Chez Max Restaurant, 10622 Patterson Avenue – The following violations were reported during a routine inspection: unable to verify that employees have been trained to identify the “big six” foodborne illnesses and their associated symptoms; temperature of brie in top of prep unit is elevated; raw egg is used in the “Not So Basic” cocktail and in Caesar salad but there isn’t an asterisk on the menu to indicate this; bleach, hand sanitizer and medicine are incorrectly stored on prep table next to clean equipment; observed one live German cockroach in the dish area and several dead roaches throughout the kitchen, dry storage, dish area and bar; cutting boards used for bread are heavily worn; butcher’s block is cracked; a dual check valve is not installed on faucet outside; dumpster is not located on pavement; FRP in dish area wall is peeling away from the wall; ceiling vents and wall behind oven are dusty.

Fat Boy’s Bar + Grill, 2 West Williamsburg Road – The following violations were reported during a routine inspection: there isn’t a hand sink at the bar; hand sink in the kitchen doesn’t have handwash signage; did not observe a parasite destruction letter from the salmon provider; salt and batter containers are unlabeled; two wet wiping cloths are sitting out on the prep table instead of being stored in sanitizer solution; facility could not provide chlorine test strips for the dish machine.

Boychik’s Deli, 4024-B Cox Road – The following violations were reported during a routine inspection: observed an employee drink without a lid or straw in front prep area; beaten egg incorrectly stored above ready-to-eat foods in the beverage style cooler on the cook line; can opener blade is dirty; utensils and pans stored clean in the dish area have debris on them; personal medications and Lysol wipes incorrectly stored on the deli case adjacent to bread; food buckets incorrectly stored on walk-in floor; to-go containers are not stored inverted in front and back kitchen; several non-food contact surfaces are soiled (shelves, cabinets, gaskets and racks in the beverage style cooler on the cook line, rack over three-compartment sink, cook line equipment sides/crevices, and walk-in fan guards); a dual check valve is not installed at the mop sink; trash cans in the women’s room stalls do not have lids; floor tiles are cracked in various spots throughout the kitchen; walls and ceiling throughout kitchen are discolored and/or dirty.

Ironclad Coffee Roasters, 2100 John Rolfe Parkway – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; unable to verify that employees have been trained to identify the “big six” foodborne illnesses and their associated symptoms; hand sink doesn’t have paper towels; boxes of food incorrectly stored on walk-in floor; wet wiping cloth is not stored in a sanitizer solution; wood in food prep areas isn’t sealed; floor tiles near walk-in and oven are damaged; mop is not being hung to dry.

Starbucks Coffee Company, 9700 Gayton Road – The following violations were reported during a routine inspection: temperatures of whipped creams in lowboy units on the front line are elevated; observed incorrect level of quaternary ammonium sanitizer in sanibuckets under espresso machines on front line.

Potbelly Sandwich Shop, 10921-E West Broad Street – The following violations were reported during a routine inspection: cutting boards at the make tables are heavily scratched; fan grate on walk-in ceiling, plastic soda racks in the walk-in, and pass-thru shelves are dusty; observed debris and dirt under nearly all fridges and tables throughout front and back kitchen.

Zara Grill, 10865 West Broad Street – The following violations were reported during a routine inspection: soda nozzles on the drink machine are dirty; rice pudding made two days ago is not date marked; a frayed and rusted metal and wood paint brush is being used for butter; cold water handle at the prep sink is not working.

NuNu’s Café, 6800 Paragon Place – The following violation was reported during a routine inspection: facility doesn’t have a certified food protection manager.

Sheetz, 7035 West Broad Street – The following violations were reported during a routine inspection: observed an employee pull up pants and continue to handle food without changing gloves and washing hands; there is 0 ppm quaternary ammonium sanitizer at three-compartment sink; observed food debris on multiple scoops that were stored clean; temperatures of whipped cream and sliced cheeses are elevated; four trays of burritos in the rightmost lowboy sandwich unit are past their expiration date; ladles are not being stored inverted; tongs at hot holding station are not being stored properly; cutting boards on front line and on both sandwich units are heavily scratched; observed debris on several surfaces (racks throughout, on top of dish machine, shelves on front line, in between gaskets on front line, in between and on sides of equipment, and in utensil containers on front line); fan guard in walk-in cooler is dusty; mop sink doesn’t have a dual check valve installed; observed debris and trash on floors throughout and stains on walls under the dish area.

New Top’s China 1, 7111 Staples Mill Road – The following violations were reported during a routine inspection: observed employees fail to wash hands after cracking open shell eggs and after washing equipment before beginning food prep; men’s restroom doesn’t have paper towels; there is no hand sink accessible to employees in the kitchen or dishwash area; several containers of cooked food (chicken, eggrolls) in the walk-in cooler are uncovered; observed an employee wash knives and containers but didn’t sanitize them; observed heavy buildup of grease and grime on wire baskets storing eggrolls in the walk-in cooler; temperatures of garlic in oil, cut cabbage and noodles are elevated; no date marking is being done for foods being prepared several days in advance (eggrolls, chicken, noodles); single cutting boards are heavily scratched and stained; wontons incorrectly stored in a used cardboard box; observed dried food debris and grease on surfaces of wok, fryers, bulk flour bins, wire racks, fan grates, prep surfaces and prep unit; ceiling panels in the kitchen are missing.

Roy’s Big Burger, 5200 Lakeside Avenue – The following violations were reported during a routine inspection: person in charge did not respond correctly to health questions from Environmental Health Services; there is not record of employee health training; dried food debris found inside microwave; temperature of cooked onions is too low; cardboard is lining drink refrigerator shelves and should be removed; observed grease and dust buildup on fans, heat lamps, Vitamix and fryer baskets; multiple ceiling tiles are in disrepair; floor and walls are in disrepair; wet mop is not being hung to dry; observed food debris and grime buildup on floors and walls.

McDonald’s, 10000 Brook Road – The following violations were reported during a routine inspection: lettuce, tomato and cheese on the line are not time marked; there is 0 ppm sanitizer in front bucket; spoons incorrectly stored in water next to the coffee station; single-service straws and to-go containers incorrectly stored on the floor in dry storage; several pieces of equipment are in disrepair (one-door lowboy under prep line, one-door freezer, sliding door on milk unit in drive-thru); gaskets are torn on fridge and small freezer; observed dirt, grime and/or grease on several surfaces (sides of grills on cook line, units on cook line, racks, gaskets, shelves along prep line, smoothie machine, coffee area, shelves next to fryer, hand sink and paper towel dispenser next to fryers, hanging wires and computers on prep line); observed low grouting throughout the kitchen which is causing food buildup between the tiles.
