Dogwood Terrace, 10300 Three Chopt Road – The following violations were reported during a routine inspection: final rinse cycle of dish machine doesn’t reach the required temperature; observed debris greater than one day old inside mixing bowl, on scoops, tenderizers and a knife stored clean, and on slicer blade in dry storage room; temperature of mashed potatoes on hot line in kitchen is too low; temperature of yogurt prepped one hour prior in reach-in unit is elevated; air temperature of this unit is elevated; multiple silverware knives in containers on cart are not stored inverted; metal pans and containers on clean rack are stacked while still wet; boxes of to-go cups and trays incorrectly stored on dry storage room floor; one light bulb is out at hood system on cook line; some racks in dry storage area are rusty; multiple cutting boards on cook line and on shelf in prep area are heavily scratched; facility couldn’t provide high temperature test strips/devices for high temp dish machine in kitchen; observed buildup on racks in dry storage and in walk-in cooler, in drawers throughout facility, and in container for clean ice scoop; observed debris/buildup on shelves throughout, on floor corners in dry storage rooms, and in between and under equipment on cook line; paint is chipping on wall in mop sink room; observed low grouting on floors in dish area; ceiling vents in service station in kitchen are stained and dusty; walls in dish area are stained.

Our Lady of Hope Health Center, 13700 North Gayton Road – The following violations were reported during a routine inspection: red cutting board on storage rack is heavily scratched and others are stained; sides of cook line equipment are greasy; observed residue on fridge handles; walk-in cooler fan guard is dusty; rack next to stove and walls at three-compartment sink and under rinse sink table are dirty; observed buildup of grease on floor behind cook line; ceiling over cook line is yellowed; one ceiling tile over dishwasher is cracked and another is bubbly; grout is low under the dish table; cook line floor tiles are cracked; coving tile near three-compartment sink is broken/missing.

Beth Sholom Gardens, 2001 Lauderdale Drive – The following violations were reported during a routine inspection: dish and front hand sinks are missing paper towels; observed buildup on lip of ice machine kick plate; slicer surfaces and crevices are dirty; rice cooled this morning is at the wrong temperature; wet wiping cloth is not being stored in a sanibucket between uses; racks in meat reach-in and both walk-ins are very rusty; cutting boards are dark and discolored; walk-in fan guard is dusty; freezers have a lot of ice buildup; front hand sink is leaking; dishwasher leaks heavily while filling; paint is peeling/bubbling from the wall under the dishwasher and over the wall shelf near the cook line; walls behind work tables are dark and mildewy.

Westminster-Promenade Canterbury, 1600 Westbrook Avenue – The following violations were reported during a routine inspection: didn’t observe an asterisk next to items that can be undercooked (hamburger, salmon); didn’t observe a thermometer in refrigerated drawers throughout cook line.

Henrico Western Government Center—Employee Cafeteria, 4301 East Parham Road – No violations reported during a routine inspection.

Knights Inn & Suites, 7201 West Broad Street – The following violations were reported during a routine inspection: no handwash signage is posted at employee hand sink near front desk check-in area; hand sinks in employee and customer restroom near front lobby area don’t have hand soap; employee restroom near front desk check-in area doesn’t have paper towels and the hand sink is not in working order; facility is not sanitizing food contact surfaces in dining room; bottle of air freshener incorrectly stored in between box of sugar and cereal; boxes of single-service cups incorrectly stored on floor in front desk check-in storage area; single-service bowls in dining room are not stored inverted; facility couldn’t provide chlorine or quaternary ammonium sanitizer test strips; observed holes in the wall and chipped paint in dining room and in employee restroom near front check-in area; observed debris in floor corners in dining room.

Best Western Executive Hotel, 7007 West Broad Street – The following violations were reported during a routine inspection: observed an employee continuously not wash hands before handling utensils and before handling and transferring foods from staff room to dining room; observed an employee rinsing utensils at hand sink in dining room (hand sinks are for handwashing only); hand sink in dining room doesn’t have hand soap, paper towels or any handwash signage; quaternary ammonium sanitizer level is too high at three-compartment sink; fruits on check-in counter are uncovered; plates and bowls on counter in dining room are not stored inverted; box of cold cups incorrectly stored on floor in staff room; observed buildup of debris on racks in staff room and on shelves in dining room; water is continuously running from hand sink in dining room; ceiling tiles in staff room where refrigeration units and cookware are located are not easily cleanable; observed buildup on floor corners and behind equipment in staff room where equipment and refrigeration units are located.

Firehouse Subs—White Oak, 4501 South Laburnum Avenue – The following violations were reported during a routine inspection: the certified food protection manager certificate is expired; temperatures of turkey and ham sliced this morning are elevated; meat is cooling in reach-in cooler, which is frequently opened, and bags are stacked on top of each other; two buckets of pickles incorrectly stored on the floor; containers of pickles are open without a cover on the cook line; observed food debris on the floor throughout the cook line.

Killa Dillas, 6114 Lakeside Avenue – No violations reported during a routine inspection.

Q Barbecue, 1070 Virginia Center Parkway – The following violation was reported during a routine inspection: spray bottles in dish area are not labeled with the chemical name.
