Outback Steakhouse, 7917 West Broad Street – The following violations were reported during a routine inspection: strainer, knife and straws found in hand sink at bar (hand sinks are for handwashing only); facility couldn’t provide a parasite destruction letter from the frozen tuna provider; raw prime rib incorrectly stored over chili in walk-in cooler; observed food debris greater than one day old on vegetable slicer, baking cups, mixing bowl and on multiple utensils hanging on wall and in utensil container; final rinse temperature at high temp dish machine is not within the required range; temperature of chili being reheated in warmer is too low (low water level detected in warmer); temperatures of several foods are elevated (raw chicken, raw beef, raw salmon, asparagus, cheese, sliced cherry tomatoes, eggs, blue cheese, ribs, chicken wings); air temperature of two refrigeration units is elevated; lettuce and tomatoes on cook line are on a time control policy but no time was marked; temperatures of chili and pasta are elevated because they are not being cooled properly; plastic pans are wet nested on shelf above prep table; cutting boards throughout the facility are heavily scratched; racks in dish area and in lowboy unit on the cook line are rusty; lexan pans on shelf in prep area are cracked; observed a gap greater than one inch on hood at the cook line; top refrigerated drawer on cook line has duct tape on the outside of the door; facility couldn’t provide sanitizer test strips or high temperature test strips for the dish machine during inspection; observed stains/dried liquid on bottoms of beer coolers at service station in back kitchen; shelves under grill on cook line and wheels at fry station are greasy; observed debris in between equipment, on hood filters, light bulbs, fryer baskets, behind fryers on cook line, on top of convection oven, behind tea urns at service station, on shelves throughout the kitchen, on racks throughout, single-service trays stored clean, and on top of dish machine; fan guards in walk-in cooler are dusty; observed leak from faucet at three-compartment sink and at mop sink; observed multiple cardboard boxes stacked on top of one another on concrete floor next to dumpsters; dumpster area has litter and debris; dumpster and recycling doors are not being kept covered; ceiling vents above mop sink are dusty; there is standing water on floor in back kitchen; wall under dish machine in corner of dish area is moldy; observed debris/litter on floor around and under ice machine near front dining area, at bar area, on wall and pipes behind fry station in kitchen, on floor of walk-in freezer, and under equipment on cook line; observed low grouting throughout the kitchen; paint is chipping on ceiling in dry storage area and on wall in prep area.

Tropical Smoothie, 9972 Brook Road – The following violations were reported during a routine inspection: back hand sink doesn’t have paper towels; temperature of sweet potatoes made three hours prior is elevated because they are not being cooled properly; observed grime and old food buildup inside of units; bottoms, tops and handles of blenders are dirty; floors are sticky and there is a buildup of grime along coving tiles; observed mold/slime buildup in drains.

Dominic’s of New York, 4401 Pouncey Tract Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; light is missing/broken under the hood; three-compartment sink is dirty; shelving is dusty; observed residue on glass door of reach-in cooler; metal flooring is rusted and coming loose; there is a large hole in the floor at the kitchen entrance; hood filters are very dark from buildup.

Bocata Arepa Bar, 10170 West Broad Street – The following violations were reported during a routine inspection: person in charge is not a certified food protection manager; utensils and pans found in hand sink on front cook line (hand sinks are for handwashing only); observed employees on cook/prep line rinsing gloves at the hand sink when they become soiled and then touching surfaces not being sanitized; observed an employee clean food contact surfaces with soapy water but not sanitize them afterwards; containers of chicken and sauces in reach-in coolers do not have date marks; allergy and pain medications and lotion are incorrectly stored on shelves with food; prep unit cutting board is heavily scratched and stained.

Mama Cucina, 4028-O Cox Road – The following violations were reported during a routine inspection: two employees changed tasks and gloves without washing hands in between; employees also didn’t wash after handling raw meats; raw chicken incorrectly stored next to other raw meats in multiple fridges; dark mold buildup found on bar soda gun nozzle; temperatures of raw chicken, salad mix and seasoned oil with fresh garlic are elevated; cook states that some foods are on time control but the person in charge was unable to locate the written time control policy; mechanical lubricant and personal medications incorrectly stored on the expo line; many foods are cooling while covered and/or stacked in coolers that are also opened too frequently to allow for proper cooling; wet wiping cloths are not being stored in a sanibucket between uses; ice scoop handle is touching ice at the bar; cutting boards are scratched and stained; multiple fridge gaskets are torn; several non-food contact surfaces are dirty (sides of equipment, top of dishwasher, top shelf of expo, vent on ice machine, wire storage racks); wall in dish area has mildew; floor under equipment in kitchen and at bar is soiled; some patches of walls in kitchen appear discolored or have a worn surface.

