Pho Sai Gon, 10190 West Broad Street – The following violations were reported during a routine inspection: observed several open drink cups stored on shelves above and with food and single-use items; observed an employee slicing jalapenos with bare hands; hand sink in kitchen doesn’t have hand soap; mildew buildup found on interior and exterior surfaces of blender container; temperature of cooked rice noodles in warmer is too low because the warmer was shut off after lunch service; temperatures of tofu and short ribs in walk-in cooler are elevated; several foods (rice noodles, chicken) incorrectly cooling in tightly covered, deep containers; observed small flies at the bar; single and prep unit cutting boards are heavily scratched and stained; observed food debris on surfaces of racks, walk-in cooler fan grates, on counters, and inside keg fridge at bar; observed grease and food buildup on floor under cook line, at bar, and in walk-in cooler.

Marco’s Pizza, 9681 West Broad Street – The following violations were reported during a routine inspection: didn’t observe a certified food protection manager certificate; cutting boards on clean rack in dish area and on sandwich unit are heavily scratched and moldy; observed debris on racks throughout kitchen and in utensil container on clean rack; mops are not being hung to dry in mop sink area; floors throughout kitchen have low grout.

Tip Thai, 8131 Staples Mill Road – The following violations were reported during a routine inspection: light fixture/bulbs in dry storage room are not shielded; a brighter bulb needs to be installed in the walk-in cooler.

Taco Bell, 11450 West Broad Street – The following violations were reported during a routine inspection: gasket in reach-in freezer is torn; walk-in cooler fan guards are dusty; shelving on the prep line and castors on the fryers are greasy; observed debris on shelving on the prep line; grout and ceiling tile linings are dirty; wall at the mop sink is stained.

Fairfield Middle School, 5121 Nine Mile Road – The following violations were reported during a routine inspection: walk-in cooler fan grates are heavily rusted; room fan in Café D is covered in dust.

Laburnum Elementary School, 500 Meriwether Avenue – The following violation was reported during a routine inspection: observed a slow leak in the line connected to the steamer.

Chipotle Mexican Grill, 1070 Virginia Center Parkway – The following violations were reported during a routine inspection: observed an employee chewing gum while preparing orders; observed small flies in back kitchen near ice bin; hand sink in women’s restroom has a leak; lights are out under the hood; one light is out in back kitchen; observed low grouting by prep sink and in dish area.

Glen Allen Elementary School, 1101 Mill Road – No violations reported during a routine inspection.

Donahoe Elementary School, 1801 Graves Road – No violations reported during a routine inspection.

Fair Oaks Elementary School, 201 Jennings Road – The following violations were reported during a routine inspection: temperature of pizzas is elevated because they were not returned to refrigeration promptly after preparation; temperatures of breakfast sandwiches and cinnamon sticks are elevated because they are cooling at room temperature; a light is out in the hood.

Freeman High School, 8701 Three Chopt Road – The following violations were reported during a routine inspection: milk crates incorrectly used to prop up table top inside walk-in freezer; hot water isn’t available at new hand sink in dish area; floor by ice machine pipes is dirty; observed construction debris in floor drain in dish room and on floor edges in dish area; floor corner to left of walk-in cooler has debris; there is exposed spray insulation around pipes under three-compartment sink and by steamer/kettles; a chunk of wall is missing in dish room.

Starbucks Coffee, 8631 Quioccasin Road – The following violations were reported during a routine inspection: back hand sink doesn’t have paper towels; single-service cups are overfilled in the dispenser; facility doesn’t have heat tapes or a max registering thermometer for monitoring sanitization of dish machine; observed coffee spillage inside cabinets; dump sink on front line has a loose faucet; floors are soiled by front hand sink and by dish machine.

Dragon Treats Dessert Café, 1404 North Parham Road – The following violation was reported during a routine inspection: no date mark found on cut melon.

Firebirds Wood Fired Grill, 11800 West Broad Street – The following violations were reported during a routine inspection: observed an employee use a cleaning rag with gloved hands and then go back to preparing food without changing gloves/washing hands; there is no handwash signage in women’s restroom; in-use dishwasher is not reaching the required sanitizing temperature; can opener, soda gun nozzles at bar and ice machine are dirty; slicer has residues on blade and in grooves of other surfaces; temperatures of three different pans of pasta in refrigerated drawers are elevated; temperatures of roast beef and au jus in lowboy unit are elevated; time is not being tracked for two items on time control (buffalo sauce containing unsalted butter and whipped butter); lowboy unit next to the grill doesn’t have an ambient air thermometer; observed a few flies in the kitchen along with a fly strip over the baking prep table; the handle of the ice scoop at the bar is touching ice; drink station fridge racks and beer cooler racks are rusty; cutting boards throughout the kitchen are discolored and/or scratched; cook line equipment has grease deposits; walk-in cooler fan guards are dusty; dishwasher and its hood vent are coated in debris/dust; potato speed rack is dirty; observed spills inside a couple bar coolers; mop sink doesn’t have a dual check valve installed; floors under equipment in the kitchen and at the bar are soiled; observed mildew on dish area walls; walls near coving are stained; beer is spilled in the back corner of the beer cooler; multiple floor tiles are cracked throughout kitchen (a couple are missing entirely); back prep hood filters are dusty and dirty.

Heavenly Burgers, 608 East Nine Mile Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; observed food debris inside microwave; used ashtray found on counter in kitchen; observed dried food debris and grease inside and on handles of back refrigerators and on walls; floors are in disrepair; grease trap is heavily rusted.

Panda Express, 1782 North Parham Road – The following violations were reported during a routine inspection: the spill kit on site doesn’t have disinfecting instructions in procedures; floor drain under drink station is soiled.

Highland Springs High School, 200 South Airport Drive – The following violations were reported during a routine inspection: observed dried food debris on slicer; hamburgers in walk-in cooler are past their expiration date; observed spilled milk and dried food on walk-in cooler floor; there is soap on the floor near multiple hand sinks; wet mop is not being hung to air dry.

Highland Springs Elementary School, 600 Pleasant Street – The following violation was reported during a routine inspection: the ceiling is leaking above three-compartment sink and prep table.

Tuckahoe Middle School, 9000 Three Chopt Road – The following violations were reported during a routine inspection: spray hose attachment in Café 2 is dirty; mop sink doesn’t have a backflow prevention device installed; door to restroom in Café 2 was left propped open; Café 2 restroom doesn’t have a covered trash can; walk-in fan grills are dusty; observed mold on condensate line from condenser.

Twin Hickory Elementary School, 4900 Twin Hickory Lake Drive – The following violations were reported during a routine inspection: two cans of fruit in dry storage room are badly dented; observed dust buildup on top of oven/steamer and on fan grate system in walk-in cooler.
