MAR. 4

Tropical Smoothie Café & Deli, 8207 West Broad Street – The following violations were reported during a routine inspection: hot water doesn’t get hot enough in women’s or men’s restrooms; observed debris greater than one day old on scoop and knife that were both stored clean on a shelf; observed bacon stored out at room temperature next to panini press on the line; temperatures of rice, chicken wrapped in parchment paper and sliced cheese are elevated; temperatures of kale salad, prepped tomatoes, pre-portioned chicken and beans are elevated because they were not cooled properly; in-use utensils are not being stored properly; forks on a shelf on the front line are not stored inverted; multiple cutting boards throughout the facility are heavily scratched; racks in dry storage area in back of facility are rusty; air temperature of rightmost lowboy unit on front line is elevated; observed debris on cart, outside of containers on cart, on racks in walk-in cooler, shelves on front line and racks in to-go station in front of house and on front line; fan guards in walk-in cooler are dusty; ceiling and wall corner on front line is dusty and stained; observed debris on line under equipment at smoothie station and on floor corners in dry storage areas; observed low grouting throughout the facility.

West Coast Provisions, 301 Maltby Boulevard – The following violations were reported during a routine inspection: letter from the fish supplier doesn’t mention parasite destruction, only HACCP; dishwasher is dispensing zero chlorine after priming attempts and hot water sanitization attempts were not reached either; can opener blade is dirty; observed buildup in ice machine crevices and bar soda nozzles; temperature of butter is elevated because it was left out on the counter; temperatures of pickled onion and half-and-half are elevated; breading at the fryers is not on a time control policy; two sauces in the walk-in are past their expiration dates; observed a vacuum sealer in the kitchen and raw meats in reduced-oxygen packaging in the walk-in freezer and cooler (foods may not be put in reduced-oxygen packaging without an approved HACCP plan/variance from the health department); cutting boards at make tables are discolored and scratched; racks in the walk-in and in the dish area are rusty; observed a leak inside a lowboy unit on the cook line; paint is chipping off the surface of a juicing tool; observed crumbs inside utensil storage bins; top shelf of pass thru line is dusty; mop sink faucet leaks; floor and casters under fryers have grease buildup; ceiling vents over the dish area are dusty.

The Lily Pad, 5956 Brook Road – The following violations were reported during a routine inspection: employee restroom doesn’t have paper towels or soap at the hand sink; temperatures of collard greens, yams and mac and cheese made the night before are elevated; pork chops and fish are thawing incorrectly; bathroom doesn’t have a covered trash can.

McDonald’s, 11280 Patterson Avenue – The following violations were reported during a routine inspection: cut lettuce on the line is not marked with a discard time; gasket of refrigerator beneath frappe machine is damaged; racks in front beverage refrigerator are rusty; facility couldn’t provide test strips for chlorine sanitizer; mop sink doesn’t have a dual check valve installed; some of the racks in the walk-in are soiled; fan guards in walk-in and wall/shelves behind the three-compartment sink are dusty; single-service cups incorrectly stored on the floor.

Tony’s Pizza Express, 8191-K Brook Road – The following violations were reported during a routine inspection: temperature of garlic and oil is elevated; sliced deli meats and cooked meats do not have date marks; pizza by the slice does not have a time mark; level of quat sanitizer in three-compartment sink is too high; temperatures of lettuce and tomatoes prepped two hours ago are elevated; temperature of wings made more than an hour ago in the lowboy is elevated; multiple units throughout the cook line have torn gaskets.

Dunkin Donuts, 3125 Lauderdale Drive – The following violation was reported during a routine inspection: drink stir spoons are being stored in sanitizer between uses (must be stored dry).

Cupertino’s, 3621-A Cox Road – The following violations were reported during a routine inspection: observed food debris greater than one day old on tongs, ladles and scoops stored clean; temperature of raw shell eggs in prep top unit is elevated; temperatures of half-and-half and bacon are elevated because they were left out; levels of quat sanitizer and chlorine sanitizer in bottles are too high; smoked salmon incorrectly thawing in reduced oxygen packaging; white powders on shelf and in bulk containers in prep area are unlabeled; two boxes of oil incorrectly stored on the floor; racks in dry storage area and near order window are rusty; cutting boards throughout the facility are heavily scratched; wooden cutting prep table in back kitchen needs to be sanded down; facility couldn’t provide chlorine or quaternary ammonium test strips; observed grease/debris on several surfaces (in between equipment, on stove, under grill on cook line, on racks throughout the facility, on shelf used to store single-service items, on shelves at drink station, walls on cook line, floors under equipment, floor corners throughout back kitchen, walls at three-compartment sink); fan on floor in front of hand sink in back kitchen and ceiling vents in back kitchen are dusty; faucet at three-compartment sink is leaking; mop sink doesn’t have a dual check valve installed; women’s restroom doesn’t have a covered trash can; ceiling tile in prep area is chipping; multiple light shields for ceiling lights are missing.

