Cracker Barrel, 3620 Mayland Court – The following violations were reported during a routine inspection: observed an employee cut up sliced tomatoes with bare hands; raw shell eggs incorrectly stored over hash browns in lowboy unit on the cook line; observed food debris greater than one day old on slicer and both vegetable slicers on and under prep table and on scoops and strains stored clean; pink and black mold found on plastic panel of ice machine in service station; final rinse for the high temp dish machine is not at the correct temperature; internal sanitizing temperature for high temp dish machine is not at the correct level; temperature of country ham stored on side of hot holding unit is too low; temperature of raw turkey in thawing box is elevated; temperature of milk at service station is elevated; temperature of pancake mix prepped this morning is elevated because it was not cooled properly; scoop is stored in water but the water is not hot enough; metal pans are wet nesting on clean rack next to hand sink; refrigerator drawers at breading station in prep area are not in proper working condition and one is missing; racks in walk-in cooler are rusty; observed debris/dust on shelves and racks throughout, on hood filters, under grill and fryers, in between equipment, inside of microwaves on the cook line, on fan guards in walk-in cooler, in utensil containers and on plates at service station, and outside of plastic lexan pans; faucet at prep sink is leaking; observed buildup inside of toilet bowl in employee restroom; observed debris/grease/stains on floors and walls throughout, especially on cook line and service station under equipment; ceiling tiles and vents on cook line and in dish area and service station are dusty and stained; there are holes in the wall next to hand sink in dish area; ceiling tiles are chipping in the dish area; observed low grouting on floors throughout; floor tiles in front of walk-in cooler are cracked.

YaYa’s Cook Book, 11674 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide a parasite destruction letter from the fish provider; containers of different raw meats are incorrectly stored next to each other and produce in the walk-in cooler; observed residue on rice strainers hanging above the three-compartment sink; temperature of brown rice in portioned baggies made yesterday is elevated because it was not cooled properly; temperature of cabbage in the Thai kitchen make table is elevated because it is overfilled; paper menu doesn’t have a consumer advisory; online menu has consumer advisory for sushi items, but not for cocktails containing eggs and other foods; torch and torch gel incorrectly stored near food; an employee’s personal item is incorrectly stored on the expo line; temperature of lexan pan of rice in reach-in is elevated because it is covered while cooling; bulk ingredients on cook line shelves are unlabeled; many boxes/buckets of food are incorrectly stored on the floor; ice scoops at the bar are stored with handles touching the ice; to-go sushi containers are not being stored covered or inverted; many wooden surfaces in the kitchen are no longer smooth; two cutting boards in back prep area are worn; the sanitizer test kit expired in 2020; observed grease buildup on sides of cook line equipment in Thai kitchen and on shelves around the grill; walk-in cooler fan grates are dusty; walls throughout the back kitchen are dirty; floor drains and the pipes leading to them are grimy; cook line floor has grease and soil buildup; ceiling tiles in Thai kitchen have dark spots; hood filters are greasy.

Kee Kee’s Place For Taste, 2709 Williamsburg Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; person in charge is unable to identify the “big six” foodborne illnesses and their associated symptoms; hand sink in employee bathroom doesn’t have hand soap; hand sink doesn’t have hot water; burgers and eggs on the menu are not attached to the consumer advisory statement at the bottom of the menu; menu states “catfish bites” but Swai fillets are being used; reach-in freezer, sandwich unit and fryer in the kitchen are all broken; air temperature of walk-in unit is elevated; racks throughout the kitchen are rusty; mop sink doesn’t have a dual check valve installed; water heater is not functioning; spare windows and various boxes should not be stored in the kitchen area; ceiling tiles in dry storage room and kitchen are stained; ceiling tiles surrounding ceiling vent in dry storage are moldy; observed low grouting on floors throughout and damaged coving throughout; two lights are out under the hood; ceiling in dry storage is damaged revealing holes.

Maple Street Biscuit Company, 11657 West Broad Street – The following violation was reported during a routine inspection: facility doesn’t have an irreversible temperature measuring device for the high temp dishwasher.

McDonald’s, 9751 West Broad Street – The following violations were reported during a routine inspection: hot water at hand sink near back drive thru area doesn’t reach the required temperature; temperature of pasteurized liquid eggs is elevated because they were left out on the cook line; temperatures of cooked eggs and raw eggs in refrigerated drawers are elevated (air temperature of the unit is elevated also); bacon on a time control policy doesn’t have time written down; observed four small flies in front service area, near ice machine near back exit door, and in dish area; both reach-in freezer units in the kitchen and lowboy unit in front drive thru area have torn gaskets; observed dust on racks throughout, under fryer cabinets, on wheels and pipes under fryers at both fry stations, in between teas and equipment at service station in front, and on top of ice cream machine; observed debris under microwaves on shelf near reach-in units; ceiling above three-compartment sink in dish area is leaking; observed buildup/trash in floor corners in drive thru and dry storage areas; floor under both fryers in the kitchen is greasy; there is standing water on floors behind ice machine and near water heater in back of facility; observed debris on floors and walls under three-compartment sink.

