Charles’ Kitchen, 10835 West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit/diarrhea; a drink without a top found on the prep unit; raw shell eggs incorrectly stored above ready-to-eat food in back reach-in; salmon incorrectly stored above a case of veggies in front reach-in; dishes stored on the clean dish rack have crumbs and sauce residues; temperature of meat patties in the pastry case is too low; some fridge racks are rusty; back kitchen light is missing its protective shielding.

El Torito Mexican Restaurant and Grill, 5600 Williamsburg Road – The following violations were reported during a routine inspection: facility doesn’t have a verifiable employee health policy; raw beef incorrectly stored over ready-to-eat diced tomatoes in prep unit; inside ice machine has dirt and grime buildup; soda gun nozzle at the bar has brown buildup inside; temperature of a bin of beef tips prepared yesterday is elevated; beef tips prepared yesterday are not marked with the date prepared; temperatures of big buckets of salsa, sliced tomatoes and ranch sauce in three-door reach-in cooler are elevated; chemical bottles incorrectly stored next to beer in outdoor dry storage area; drinks and apple cider vinegar incorrectly stored on the floor; forks and ice buckets are not stored inverted; large plastic bins are stacked wet; test strips for the three-compartment sink are soaked through and no longer work to test sanitizer levels; observed food debris and food splatter more than one day old in bottom of refrigeration units; trash is being piled on dumpsters in refuse area.

Chicken Salad Chick, 9631 West Broad Street – The following violations were reported during a routine inspection: employee drink incorrectly stored inside of hand sink next to mop sink; tea nozzles in self-serve area of dining room have buildup; observed food debris on scoops, slicers and multiple utensils stored clean on rack; temperatures of lettuce and cheeses on the line are elevated; quaternary ammonium sanitizer level at three-compartment sink is too high; temperature of chicken salad prepped this morning is elevated; temperature of chicken salad in reach-in unit is elevated because it is cooling in deep containers with lids, stacked on top of one another; observed multiple small flies at drain under three-compartment sink; metal pans are stacked wet on clean rack in dish area; cutting board on sandwich unit is heavily scratched; walk-in cooler fan guard is dusty; observed dust/debris on racks throughout kitchen and on racks in walk-in cooler; observed debris outside of pans stored clean on shelves; trash and debris are on the floor under and behind equipment in kitchen; there is low grouting on floor throughout kitchen.

Burger King, 8081 Brook Road – The following violations were reported during a routine inspection: observed buildup on shield and sides of ice machine; temperature of milk in under-counter unit up front is elevated because the unit door was left open; lettuce and tomatoes sitting out at room temperature are not being tracked for time; bags and an open box of cups are incorrectly stored on the floor; observed buildup of grease on and under fryers; grime is building up in hand sinks and on handles of units; a container for employee food is moldy; floor drains and floor area by oil tank are soiled; there is standing dirty water in multiple areas.

JULY 13

Rapid Pizza, 1592 Mountain Road – The following violations were reported during a routine inspection: temperature of marinara sauce in hot holding unit is too low; cigarettes and WD-40 incorrectly stored near food; three spray bottles on bottom shelf by hand sink are unlabeled.

McDonald’s, 4423 South Laburnum Avenue – The following violations were reported during a routine inspection: observed a layer of biofilm buildup on ice machine; temperature of Canadian bacon on the line is elevated; sausage gravy incorrectly cooling in covered container with other products stacked on top; sweet tea and boxes of biscuits incorrectly stored on the floor; an employee with long hair is not wearing a hair restraint while prepping food; wet wiping cloth found in front hand sink (hand sinks are for handwashing only); platters, platter lids, a gravy platter and cinnamon roll boxes are not stored inverted; soiled/greasy cardboard found under the fryer.

Red Salt, 12221 West Broad Street – The following violations were reported during a routine inspection: employee beverage at the serve station doesn’t have a lid; raw shell eggs incorrectly stored next to ready-to-eat foods in the walk-in cooler; observed old sticker residues on some lexan pans; steel cup on dish rack is sticky; breading and garlic butter is on a time control policy but the time isn’t being tracked; roasted potatoes were placed in the prep top to cool raising temperatures of foods next to them; tempura shrimp incorrectly cooling while piled high on a sheet pan; boxes and pans of food incorrectly stored on the freezer floor; to-go containers on the cook line are not stored inverted; cutting boards on the line and at the bar have deep scratches; wire storage racks are getting rusty; shelving unit used to hold sushi rice is in poor repair; two lights burned out under the hood system; walk-in cooler door handle is broken/missing; fan guard in the freezer is broken; blender surfaces are dirty; observed debris on top of dishwasher; walk-in cooler fan guards are dusty; observed buildup in crevices of soda machine and around nozzles; bar hand sink has a leaky faucet; grout is worn down and dirty throughout the kitchen; cook line floor under equipment is greasy and soiled; observed debris on floor under sushi fridges; floor drain in kitchen needs cleaning; floor drain at the bar is moldy.

