Bryan Park Bar and Grill, 5516 Lakeside Avenue – The following violations were reported during a routine inspection: no hand wash signage located at sink in bathroom; there is 0 ppm of sanitizer in dish machine; cole slaw, soup, chicken and prime rib are all past their expiration dates; observed a leak at the dish machine; wall behind ice machine is dusty; observed heavy grease buildup behind cooking equipment; observed dirt and grime buildup under dish area and in mop room.

Canastas Chicken, 5233-35 South Laburnum Avenue – The following violations were reported during a routine inspection: facility doesn’t have a verifiable employee health policy; hand sink doesn’t have soap; rubber gloves and a knife were found in separate hand sinks (hand sinks are for handwashing only); hand sink in men’s restroom doesn’t have paper towels; bleach sanitizer at three-compartment sink is less than the required 100 ppm; temperature of fried rice on the line is too low; cooked chicken, tub of cooked beans and green sauce in walk-in cooler do not have date marks; observed many flies throughout the kitchen; pepper and bottles of vinegar on cook line shelf are uncovered; food incorrectly stored on floor in both walk-in coolers and in dry storage in between bathrooms; sanitizer level in wiping cloth bucket is too high; racks in both indoor and outdoor walk-in coolers are rusty; lights are dim and insufficient in both units; observed soiled cardboard on shelf next to fryer; front cutting board on line is heavily scratched and stained; food spills found in bottom of two-door freezer; sides of fryers are greasy; racks in both walk-in coolers are dirty and dusty; light is out under chicken hood on front line; there is a hole in the ceiling just outside the indoor walk-in cooler; damaged ceiling tiles found in dry storage room; floor is dirty in dry storage room; hood vents above chicken oven on front line are very dusty and greasy.

Little Caesars, 1547 North Parham Road – The following violations were reported during a routine inspection: observed an employee’s drink without a lid; cooked wings are not being time tracked; soda crates incorrectly used as shelving in the walk-in; floor drain under three-compartment sink is dirty.

KanPai Japanese Steakhouse, 11964 West Broad Street – The following violations were reported during a routine inspection: observed an employee’s drink without a lid; packages of raw beef incorrectly stored next to lettuce in reach-in cooler near sushi station; a package of beef is leaking blood and blood has pooled under the bottom box of lettuce; scoops on clean dish rack have debris/food residues stuck to them; temperature of rice made last night in the walk-in is elevated; multiple spray bottles of chemicals are unlabeled; cutting boards are discolored and/or have deep scratches; equipment surfaces on the cook line including sides of fryer/stove and pipes are encrusted with grease; refrigerator surfaces are smeared with residues; walk-in cooler fan guards are dusty; cooler racks have debris on them; microwaves at sushi station are dirty; walls and ceilings throughout the kitchen are stained and splattered with grime; dish area wall below the washer is caked in mildew; hood filters are greasy.

JULY 6

Eat 33, 6901 Staples Mill Road – The following violations were reported during a routine inspection: temperature of meatloaf cooked yesterday in walk-in cooler is elevated; temperatures of burrito mix, sausage with veggies and pancake mix are elevated; several food items (sliced tomato, tuna salad, salmon cake mix, egg salad) are incorrectly covered, stacked and in deep containers in walk-in cooler and prep unit; prep cutting boards are scratched and stained; gasket on walk-in cooler door is torn; walk-in cooler is not maintaining foods at the proper temperature; observed dust and food debris on prep tables, counters, wheels of prep tables, dry storage racks and cup bar mats; dried food debris and grime are embedded in grout on the floors; ceiling tiles are stained.

Wing Zone, 8310 Staples Mill Road – The following violations were reported during a routine inspection: back hand sink doesn’t have paper towels; temperatures of cheese and bacon in small prep unit are elevated; small prep unit is not maintaining foods at a proper temperature; cleaning sprays incorrectly stored on shelf next to food; knife blades are chipped; fryer baskets are in disrepair; containers are cracked; prep and single cutting boards are heavily scratched and stained; observed grease residue and dust on wire storage racks, wheels of prep tables, prep unit, fryer timer touchpads, sauce bottles, oil filter machine and on fan grates in walk-in cooler; floors have dirt, grime and food debris on them; ceiling panels and vents are dusty.