Full Kee Restaurant, 6400 Horsepen Road – The following violations were reported during a routine inspection: there is no one in charge at the time of inspection; facility doesn’t have written procedures for cleaning up a vomiting or diarrheal event; no handwash sign is posted at two hand sinks; there is uncovered food stored in the walk-in; slicer is dirty; observed an employee stand on a table to reach equipment overhead and didn’t sanitize surface afterwards; temperatures of noodles in reach-in and duck eggs in make table are elevated; several foods (tofu, fried tofu) do not have date marks; chemical sprayer is unlabeled; dry goods and crystals are unlabeled; live roach found inside lid to make table; dirty cloths found on counters (should be stored in sanitizer between uses); rice scoop is stored in water but the water is not hot enough; tin cans and cardboard should not be reused for food storage; cutting board is part of a tree, which is not cleanable and unacceptable; wooden prep table is not cleanable; foil incorrectly lining shelves and other surfaces; strainer incorrectly wrapped in linen; cement blocks, wood support and cardboard are being used to level tea urn table (these are not cleanable and shouldn’t be used); many surfaces are dirty (interior of dish machine, drip trays in front and back kitchens, stove tops, fryer baskets, pipes to cook line, exterior of chest freezers, defrost chest freezers, exterior of dry good bins, sesame seed container, three-compartment sink, pipes to dish machine, freezer and refrigerator walk-in floors, wall in back prep room, and cook line floor); floor behind tea urns has trash; observed peeling paint on ceiling in small storeroom; spray foam is exposed in several areas; wall behind tea urns is in poor repair; hood filters are greasy; there is insufficient light in the walk-in; a light in the back prep area is unshielded.

Papa John’s Pizza, 602 East Laburnum Avenue – The following violations were reported during a routine inspection: can opener blade, thermometer and pizza dough roller are dirty; box of hand sanitizer incorrectly stored on and over food contact surfaces/equipment; spray bottle is unlabeled; pizza screens are rusty; broken pizza handle; observed standing water in lowboy refrigeration unit; pizza screen incorrectly stored in cardboard; no backflow prevention device is installed at the mop sink faucet; some patches of floor are not smooth; bulkhead has broken edges; there is unnecessary equipment in the facility that should be removed; hood is greasy.

Barnes & Noble Booksellers, Inc., 9850 Brook Road – The following violation was reported during a routine inspection: three gallons of milk are past their expiration date.

West Coast Provisions, 301 Maltby Boulevard – No violations reported during a follow-up inspection.

Kabuto Japanese House of Steaks, 8052 West Broad Street – The following violations were reported during a routine inspection: the consumer advisory and disclosure statements are not printed on the paper lunch and dinner menus; all single and prep unit cutting boards are heavily scratched and stained; wire racks in walk-in cooler are rusty; observed dried food debris and grease residue on wire racks in storage areas and on surfaces of refrigeration units and equipment; wire racks and fan grate system in walk-in cooler are heavily molded; wall behind washing machine is damaged (paint and drywall chipping); observed grease and food accumulation on floors under and behind fryers, in grill area in kitchen and at bar, and in walk-in cooler.

Roma’s Italian Restaurant, 8330 Staples Mill Road – The following violations were reported during a routine inspection: temperature of a pan of tomato bruschetta in prep unit is elevated; no date marks found on foods prepared and held for more than 24 hours (lasagna, tomato sauces); cleaning chemicals incorrectly stored next to bags of open sugar and onions in storage room; prep unit and single cutting boards are heavily scratched and stained; butcher block board is no longer sealed; observed dried food debris and dust on fan grate system and wire racks in walk-in cooler; blue dish racks have buildup of limescale.

Mi Guerrero Grill, 61 South Laburnum Avenue – The following violations were reported during a routine inspection: observed employees handle raw animal foods and switch tasks without washing hands; temperature of reheated rice is too low; temperature of meat prepared the previous evening is elevated; temperatures of multiple foods in two-door reach-in are elevated (carne molida, asade, lengue); two-door reach-in unit is not maintaining foods at the proper temperature; ceiling tiles are heavily damaged; wall coving in women’s restroom is in disrepair; light is damaged; ceiling is leaking.
