Anokha Indian Cuisine, 4015 Lauderdale Drive – The following violations were reported during a follow-up inspection: cutting boards at make tables are scratched and stained; walk-in cooler racks are rusty; gasket in under-counter freezer is dirty; cold water handle at the prep sink is missing but the sink is still operable; wall behind the dishwasher has mildew.

JUNE 15

Buz and Ned’s Real Barbecue, 8205 West Broad Street – The following violations were reported during a follow-up inspection: facility could not provide a copy of the certified food protection manager certificate; hand sink in wait station doesn’t have hot water; hand sinks at bar and in kitchen don’t have hand soap; hand sink on main line doesn’t have paper towels; a few bags of beef stew in the walk-in freezer are not marked with dates; multiple cutting boards throughout the facility are heavily scratched and/or stained; racks in walk-in cooler are rusty; gaskets on multiple refrigeration units are torn; observed low grouting throughout the facility; multiple light bulbs are out under hood system in prep area; one light bulb is missing in hood system on front line; ceiling lights are not working properly in dry storage area.

Regal Cinemas, 10091 Jeb Stuart Parkway – No violations reported during a routine inspection.

McDonald’s, 5106 Nine Mile Road – The following violations were reported during a routine inspection: observed dried food buildup on slicer and smoothie mixer; grime buildup found inside ice machine and ice bin in drive-thru area; time stickers are incorrect on lettuce containers on cook line and in sandwich unit; ice scoop incorrectly stored with handle touching ice.

J. Sargeant Reynolds Community College—Campus Bookstore, 1651 East Parham Road – No violations reported during a routine inspection.

Terra Nova Bar & Grille, 4700 South Laburnum Avenue – No violations reported during a follow-up inspection.

Vee’s Grill, 4360 South Laburnum Avenue – The following violations were reported during a routine inspection: facility could not provide a copy of the certified food protection manager certificate; no handwash signage observed at hand sink; floors are dirty.

Kroger, 9480 West Broad Street – The following violations were reported during a routine inspection: did not observe a certified food protection manager certificate during inspection; internal sanitizing temperature at high temp dish machine doesn’t reach the required temperature; final rinse for the high temp dish machine doesn’t reach the required temperature; temperatures of fries and fried chicken on the hot line are not hot enough; to-go trays at deli station are not stored inverted; water is leaking from corner of combi therm at rotisserie chicken station; facility couldn’t provide high temperature test strips; observed debris on floors and walls under equipment at both rotisserie and fry station, on shelves, in utensil container on front deli line, on light bulb covers on hood system, on sides of fryers and under combi therm at rotisserie chicken station; observed low grouting throughout the kitchen; there are holes in the wall and on ceiling in dish room; observed slight debris/trash on floor in walk-in freezer.

JULY 16

Plaza Azteca Mexican Restaurant, 10099 Brook Road – The following violations were reported during a routine inspection: open drinks found over prep unit and on prep table; observed an employee put raw meat on grill and then wrap tortillas without washing hands or changing gloves; sanitizer and utensils found in hand sink at dish area (hand sinks are for handwashing only); hand sinks by cook line and dish machine do not have paper towels; raw shell eggs incorrectly stored over cut onions and cilantro; observed mold buildup on soda nozzles at bar and on shoot of ice machine; did not observe date marks on items made the previous day (black beans, salsa, cut cabbage); fly tape found over prep table where chicken was being cut; lettuce, boxes and food incorrectly stored on freezer floor; items in prep units are not covered; hand sink in dish area and soda lines in back kitchen are leaking; mop sink doesn’t have a backflow prevention device installed.

City Lights of China, 12391 Gayton Road – The following violations were reported during a routine inspection: observed the person in charge touch cooked orange chicken with bare hands to check the temperature; hand sink in back storage area doesn’t have paper towels; strainer of cooked orange chicken incorrectly stored on an uncovered sheet pan of raw chicken in the walk-in; temperature of orange chicken prepared two hours ago is elevated; temperatures of garlic in oil, wontons, noodles and orange chicken are elevated; some prepared foods (noodles, egg rolls, wontons, tofu, chicken) are not marked with dates; boxes and buckets of food incorrectly stored on the floor of the walk-in and kitchen; chest freezer lid is damaged and duct taped; walk-in cooler is not operable; observed water in the bottom of the lowboy refrigerator and a pan of water underneath the refrigerator (actively dripping); Homestyle white refrigerator on the line had marginal food temperatures; cardboard should not be lining shelves; many non-food contact surfaces in the kitchen are soiled with grease and food residue (shelving, sides of fryers, refrigerators, fan, floors and walls); faucet handles of hand sinks are sticky; there is no lighting in the dry storage area hallway; hood filters above woks on the lefthand side are dripping grease.

DQ Grill & Chill, 3275 New Market Road – The following violations were reported during a routine inspection: dishes found in front hand sink (hand sinks are for handwashing only); temperatures of chili and nacho cheese in hot holding unit are too low; hot holding unit leaks; to-go containers are not stored inverted; metal skimmer above three-compartment sink is rusty; mop sink, white prep table near sodas, can opener holder and front shelves are dirty; faucet at three-compartment sink falls below the flood line; several tiles on the ceiling have water damage; ceiling vent above three-compartment sink is dusty; there is mold on the corner of the wall near walk-in freezer; several surfaces are dirty (walls throughout, back door, floor near sodas and water heater).

Panera Bread, 10301 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide certified food protection manager certificate during inspection; paper towel dispenser at drive-thru hand sink is not working; chemical dish machine is not operable; observed debris on scoops and ladles stored clean; chicken in hot holding unit on front line isn’t hot enough; temperatures of lettuce and melon in unit on front line are elevated; temperatures of pre-portioned bagged chicken, spring mix, cheese, lettuce, eggs, chicken, hummus, liquid eggs, tuna salad, turkey, tomatoes and steak in prep top units are elevated; temperatures of arugula, turkey, pizza and roast beef in refrigerated drawers in both front and drive-thru lines are elevated; observed multiple small flies on front line and near drain on floor on front line; scoops are not stored inverted on shelf in dish room; multiple plastic containers are wet nested on shelf above prep sink; multiple cutting boards throughout the facility are heavily scratched and stained; reach-in unit in bakery section has torn gasket; walk-in freezer is not closing properly causing heavy ice accumulation on floors and racks; fan guard in walk-in cooler is dusty; observed debris in between and on racks and shelves throughout, on top of dish machine in dish room, on shelf under soda machine in dining room, and in between equipment on front and drive-thru lines; mop sink doesn’t have a backflow prevention device installed; observed low grouting on floors throughout facility; there is a missing ceiling tile in bakery section; ceiling vents and tiles on front line are dusty; there are stains/dust on walls throughout; standing water found on floor in mop sink room; observed heavy food debris on floors and behind equipment on both drive-thru and front lines.