Einstein Brothers Bagels, 1368 Gaskins Road – The following violations were reported during a routine inspection: back hand sink doesn’t have any hand wash signage displayed; pink and black mold found in ice chute at soda dispenser in dining room; observed food debris on four scoops stored clean; temperatures of lettuce, deli ham, strawberry cream cheese and raw shell eggs in prep top unit are elevated; metal pans are wet nested on clean shelf in dish area; racks in dry storage area are rusty; gasket in walk-in cooler is torn; there is heavy ice accumulation on pipes of walk-in freezer; cutting boards are heavily scratched and moldy; observed buildup in microwave on front line; observed dust/debris on tops of cans in dry storage area, on shelves and racks throughout, outside of Blentec on front line, in lowboy unit on front line, and on soda box holder in back of facility; ceiling vents are dusty; walls in dish, front line and prep areas are stained; there is debris/buildup on floors throughout the facility, especially under equipment on front line; floors throughout facility have low grout; ceiling light shields/covers are missing in back kitchen.

Ho Ho Chinese Restaurant, 10 East Laburnum Avenue – The following violations were reported during a routine inspection: bowl of raw chicken incorrectly stored directly on top of an uncovered container of cooked ready-to-eat noodles; cooked chicken wings on counter and in fryer basket and garlic and oil mixture on cook line are at the incorrect temperature; pesticide spray and bug spray incorrectly stored on shelves with food; rodent bait is in an uncovered container; bulk containers of flour and sugar are unlabeled; observed dried food debris and grease buildup on walls throughout kitchen, outside of hood system and on floors under cook line; observed heavy buildup of grease in hood system.

Sun Top’s China, 5660 Brook Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; temperature of fried rice in back hot holding unit is too low; temperatures of raw chicken in sink and chicken wings, cabbage, noodles, bean sprouts, and garlic and oil on prep surfaces are elevated because all these items were left out; egg rolls, chicken and pork made 2-3 days prior do not have date marks; large bulk bins of sugar, flour and msg are unlabeled; observed mouse droppings on shelves and in back storage area and dead roaches in the mop room; there are uncovered items in units and food stored on the floor of walk-in cooler and freezer; old jugs have been cut and are being used for scoops; frozen items incorrectly stored in plastic grocery bags; paper menus incorrectly used to line bins for egg rolls; facility couldn’t provide test strips for three-compartment sink; observed grime on racks throughout and soiled cardboard; food debris is built up on gaskets and side of rice hot holding units; gaskets and handles of units are sticky; ceiling tiles in mop area are missing; lights are out in dry storage area; back screen door is in disrepair; several areas need cleaning (under cook line, hood filters, back dry storage area and mop room).

Chipotle Mexican Grill, 10501 West Broad Street – The following violations were reported during a routine inspection: temperature of steak in warmer unit on cook line is too low; temperature of super greens placed on ice on the line is elevated; quaternary ammonium sanitizer level at three-compartment sink is too high; observed multiple small flies in back of facility where soda boxes are stored; metal pans are wet nested on clean rack; racks where clean dishware is stored are rusty; observed low grouting in back of facility.

Padow’s Hams & Deli, 4120-C Cox Road – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit/diarrhea; ketchup funnels have dark stains; dried food residue found on a scoop; temperature of lettuce in the make table is elevated because it was overfilled; salad bar, which doesn’t maintain foods at the proper temperature, is meant to be on time control but person in charge states that foods may be saved based on their appearances; cutting boards at sandwich units are stained and scratched; refrigerator surfaces have smeared residues on them; chemical lines on the dishwasher and walk-in cooler condenser are dusty; ceiling tiles in back kitchen are discolored.

Nick’s Roman Terrace, 8051-A West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide copy of certified food protection manager certificate; raw eggs incorrectly stored in between creamer and lemon box in walk-in cooler; raw chicken incorrectly stored next to rice in lowboy unit on cook line; observed buildup in tea nozzle in kitchen service station; temperature of fresh garlic and margarine is elevated because it is sitting out on the cook line; temperature of spanakopida in lowboy unit is elevated; meat sauce incorrectly cooling in deep container with saran wrap; temperatures of cut lettuce, cut cabbage and chicken salad are elevated; beer and wine boxes incorrectly stored on the floor at bar area; oil boxes and canned marinara incorrectly stored on the floor in storage area; torn gaskets found in reach-in freezer and lowboy units; observed rust on top of ice machine in back storage area; facility doesn’t have a high temp measuring device; final rinse for high temperature dish machine doesn’t get hot enough; debris found on shelves in back storage area; racks and fan in prep area, ceiling fan in back prep area and ceiling light shield in kitchen are dusty; observed grease/debris on pipes behind equipment, outlet on wall on cook line, and in between equipment on cook line; observed debris on cart, bottom shelf where clean plates are stored, and on racks in walk-in cooler; observed grease/buildup/debris/stains on walls and floors under equipment on cook line and in dry storage area; kitchen floor and back prep area floor have low grouting; paint on ceiling tiles in prep area is chipping.
