Rock Bottom Restaurant & Brewery, 11800 West Broad Street – The following violations were reported during a routine inspection: did not observe a ServSafe certificate during inspection; facility doesn’t have procedures for cleaning up vomit; facility couldn’t provide parasite destruction documentation from the salmon provider; observed leftover stickers on clean pans and food debris on one lexan pan; inside of ice machine is moldy; one nozzle at the bar has buildup; temperature of queso is too low; multiple spray bottles of peroxide cleaner in the kitchen are unlabeled; found a can of Raid in the office (this pesticide cannot be used/stored in a food establishment); red cutting board at the bar is heavily scratched; chemical rack is rusty; observed grease deposits on sides of cooking equipment; meat cage in the walk-in and dishwasher are dirty; monitors and receipt printer are dusty; first cook line hand sink is slow to drain and the faucet is leaking; second cook line hand sink has the cold water turned off (possibly due to plumbing repairs in progress); observed a lot of water under the drink station but unable to determine if there is a leak; ceiling tiles, walls and floor drain under drink station need cleaning; wall at dish area has mildew; hood filters are dirty.

Starbucks Coffee Company, 4101 Dominion Boulevard – The following violations were reported during a routine inspection: temperature of milk in drive-thru lowboy is elevated because it is being taken in and out frequently; reach-in freezer has a torn gasket; facility doesn’t have thermo labels for the high temp dishwasher.

Varina Elementary School, 2551 New Market Road – No violations reported during a routine inspection.

Pocahontas Middle School, 12000 Three Chopt Road – The following violations were reported during a routine inspection: reach-in unit next to warmers has a torn gasket; observed low grouting on floors throughout the kitchen.

Echo Lake Elementary School, 5200 Francistown Road – The following violation was reported during a routine inspection: cutting board at the front service line has chipped edges and corners.

Short Pump Elementary School, 3425 Pump Road – No violations reported during a routine inspection.

Nuckols Farm Elementary School, 12351 Graham Meadows Drive – The following violations were reported during a routine inspection: slicer cover has debris on it; walk-in cooler fan guards are dusty.

Arby’s, 5220 Williamsburg Road – The following violations were reported during a routine inspection: temperatures of sliced tomatoes, gyro meat, corned beef and roast beef in prep unit and prep unit lowboy are elevated; prep unit is not working properly; observed many flies and small flies in the kitchen; there is 0 ppm sanitizer in bucket for wiping cloths; observed leak at back of ice machine; dumpsters outside were left open.

Whisk, 8308 Staples Mill Road – The following violations were reported during a routine inspection: an open cup of coffee incorrectly stored on prep table in back kitchen; spreading utensils incorrectly stored in sanitizer solution next to prep unit.

Harvie Elementary School, 3401 Harvie Road – The following violation was reported during a routine inspection: multiple boxes of food incorrectly stored on floor of walk-in freezer.

Ridge Elementary School, 8910 Three Chopt Road – The following violation was reported during a routine inspection: floor drain under three-compartment sink is dirty.

Seven Pines Elementary School, 301 Beulah Road – The following violation was reported during a routine inspection: mop sink doesn’t have a dual check valve installed.

Crestview Elementary School, 1901 Charles Street – The following violations were reported during a routine inspection: plastic square single-service cups are stored unprotected in the storeroom; spray hose in dish room and shelf above mop sink are dirty; dumpster was left open; there is a small open pipe/conduit in the dish room; ceiling panels around the dish room hood are not smooth and cleanable and one has a crack; mop sink is dirty.

Mexico Restaurant, 11621 West Broad Street – No violations reported during a follow-up inspection.

Buffalo Wild Wings Grill & Bar, 7801 West Broad Street – The following violations were reported during a routine inspection: bar hand sink doesn’t have hand soap; there is 0 ppm sanitizer in the dish machine; observed buildup of mildew on ice machine; observed large flies in back kitchen and small flies at the bar; mop sink doesn’t have the proper backflow prevention device installed; drains throughout the kitchen have buildup; dish area and chemical storage room need cleaning.

My Empanada, 1421 Blue Jay Lane – The following violations were reported during a routine inspection: did not observe a certified food protection manager certificate during inspection; temperatures of chicken empanada, beef empanada and breakfast empanada in warmer are too low; three chemical spray bottles are unlabeled; reach-in unit in back storage area has torn gasket and missing handle; observed slight buildup in microwave in kitchen; ceiling tiles in dry storage area are not easily cleanable; observed low grouting on floors in dish area.

Hai Ky Mi Gia, 1601 Willow Lawn Drive – The following violations were reported during a follow-up inspection: temperatures of three buckets of beef pho in reach-in and two buckets of beef pho in walk-in are elevated; temperature of bean sprouts is elevated; no date marks found on pho or brisket.

Starbucks Coffee Company, 3820-A Gaskins Road – The following violations were reported during a routine inspection: did not observe a certified food protection manager certificate during inspection; coffee kettle found in hand sink in dish area (hand sinks are for handwashing only); cups are blocking access to hand sink on front line; temperatures of two cans of whipped cream and milks are elevated; middle reach-in unit has torn gasket; observed debris under espresso machines on front line; observed slight debris in utensil container and on racks in dry storage area.

Shawarma Bistro, 12422 Gayton Road – The following violations were reported during a routine inspection: lemon wedges incorrectly stored in ice that is being used for customers’ drinks; utensils incorrectly stored in water; ice scoop handle is touching ice.
