Breaker’s Sports Grille, 9127-V West Broad Street – The following violations were reported during a follow-up inspection: did not observe a certified food protection manager certificate; degreaser spray incorrectly stored next to oils on top shelf on cook line; quaternary ammonium sanitizer level is greater than 400 ppm at three-compartment sink in smoking section of bar; beers and drinks incorrectly stored on the floor throughout dry storage and older bar area next to kitchen; box of to-go trays incorrectly stored on the floor in dry storage area.

Mezeh Mediterranean Grill, 11710 West Broad Street – The following violations were reported during a follow-up inspection: gasket on lowboy across from the line is torn; handles on this unit are missing; center line rack is rusty; observed patches of low grout in the kitchen.

AB & R Catering, LLC, 601 North Parham Road – No violations reported during a routine inspection.

Jersey Mike’s Subs, 8654 Staples Mill Road – The following violations were reported during a routine inspection: observed an employee wearing bracelets and rings; observed grill cook handle bracelets and fail to wash hands before continuing food prep; temperature of sliced tomatoes in the walk-in cooler is elevated; observed dust and debris buildup on fan grate in walk-in cooler and on portable fans; floor tiles at back door are missing and broken.

Texas de Brazil Churrascaria, 11800 West Broad Street – The following violations were reported during a routine inspection: old stickers found on clean pans; soda gun nozzles at bar are dirty; observed pink film inside ice machine; spray bottle of cleaner at the bar is unlabeled; observed small flies in the meat cooking area (seem to be coming from under the work table); cutting boards are scratched and stained; a few lexan pans are cracked/chipped; observed buildup in fridge gaskets/grooves and on storage and dish crates; top expo shelf and surfaces of bar fridges are soiled; dish rack has a powdery residue; blood is pooled on the bottom tier of meat lowboys; salad bar cabinets are discolored inside; floors under equipment are soiled; observed debris in ceiling lights; walls at the prep sink and near the walk-ins are dirty; ceiling tiles are a little discolored; coving tiles near the bulk ice machine and at the bar are falling off/missing.

Tony’s Italian Restaurant, 3409 Cox Road – The following violations were reported during a follow-up inspection: no handwash signage posted at hand sink in kitchen; multiple foods (canned tomato sauce, raw beef, etc.) incorrectly stored on the floor in and next to the walk-in cooler; reach-in units throughout facility have torn gaskets; air temperature is elevated in beer cooler and lowboy unit at pizza station; cutting boards hanging near dish area are heavily scratched; facility couldn’t provide quaternary ammonium sanitizer test strips.

BJ’s Restaurant & Brewhouse, 12010 West Broad Street – The following violation was reported during a follow-up inspection: observed small flies in kitchen and at bar.

Tier 7 Eatery, 4600 Cox Road – No violations reported during a routine inspection.

Burgerworks, 10321 West Broad Street – The following violations were reported during a follow-up inspection: did not observe written times for blanched fries stored out at room temperature and for parmesan cheese stored in ice bath; observed an opening on the bottom of exit door where dry storage is located; racks for chemical and clean dishware storage are rusty; there is a gap greater than one-inch in between hood filters on cook line; reach-in freezer, walk-in cooler and lowboy unit on cook line have torn gaskets; observed low grouting on floors throughout facility; one ceiling tile is missing in dry storage area where walkway to exit is located.

Picnic RVA, 7311 Capehart Road – No violations reported during a routine inspection.

Tres Machos, 2313 Westwood Avenue – The following violations were reported during a follow-up inspection: temperature of refried beans made the previous day is elevated; temperature of mashed potatoes sitting out on counter is elevated.

Backwoodz BBQ, 400 West Williamsburg Road – The following violations were reported during a routine inspection: hand sink isn’t working so employees are having to wash hands in the prep sink; temperatures of pulled pork and pork butt prepared yesterday are elevated; temperatures of brisket, wings, ribs and pulled pork in hot holding cabinet are too low; no date marks found on meat, mac n cheese or coleslaw; improper cooling methods observed (pans are closed and stacked); reach-in freezer is broken; there is a leak under the three-compartment sink.

Firebirds Wood Fired Grill, 11800 West Broad Street – No violations reported during a follow-up inspection.

Chicago’s Pizza with a Twist, 3641-F Cox Road – The following violations were reported during a follow-up inspection: hot water at hand sink in dish area doesn’t reach the required temperature; chemical dish machine is not in working order; cutting boards throughout the facility are heavily scratched and/or stained; reach-in freezer, reach-in unit in back of facility and walk-in cooler have torn gaskets; racks in dish area and in walk-in cooler are rusty; observed a gap in between hood filters above convection oven; one light bulb is out at hood above convection oven; one ceiling light is out in the dish room.

Chicken Salad Chick, 9631 West Broad Street – The following violations were reported during a follow-up inspection: there is 0 ppm quaternary ammonium sanitizer at three-compartment sink; temperatures of pasta, tomatoes, chicken salad, broccoli salad and turkey in prep top and lowboy unit next to kitchen entrance door are elevated; air temperature of this unit is elevated; temperature of lettuce prepped 30 minutes prior is elevated; improper cooling method observed for chicken salad (cooling with lid on); observed 2-3 small flies near three-compartment sink; metal pans are stacked while still wet on clean rack in dish area; cutting board on sandwich unit on line is heavily scratched; air temperature of sandwich unit is elevated; observed low grouting on floors throughout kitchen.