Sportsman’s Restaurant & Lounge, 3306 Williamsburg Road – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; hand sink in employee restroom doesn’t have hand soap or paper towels; temperatures of bacon and raw beef in the reach-in cooler are elevated; reach-in cooler is not maintaining foods at the proper temperature; pasta, mac ‘n cheese, gravy, crab dip and potato salad don’t have date marks; mold is growing on plastic flaps in the doorway of the beer walk-in cooler; coving in employee restroom is missing which is revealing holes in the wall; wet mop is not being hung to dry; hoods are dusty and greasy.

Chuy’s, 11229 West Broad Street – The following violations were reported during a routine inspection: person in charge did not respond correctly to employee health questions from Environmental Health Services; observed employee handle ready-to-eat chips with bare hands; hand sink at wait station doesn’t have hand soap and it is being used as a dump sink; multiple hand sinks are blocked by equipment; observed dried food debris on food processor blades in clean storage; observed mold/mildew buildup in ice machine and ice bins, on citrus dicer and on bar soda sprayer; ranchero on the hot line is not at the correct temperature; temperatures of chimichanga and shredded cheese in prep top unit are elevated; concentration of sanitizer in bar three-compartment sink is too high; containers of white powder are unlabeled; buckets of drink supplies and citrus are incorrectly stored on the floor of the beer cooler; there is zero sanitizer in bucket for wet wiping cloths; single-use plates in storage are not stored inverted; a tray of mugs needs cleaning; cast iron fajita trays are cracked and chipped; plastic lexan containers are cracked; wire basket and shelving throughout the kitchen are rusty; dicer blades are in disrepair; cutting boards are deeply scratched and stained; wood shelf in back storage needs sealing; observed dried food debris and grime buildup on prep top units, racks in freezer, speed racks and on shelves under drink area hand sink; observed grime and dust buildup on ceilings and drains under bar sinks; floor around bar area drains is in disrepair.

Sparrow’s Pizza, 6313 Rigsby Road – The following violations were reported during a routine inspection: facility does not have written procedures for cleaning vomit/diarrhea; a tin can is being reused to hold the pizza cutter; a paper cup is being used to store sugar; ceiling is not smooth and easily cleanable.

Place at Innsbrook, 4036-C Cox Road – The following violations were reported during a routine inspection: observed dark buildup in kick plate crevice inside the ice machine; foods past their expiration date found in the walk-in; two cutting boards have dark stains; walls throughout the facility have staining, particularly behind the water heater, in the dish area, and in the hall towards chemical storage; large shelving in the furniture storage room has stained and chipped paint; observed buildup of grease under equipment and on casters on the cook line; metal strip at the bottom of the walk-in freezer door is rusty; observed rust in the cracks of metal flooring in the walk-in cooler.

True Taste 1, 1090 Virginia Center Parkway – The following violations were reported during a routine inspection: hand sink is blocked by boxes and there are items in the sink (dish soap, sponge, etc.); cooked chicken incorrectly stored in raw chicken cardboard box; several foods are uncovered (orange peels in dry storage, raw meats and cut vegetables in lowboy, and noodles, raw meats, egg rolls, cut cabbage and broccoli in walk-in cooler); raw beef incorrectly stored next to cooked noodles and over vegetables in lowboy; raw beef sticks incorrectly stored over egg rolls in standing fridge; observed mold buildup on ice machine and soda nozzles; no date marks found on egg rolls, fried rice, wontons and noodles; packs of cigarettes incorrectly stored on shelf near food; orange peels incorrectly stored with nails, tools and other hardware; rice noodles, onions, cut veggies and beef incorrectly stored on the floor; scoop for rice is being stored in standing water; facility doesn’t have test strips for three-compartment sink; several surfaces need cleaning (inside prep units, gaskets, racks, small hanging fans, top part of rice hot holding units, underside of main cooking station); hood filters on the cook line have heavy accumulation of grease.
