Ledo Pizza & Pasta, 4925 West Broad Street – The following violations were reported during a routine inspection: did not observe a certified food protection manager certificate; observed food debris greater than one day old on vegetable slicer and regular slicer on shelf under prep table; observed pink and black mold on plastic panel of ice machine; observed buildup on can opener blade on prep table; temperatures of cut lettuce, raw beef, parmesan cheese, mozzarella and sliced cheese are elevated; air temperature of prep top unit next to cook line is elevated; no written dates found on multiple pre-portioned deli meats prepped Sunday in walk-in cooler; temperatures of sliced tomatoes and meat patties are elevated because of improper cooling methods; to-go boats and trays are not stored inverted; lowboy unit next to reach-in in kitchen has torn gasket; air temperature of sandwich unit next to cook line is elevated; observed buildup on racks and shelves throughout kitchen, light bulb covers above pizza convection oven, bottoms of hood filters, sides of hood, in between and on sides of equipment, under grill on cook line, on cart next to sandwich unit, inside of single-service container on cart next to sandwich unit, in between gaskets of sandwich unit, on outlets and pipes on cook line, in storage containers on prep table, and outside of pans stored clean on shelf; three-compartment sink faucet is leaking; floors throughout facility have low grouting; there is a hole in the ceiling above the mop sink in back of kitchen; observed buildup/stains/dust on ceiling vents in kitchen, walls behind equipment throughout back kitchen, floors in bar area, and under ice bin in front of house.

The Westin Richmond, 6631 West Broad Street – The following violations were reported during a follow-up inspection: did not observe a certified food protection manager certificate; temperatures of burgers, raw eggs and deli meat in prep top unit on cook line are elevated; air temperature of this unit is elevated; did not observe asterisks next to eggs and on consumer advisory statement on breakfast menu and on consumer advisory statement on dinner menu; temperatures of salads and tomatoes prepped in the last hour are elevated because of improper cooling methods; multiple refrigeration units in kitchen have torn gaskets; one light bulb is out at hood system on cook line; two light bulbs out at hood system in prep area; walk-in cooler racks are rusty; reach-in unit door handle is in disrepair; facility couldn’t provide test strips for high temp dish machine.

Dunkin Donuts, 2300 Hungary Road – The following violation was reported during a routine inspection: observed whisk and spoons being stored in water.

Sandano’s, 11389-B Nuckols Road – The following violations were reported during a routine inspection: temperature of chicken sitting on prep unit is elevated; medicine bottle incorrectly stored above food in prep unit; observed tracking powder being put down by person in charge (contact pest control if you see pests); single and prep cutting boards are heavily scratched; observed dust and food residue on surfaces of prep counters, under grill and on spice shakers; items that are unnecessary to the operation or maintenance of the establishment shouldn’t be stored on the premises; walk-in cooler used to store dry goods and equipment doesn’t have a light.

Casa del Barco at Short Pump, 11800 West Broad Street – The following violations were reported during a routine inspection: uncovered sheet pans of chicken are stacked on top of each other with the bottoms of pans touching the chicken of the pan below it on the line; observed food debris and old tape labels stuck to clean pans; upstairs bar glass washer is not sanitizing (has 0 ppm chlorine); soda gun nozzle at the downstairs bar has buildup; multiple foods (chorizo mix, salsa fresca, aji sauce, habanero sauce) in the walk-in are past their expiration dates; guacamole on the line and bottles of sauces on expo are kept on time control but the time is not marked; dirty wiping cloths were not sanitized after use at the upstairs bar; ice scoop handle is touching the ice; cutting boards on storage rack are heavily scratched; bottom tiers of walk-in cooler racks and soda dispenser drip tray are rusty; walk-in cooler fan guard is dusty; racks and pipe to the left of the beer cooler have mold growth; chip holder in upstairs bar needs wiping out; interior of utensil storage pans and drawers have debris inside; wall behind the margarita maker at the upstairs bar is dirty.

Hunan Café, 9117 Staples Mill Road – The following violations were reported during a routine inspection: employee failed to change gloves and wash hands after cleaning with bleach; employee failed to change gloves and wash hands after handling raw meat; raw chicken sitting on a container of raw broccoli near wok station; temperature of chicken sitting on wok station is elevated; single-use containers are stored on a dirty counter above prep unit; fryer baskets are in disrepair; single cutting boards are heavily scratched and pitted; observed dried food debris and grease buildup on surfaces of fryer, wok, knife strip, bulk spice bin/buckets, prep counters and inside reach-in freezers; observed grease and food buildup on floor inside walk-in cooler and under wok/fryers; lighting in the walk-in cooler is not sufficient.

Kabob Place, 8906-G West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have a verifiable employee health policy; observed mold on outside of containers of dressing, pickles and olives in walk-in cooler; no food in reach-in or walk-in coolers have date marks; spray bottle of bleach is unlabeled; food is uncovered in the walk-in cooler; spices are not covered on the cook line; dirty/wet wiping cloth is not being stored in a sanitizer bucket; two metal skimmers are broken; big cutting boards on back prep table are heavily scratched and stained.

Olive Garden Italian Restaurant, 4467 South Laburnum Avenue – The following violations were reported during a routine inspection: hand sink on cook line doesn’t have paper towels; temperatures of lasagna and meatballs in lowboy hot holding unit are too low.

Puck’s Market, 435-A North Ridge Road – The following violations were reported during a routine inspection: kitchen hand sink doesn’t have paper towels or hand soap; no thermometer found inside small glass door reach-in; interior of two-door lowboy and white refrigerator are dirty.

Cracker Barrel, 4435 South Laburnum Avenue – The following violations were reported during a routine inspection: microwave on the cook line is soiled with food debris; temperatures of coleslaw, pico de gallo and roast beef are elevated; batter for frying and pancake batter are on time control with no/incorrect stickers; large plastic bins are stacked while still wet on clean dish rack; two-door hot holding cabinet in back kitchen has torn gaskets; lowboy unit on cook line and four-door reach-in cooler on service line are broken and not in use; air temperature of prep unit on service line is elevated; heavy layer of food debris greater than one day old found under drawers of refrigeration unit on cook line; floors are dirty; ceiling near chicken batter station is dusty.

Hardee’s, 4841 South Laburnum Avenue – The following violations were reported during a follow-up inspection: dish machine is measuring at 0 ppm bleach; mop sink doesn’t have a dual check valve installed; observed food debris buildup in grouting.

Parham Doctors’ Hospital, 7700 East Parham Road – The following violations were reported during a routine inspection: temperature of pans of shrimp and rice in the warmer is elevated; prep unit cutting board is heavily scratched and stained; observed grease and food buildup on fryers, ovens, under grill, inside reach-in coolers, under cook line equipment and under prep sinks.