Funny Bone Comedy Club, 11800 West Broad Street – The following violations were reported during a routine inspection: neither hand sink in the kitchen has paper towels; raw hamburgers incorrectly stored next to ready-to-eat food in the make table; ice machine in kitchen has pink slime patches inside; soda gun nozzles at the bar are dirty; temperature of whole rotisserie chickens made last night is elevated; cutting board at the make table is discolored and scratched; wall in the dish area is dirty.

Subway, 4386 South Laburnum Avenue – The following violations were reported during a routine inspection: mops are not being hung to dry; ceiling tiles above the oven are stained.

O’Charley’s Restaurant, 6291 West Broad Street – The following violations were reported during a routine inspection: bowl of straws found in hand sink at bar (hand sinks are for handwashing only); automatic paper towel dispenser at hand sink at end of cook line is not working; facility couldn’t provide a parasite destruction letter from the mahi mahi distributor during inspection; facility couldn’t provide written agreement from salmon supplier stating that salmon is farm raised and pellet fed during inspection; raw pork and raw bacon incorrectly stored next to cooked bacon and chicken base in walk-in cooler; observed food debris greater than one day old on slicer on prep table, on fryer baskets and on can opener blade; observed pink mold on edges of metal panel on ice machine; temperature of lettuce prepped yesterday is elevated; temperatures of cheese, mac and cheese, and rice pilaf are elevated; temperatures of gravy, mashed potato and rice pilaf are too low; manager states that breading and buttermilk at fry station are replaced every four hours but there is no time written down; salmon is incorrectly thawing in reduced oxygen packaging; salts and sugars on table at bar area are not being covered when not in use; lowboy unit on cook line has torn gaskets; water is pooling in multiple lowboy units on cook line; racks in prep area are rusty; observed ice accumulation on rightmost sandwich unit on cook line and on pipes of walk-in freezer; multiple cutting boards are heavily scratched; observed debris and/or grease on shelves and racks throughout the kitchen, outside of weigh scales and drawers, on top of knife holder, on hood filters, under fryers and grills, on pipes behind equipment, inside tortilla chip holder, in microwaves and reach-in freezers, and on top dish machine; mop sink doesn’t have a backflow prevention device installed; low grouting found on floors throughout the facility; ceiling tiles in dry storage area are in disrepair; observed debris and stains on floors and walls throughout the facility; observed heavy grease on walls and floors under and behind equipment on cook line.

California Pizza Kitchen, 11800 West Broad Street – The following violations were reported during a routine inspection: observed buildup on can opener blade; soda gun nozzle at the bar is moldy; temperature of sliced tomatoes in the make top is elevated because they were overfilled; temperature of spaghetti cooling in refrigerated drawers on the cook line is elevated; sanibucket on the cook line doesn’t have the proper concentration of sanitizer; facility doesn’t have an irreversible temperature measuring device for the high temp dish machine; there is a leak at the spray sink and at the front cook line hand sink; ceiling vent over ice machine is very dusty; door frame of the walk-in cooler (pass thru door) has food debris and buildup.

Papa John’s Pizza, 8903 Patterson Avenue – The following violations were reported during a routine inspection: temperatures of cheese sticks, ham, sausage, chicken, cheese, tomatoes and spinach are elevated; cutting board in dish area is heavily scratched; air temperature of lowboy unit on prep line is elevated; gasket on leftmost lowboy unit is not installed properly; gasket on walk-in cooler is torn; racks in walk-in cooler are rusty; observed debris outside of convection oven, on hood filters, on shelves and racks throughout, and in floor corners in dish area and employee restroom; inside of toilet bowl in employee restroom is dirty; observed low grouting on floors throughout; there are holes in walls and sticker residue on walls in prep and front of house areas; walls in dish area are stained;

Little Caesar’s, 6040 Brook Road – The following violations were reported during a routine inspection: hot water at the front hand sink doesn’t get hot enough; did not observe timers set for pre-made pizzas; plastic containers are wet nesting on clean dish rack; observed buildup of grease, grime and dust on racks throughout the facility; observed old food buildup on units’ gaskets and inside units.

Burger King, 4240 Pouncey Tract Road – The following violations were reported during a routine inspection: observed employees picking up containers of hash browns from the top of the pack where hash browns are exposed to bare hands; observed dust and residues on monitors and control panel in the back kitchen near fryers; walk-in cooler rack has debris; observed sticky dust buildup on top shelves in prep area; mop sink doesn’t have a dual check valve installed; there is a leak at the chemical hose connected to the three-compartment sink; observed grease buildup under the fryers; floor drain in drive thru is dirty.
