Chipotle Mexican Grill, 4930 West Broad Street – The following violations were reported during a routine inspection: observed an employee adjust his hat and continue to prepare food without washing hands; observed an employee take dishware out of the chemical dish machine before the cycle ended; observed food debris inside colander and multiple metal pans stored clean; temperatures of brown rice, white rice and chicken on the front line are too low because water levels are low; temperatures of super greens and shredded cheese on front line are elevated; steak on the line in back kitchen is not time marked; pans on clean rack in dish room are stacked while still wet; to-go trays on front line are not stored inverted; cutting boards throughout facility are heavily scratched; observed buildup/grease/debris on hood filters on cook line, in between and on sides of equipment, on racks, on shelves throughout, on fan guards in walk-in cooler and on juice concentrate containers on shelf; observed stains/debris/buildup on floors under and behind equipment throughout and on walls throughout; ceiling tiles and vents are dusty and stained; there is standing water on the floor in the dish room; floor in dish room has low grouting.

Buckhead’s Chop House & Restaurant, 8510 Patterson Avenue – The following violations were reported during a routine inspection: no handwash signage posted at bar; temperature of demiglace in bain marie is too low; bottle of degreaser in dish room is unlabeled; bar glassware incorrectly stored on linen; drain line and pipe under bar ice bin are dirty; vent hood above dish machine is greasy and dusty; hood filters, wall behind dish machine and ceiling tiles in dish area are dirty.

Kokonut Box, 10921-F West Broad Street – The following violations were reported during a routine inspection: can of Raid found at the mop sink; rice scoop water is not hot enough; walk-in cooler fan guard is dusty.

Waffle House, 8400 Brook Road – The following violations were reported during a routine inspection: observed an employee handle raw food and fail to wash hands before touching clean plates and ready-to-eat food; temperatures of ham, tomatoes, hashbrowns and waffle mix in the prep top/lowboy are elevated; time is not marked on eggs over cook top; sanitizer level is too high when dish machine was tested; observed food debris and grime buildup in units along cook line including gaskets; observed food splatter on cook line; hood filters are heavy with grease buildup.

McDonald’s, 1778 North Parham Road – The following violations were reported during a routine inspection: observed open sauce and fries on top of burger bin on cook line (employees are not to eat while working); time is not being tracked for cheese slices; time had expired for shredded lettuce.

Tropical Smoothie Café, 1513 North Parham Road – The following violations were reported during a routine inspection: hand sink in back is blocked; just sliced tomatoes were placed directly into make table (cool quickly prior to placing in make table); walk-in floor is very sticky.

Arby’s, 5205 Brook Road – The following violations were reported during a routine inspection: observed dried food debris on parts of the slicer; observed grease and food residue on bottles and caps stored near prep sink; temperatures of brisket, ham and turkey in top portion of prep units are elevated; there is a small fly infestation throughout the kitchen; prep unit cutting boards are heavily scratched; muffin pans are in disrepair; observed dust and grease residue on surfaces of bread racks, monitors, cables, wire racks, fan system in walk-in cooler and interior of warmer unit; observed buildup of food debris and grime on walk-in cooler floor, between front service line and prep line, on walls and in mop area; some floor tiles are missing and/or broken; tile coving near walk-in freezer is damaged.

McDonald’s, 7527 Staples Mill Road – The following violations were reported during a routine inspection: observed grease and food residue on wire baskets and pans stored on dry rack; temperature of raw bacon is elevated because it is sitting out on the prep counter; temperatures of sausage gravy (stacked and covered) and burritos (covered) in the reach-in are elevated because of improper cooling methods; in-use utensils incorrectly stored on egg cartons above grill; grease traps attached to the grill are damaged and do not catch grease and food adequately (facility is using single-service containers to catch the grease on the floor as this has been an ongoing issue for several months); there is heavy buildup of grease and food under the grills/traps; observed heavy buildup of grease, dust and grime on surfaces of fryers, grills, wire racks, fan system in walk-in cooler, inside reach-in cooler and freezers, counters, cup and wrapper holders, cables, monitors and beverage dispensers; observed heavy buildup of grease and food debris on floors under grills, fryers, in walk-in cooler, under three-compartment sink, and at drive-thru; there is food splatter on the walls.

E-Loteria Botanas & Ice Cream, 9022 West Broad Street – The following violations were reported during a routine inspection: temperature of horchata in drink machine on front line is elevated; temperature of milk in lowboy unit is elevated; temperature of tomatoes prepped at noon is elevated because they are cooling in a deep container with a lid on top; box of colas incorrectly stored on dry storage area floor; two knives incorrectly stored in between crevices of hand sink and three-compartment sink in back kitchen; boats and to-go trays are not stored inverted; cutting boards throughout the facility are heavily scratched; paint is chipping on wall on front line and on floors in walk-in cooler.

Waffle House, 7109 West Broad Street – The following violations were reported during a routine inspection: pink mold found on plastic panel of ice machine in back kitchen; temperatures of grits and sausage gravy in warmer units are too low; temperatures of cubed ham, waffle batter and country ham are elevated; observed a few dead cockroaches on floor corner of dry storage room in back kitchen; multiple refrigeration units on front line have torn gaskets and rusty racks; wooden cutting board on front line is heavily scratched; observed buildup/grease/debris on and under hood filters, in between and under equipment on cook line, on bottoms of lowboy units on front line, on bottoms of refrigeration units and on racks and shelves throughout facility; fan guards in walk-in cooler and ceiling vents and light shields throughout facility are dusty; floors throughout facility have low grouting; observed buildup/stains/debris on floors under equipment throughout and on walls throughout facility; debris found on floor of walk-in freezer; there is standing water in middle floor tiles on front line.

Olive Garden Italian Restaurant, 11829 West Broad Street – The following violations were reported during a routine inspection: open canned beverage found near salad station; observed sauce on clean knife handles and food debris on lips of clean pans; soda drip trays are rusty; one light under back hood system is out and one light in freezer is out; observed grease deposits on side surfaces of cook line equipment; back prep area racks and cooler speed racks are dirty; walk-in cooler and freezer fan guards are soiled; observed black buildup on shelves of storage rack near front end cook line; dish area hand sink leaks profusely; mop sink doesn’t have a dual check valve installed; cove molding in the produce cooler is loose from the wall; grout is low in some areas of kitchen floor; liquid (possibly beer) found on the floor in the very back of the walk-in; an abundance of cardboard is piled on the floor in the back hall to the office; hood filters over fryers have some grease buildup.

Pho Thien Phat, 6403 Rigsby Road – The following violations were reported during a routine inspection: raw chicken incorrectly stored over cooked brisket in two-door unit; slicer, grater and cutting boards are soiled; temperature of cooked pork on counter is too low; small make table is not working (temperatures of pork, sprouts and lettuce inside are elevated); a lowboy unit is not working; temperatures of eggs and cooked onions in oil are elevated; bleach container incorrectly stored on top of noodle box; no thermometer found in display or in glass door units; clean pans incorrectly stored on grease interceptor (store in a clean area); a plastic bag is being used to hold the dish machine screen in place; top of grease interceptor, lower shelves, handles to mop sink faucet and interior of upright freezer are dirty; mop sink faucet is dripping; garbage/outside area is dirty (many cigarette butts found on the ground); dish room wall has mildew; exterior of outside freezer door, threshold between kitchen and back room, mop room floor and walls, and cook line floor are dirty; entire facility floor is collapsing; some floor tiles are broken.
