Poke Sushi Bowl, 11628 West Broad Street – The following violation was reported during a routine inspection: pans drying on the dish rack shouldn’t be wet nested.

Chili’s Grill & Bar, 9950 Brook Road – The following violations were reported during a routine inspection: hand sink at bar doesn’t have paper towels; temperatures of cheese, cooked chicken and raw chicken in middle unit by the grill are elevated; clean utensils shouldn’t be stored utensil side up; sides of equipment on the cook line are greasy; observed grime and/or food debris on gaskets of units, units on cook line, salsa unit, racks in walk-in, and in dish area; observed grease and other buildup on hot holding units; observed black buildup on outside of chip bins and lids; hose with nozzle attached doesn’t have a dual check valve installed; low grouting found throughout kitchen and bar area causing food buildup; a few floor tiles are cracked; floor behind cook line and fryers is greasy; ceilings above dry storage and above ice machine need cleaning; floor and walls near dish machine, prep sink and three-compartment sink are soiled.

Melito’s, 8815 Three Chopt Road – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomiting/diarrhea; open drinks found at bar and on cook line; observed the cook scoot food off spatula onto bun (use gloves or another spatula); key lime pies have been prepared by a worker at home and not the facility; slicer, can opener, can opener holder and drip trays are dirty; temperature of tomato soup prepared yesterday is elevated; temperature of cooked onions on ledge above stove is too low; temperatures of sweet potato fries, leaf lettuce, bleu cheese dressing, sliced turkey and sliced ham are elevated; no dates found on key lime pie and other TCS desserts; there isn’t a disclosure on the menu about burgers or salmon being cooked to order; spray bottle incorrectly stored beside water pitcher; spray bottles at service area and in back of house are unlabeled; ground beef, small steaks and hotdogs are incorrectly cooling on a rack at room temperature; no thermometer found in bar box holding half-and-half; a dry good is unlabeled; wet wiping cloths are stored on the counter instead of in a sanitizer solution between uses; inside beer box is rusty; unsealed wood in liquor cabinet and wood box used to store tea spoons; salt/pepper incorrectly stored in Styrofoam; cloth on carbonator shelf should be removed; no faucet handles at spray hose sink; grate is missing from floor opening; toilet room door is propped open; walk-in wall is rusty; floor under fryer is greasy; ceiling in storeroom is not smooth and cleanable; a “no smoking” sign is not posted.

Ming’s Dynasty, 1200 North Concord Avenue – The following violations were reported during a routine inspection: no date marks found on egg rolls, chicken, dumplings or noodles; no time label or written policy found on garlic and oil, cabbage or chicken; cold medicine incorrectly stored over open prep unit; wiping cloths are not being kept in a sanitizer solution between uses; sanitizer level in bucket is at 0 ppm; a cut jug should not be used as a scoop; several areas need cleaning (sides of cooking equipment, cooking table holding woks, racks in walk-in and over prep tables, inside of prep units, outside of hot-holding units and piping under three-compartment sink); observed soiled cardboard and egg cartons throughout the facility; floors and walls behind cookline, floor of walk-in cooler and floors under prep units need cleaning; observed buildup of grease and dust on hood filters.

Atlas 42, 4032-A Cox Road – The following violations were reported during a routine inspection: packages of raw meat and eggs incorrectly stored over bread and produce in both walk-in coolers; can opener blade is dirty; a spatula has a taped handle; observed grease buildup on sides of cook line equipment; observed debris on walk-in cooler racks; southern bar beer cooler has a purple substance spilled on it.

Bojangles, 11 South Laburnum Avenue – The following violations were reported during a routine inspection: hand sink by fry line has dishes blocking it; observed an employee use hand sink to clean dishes (hand sinks are for handwashing only); observed dried food debris and grease buildup on sides, under and behind cooking equipment and refrigerators by fry line, on drip trays of grill/stove, outside of flour bins, and on can opener holder; observed mold around door of chicken walk-in cooler.

Domino’s Pizza, 5526 Lakeside Avenue – The following violations were reported during a routine inspection: there is 0 ppm of sanitizer in three-compartment sink; facility couldn’t provide test strips for sanitizer; wall tiles are missing by dough prep table; there is low grouting in the dish area.

Country Style Donuts, 8902-B West Broad Street – The following violations were reported during a routine inspection: a tube is running into the hand sink in back kitchen (hand sinks are for handwashing only); pink slime and mold found growing inside ice machine; there is a gap large enough for pests to enter at the bottom of the back door; a sticky trap at the ceiling near the back door is full of small flies and needs replacing; gaskets on the proofing box are torn; sanitizer dispenser at three-compartment sink is broken; many items in the kitchen are dirty (standing fans near donut rack and donut prep tables, outside of white bulk bins, back kitchen hand sink and proofing box gaskets and interior); back door has chipping paint; there is a hole and chipping paint on the back wall; ceilings in mop room and dry storage/office space have holes; wet mop incorrectly sitting on the floor (invert mops to allow to air dry); many areas in the facility are dirty (walls near door to seating area, floor under wooden crate up front, wall behind prep table, wall next to donut prep table, floor of proofing box, floors and walls behind fryers, floor near back door and the back door itself); floors throughout are sticky; ceiling around vent is dusty; there is mold on floors around stand mixers and under racks in back kitchen; hood filters are dusty.

Sheetz, 550 South Airport Drive – No violations reported during a routine inspection.

Clean Eatz Richmond, 11801 West Broad Street – The following violations were reported during a routine inspection: person in charge is unable to identify the “big six” foodborne illnesses and their associated symptoms; facility doesn’t have written procedures for cleaning up vomit/diarrhea; can opener blade is dirty; temperature of rice prepared yesterday in the walk-in is elevated (wasn’t cooled properly); temperature of Tzatziki sauce in the make table is elevated because it was stored in a pan above the air curtain.

Pho 1 Grill, 201 Towne Center West Boulevard – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; raw beef incorrectly stored next to tofu in lowboy; observed dark buildup inside ice machine; foods stored on ice are supposed to be on a time control policy, but employee states that they may be saved and cooled back down after lunch (time controlled foods cannot be saved); pans and bins of food incorrectly stored on kitchen, cooler and freezer floors; knives incorrectly stored in crevices on the cook line; walk-in cooler racks are rusty; cutting boards at the make tables have dark deep scratches; walk-in fan guards are dusty; observed dust on top edges of kitchen walls; there is grease buildup under the fryers.

Red Lobster, 4415 South Laburnum Avenue – The following violations were reported during a routine inspection: temperatures of sliced tomatoes, lettuce, raw shrimp and ready-to-eat crab on ice on the cook line are elevated; fish is incorrectly thawing in reduced-oxygen packaging in the walk-in; one dead American cockroach found in light above dish area and another dead cockroach was found on the floor drain in the kitchen; bottle of dressing in kitchen and a bin of batter in dry storage are uncovered; metal and plastic bins above the three-compartment sink are wet nested to dry; observed food debris inside two-door seafood unit next to fryer and inside lowboy freezer on the cook line; floors on cook line are dirty, especially under equipment; water is pooling inside the walk-in; coving near back door is broken; caulking at service hand sink inside kitchen is damaged; hood filters above cooking equipment are greasy; grease is accumulating on the lights under the filter.
