Samurai Sushi Restaurant, 11391-A Nuckols Road – The following violations were reported during a routine inspection: person in charge is unable to identify the “big six” foodborne illnesses and their associated symptoms; facility doesn’t have a written employee health policy in place; observed an employee handle sanitizer bucket/bleach and fail to wash hands before continuing food prep; person in charge could not provide parasite destruction letters from distributors; observed an employee washing gloves to re-use; ice machine interior has pink and black residue; cooked sweet potato in lowboy has exceeded six-hour timeframe; temperature of fried rice in warmer is too low; temperatures of cream cheese, fried crab and sprouts in prep and lowboy units are elevated; food being prepared and held for more than 24 hours is not being date marked (cooked sweet potato, chicken, noodles); time for sushi rice is not being tracked; foods on the menu do not contain asterisks that link to the disclosure statement; burn spray and other personal medications incorrectly stored on a shelf with food; observed improper cooling methods being used; bulk ingredient bins are unlabeled; menu states real crab but the facility is using imitation crab; food incorrectly stored on walk-in cooler and freezer floor; wet wiping cloths are not being stored in a sanitizer solutions; wiping cloths are heavily soiled; rice scoops incorrectly stored in standing water; prep unit cutting boards are heavily scratched and stained; wire storage racks are rusty; containers and lids are broken; observed dried food debris, grease and grime buildup on grill, fryers, broiler, portable fan, utensil storage bins and wire storage racks; observed grease and dirt buildup on floors, walls and ceiling panels.

Shagbark Restaurant, 4901 Libbie Mill East Boulevard – The following violations were reported during a routine inspection: observed an employee handle ready-to-eat tomatoes with bare hands; cook line hand sink doesn’t have paper towels; dish area hand sinks don’t have hand soap; shellstock tags are not maintained in chronological order; grime buildup found inside ice machine; temperature of cooked carrots on the line is elevated because the container is overfilled; house made bread and butter pickles are past their expiration date; mop sink doesn’t have a dual check valve installed; ceilings and hood vents are dusty.

Western Government Center’s Employee Cafeteria, 4301 East Parham Road – No violations reported during a routine inspection.

Bojangles, 8823 Staples Mill Road – The following violations were reported during a follow-up inspection: observed grease and food residue on containers, pans and utensils stored on wire racks above the three-compartment sink; temperatures of chicken breast on front counter and tenders on front service line unit are too low; observed male employees with long beards not wearing beard guards; prep unit cutting board is heavily scratched and stained; observed heavy buildup of grease, food residue, flour and grime on prep units, ovens, fryers, touchpads, tea makers, hot water dispensers, wire racks, all rolling carts, prep tables, wrapper holders, warmer units and inside all reach-in coolers and freezers; there is heavy mold buildup inside hand sinks and on fan grate and storage racks in walk-in coolers; observed heavy buildup of flour and grime on floors, walls and ceiling panels/vents in kitchen and inside all walk-in coolers.

Chen’s Chinese Restaurant, 11361 Nuckols Road – The following violations were reported during a routine inspection: observed employee rinsing their hands at three-compartment sink (handwashing should be done at hand sinks); observed an employee come from outside to continue food prep without washing hands; temperatures of three baskets of eggrolls prepared the previous day are elevated; containers of spices and oils are unlabeled; rice scoops incorrectly stored in standing water; prep unit cutting boards are heavily scratched and stained; observed dried food debris and grease on fryers, grill, wire storage racks in walk-in cooler, floor mixer and bulk ingredient bins; observed dried food debris and grime on floors throughout the kitchen.

Artisans by Alyah Snacks Bakery & Café, 3302 Williamsburg Road – The following violation was reported during a routine inspection: facility doesn’t have a certified food protection manager.

Chipotle Mexican Grill, 5300 Wyndham Forest Drive – The following violation was reported during a routine inspection: single green cutting boards are heavily scratched.

Jason's Deli, 1700 Willow Lawn Drive – No violations reported during a follow-up inspection.

Muse Paintbar, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: facility couldn’t provide certified food protection manager certificate; facility doesn’t have written procedures for cleaning up vomit/diarrhea; hummus and salsa are past their expiration dates; two bottles of blue soap are unlabeled; facility doesn’t have a metal-stemmed thermometer; plastic utensils are not stored inverted; lids to condiment cups are unprotected; hand sink has rust.

MOD Pizza, 8671 Quioccasin Road – The following violations were reported during a routine inspection: cutting boards are dirty; small flies found under drink station; boxes of food incorrectly stored on walk-in freezer floor; clear lids are stored unprotected; several areas are dirty and need cleaning (make table door gaskets, reach-in interior, can opener holder, table edges, shelf under can opener, dough express in back, walk-in floor, mop sink, and floor under dish table/dish machine).

Bowlero—West End, 8037 Shrader Road – The following violations were reported during a routine inspection: bar hand sink doesn’t have soap or paper towels; observed grease and food residue on containers, lids and utensils stored on dry rack above three-compartment sink; wires on fryer baskets are bent; prep unit cutting boards are scratched and stained; observed dried food debris, grease and grime buildup on fryers, grill, wire storage racks, condiment shakers, chili and cheese dispensers, dish dry rack, utensil storage bins, counter/prep surfaces and inside reach-in coolers and freezers; wall is damaged near prep unit entry door; observed food debris, grime and dust on floors, walls, ceiling vents and panels.

Taco Bell, 4388 South Laburnum Avenue – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit/diarrhea; temperature of shredded cheese in prep top and on prep line is elevated; plastic trays incorrectly stacked wet; ceiling is leaking in several places in dry storage area near back door, near three-compartment sink and at the office entrance; lights are out above three-compartment sink; food debris found in grout outside three-compartment sink; wet mop is not being hung to dry.

Virginia Barbeque Company, 6920-E Lakeside Avenue – The following violations were reported during a routine inspection: temperature of ribs in the prep unit is elevated (they may have been left out too long during prep); observed mold buildup and peeling paint behind three-compartment sink.

Rick’s Rock Café, 1500 Eastridge Road – The following violations were reported during a routine inspection: facility couldn’t provide certified food protection manager certificate; facility doesn’t have written procedures for cleaning up vomit/diarrhea; slime found inside the ice machine; cooked mac and cheese is not date marked; wrong dates are marked on bags of turkey and mushrooms; single-use bags are being reused; legs to bar support are made of bare (unsealed) wood; shelf incorrectly lined with wax paper; facility doesn’t have test strips for the sanitizers being used; drip trays to equipment are dirty; hot faucet at three-compartment sink doesn’t work; threshold to walk-in is in disrepair; spray foam is exposed under two-compartment sink; floor by fryer is dirty.