Rose Marie Inn, 8923 Patterson Avenue – The following violations were reported during a routine inspection: did not observe a certified food protection manager certificate during inspection; observed an employee washing hands at three-compartment sink (use hand sink only); employee restroom doesn’t have paper towels or any handwash signage; hot water at bar hand sink doesn’t reach the required temperature; hand sink at bar doesn’t have soap; raw breakfast sausage incorrectly stored next to ham in reach-in unit; quaternary ammonium level at three-compartment sink is at zero; facility couldn’t provide quaternary ammonium test strips; black mold found on back of ice machine; observed buildup on soda gun and holder at bar area; temperature of chicken bake in reach-in unit is elevated because it is covered with saran wrap; temperatures of turkey and pastrami in prep unit and temperatures of pasta, raw ground beef, marinara and ham in reach-in unit are elevated; no dates marked on soup and two packages of ham in reach-in unit; menu doesn’t have an asterisk next to eggs; liquid shortening incorrectly stored next to Pinesol and other chemicals; isopropyl alcohol incorrectly stored in between gravy mix and balsamic vinegar on shelf; fish incorrectly thawing in reduced oxygen packaging; drink, beer boxes and shortening incorrectly stored on floor; observed grease/debris/buildup on hood filters, in between and under equipment on cook line, on floors and walls at cook line, on shelves and racks throughout facility, on fans throughout kitchen, inside microwave and on top of ice machine; cold water isn’t working at hand sink in kitchen; walls throughout facility have stains.

JULY 18

Ray’s Other Place, 3061 Lauderdale Drive – The following violations were reported during a routine inspection: dry storage rack is rusty; gasket in two-door reach-in freezer is torn; chest freezer in the dish area has a broken lid with exposed insulation; wall behind the dishwasher has mildew.

Virginia Crossings Hotel & Conference Center, 1000 Virginia Center Parkway – The following violations were reported during a routine inspection: observed mildew buildup on shield of ice machine; potatoes, chicken and cooked veggies made a few days ago are not date marked; there are items pulled from the freezer that haven’t been re-dated; temperature of jalapenos in hot holding unit is too low; observed ice buildup on two-door freezer downstairs and in walk-in freezer; lowboy unit on cook line and lowboy unit at bar have elevated temperatures; observed buildup on racks in units; observed grease buildup on sides of fryers and on pipes behind cook line; fans in walk-in are dusty; paint is chipping in mop room and banquet kitchen; coving tiles are missing throughout facility.

JULY 19

Qdoba—Short Pump, 11282 West Broad Street – The following violations were reported during a routine inspection: pair of scissors in the prep area were stored with residue on them; temperature of pinto beans in the hot box is too low; wire racks in the kitchen are rusty; casters on the cook line are greasy; walk-in cooler fan guards are dusty; observed white buildup in grooves of walk-in cooler racks.

Five Guys Burgers & Fries, 12210 West Broad Street – The following violation was reported during a routine inspection: temperature of sliced tomatoes on the line is elevated.

RVA Pizza, 9742 Gayton Road – The following violations were reported during a routine inspection: did not observe a certified food protection manager certificate during inspection; observed an employee washing hands at three-compartment sink (use hand sink only); hand sink next to three-compartment sink doesn’t have paper towels; observed food debris on slicer in back kitchen and inside strainer hanging clean above three-compartment sink; temperatures of marinara, sausage and cheese in prep top unit are elevated; nail polish remover and WD-40 incorrectly stored next to cups/plates and flour; quaternary ammonium level at three-compartment sink is too high; ladles, spatulas and to-go trays are not stored inverted; cutting boards throughout facility are heavily scratched; air temperature of both lowboy units next to convection oven is elevated; racks in walk-in cooler are rusty; fryer door on cook line needs repair; lowboy unit on left has torn gasket; observed debris on shelves and racks throughout facility; fan guards in walk-in cooler are dusty; observed grease/debris/buildup in between and on sides of equipment, on light bulb covers, and above and below hood filters on cook line; observed debris on top of canned foods in walk-in cooler, inside microwave and on rack near walk-in cooler; there is a leak coming from under the faucet at three-compartment sink; walls and floors, especially throughout cook line and behind/under equipment, have grease/buildup/stains; ceiling vent above three-compartment sink is dusty; ceiling light bulbs above prep table aren’t working; observed low grouting on floors throughout kitchen; ceiling tiles have holes and chipping paint in back kitchen.

America’s Best Wings, 4730 Finlay Street – The following violations were reported during a follow-up inspection: temperature of nacho cheese is too low; temperature of milk near batter station is elevated; batter, bowls for sauce and rice scoop are on time control without a written policy; one live adult German cockroach found at mop sink; mop sink faucet handles are leaking; mop sink doesn’t have a dual check valve installed; grease trap near building needs cleaning; side door of dumpster was left open; one light is missing under right side of hood system.

JULY 20

Lulabelle’s Café & Catering, 2012 Staples Mill Road – The following violations were reported during a routine inspection: observed debris on ladles; multiple foods have been held for more than 24 hours without a date mark; observed dried food debris buildup on inside of sandwich units and outside of three-compartment sink; observed dried food debris/grime/dust buildup on floors by three-compartment sink and ceiling vent.
