well, sausage, hashbrowns and sliced tomatoes at improper temperatures; several stacks of to-go containers are not stored inverted; lowboy near flat top grill doesn’t completely close; walk-in cooler door is rusty; refrigeration on the prep line for lettuce, tomatoes, etc. doesn’t work; drip tray under hashbrowns is completely full of grease and food; inside and outside of two-door reach-in cooler is dirty with food debris; observed food debris greater than one day old on hot holding cabinet, lowboy under hashbrowns, and trash can near back door; mop sink doesn’t have a dual check valve installed; mop sink has a leak at the AVB; floor behind fryers is greasy and dirty; floors and walls throughout are dirty; wet mops are not being hung to air dry.
