Thai Diner, 8059 West Broad Street – The following violations were reported during a routine inspection: facility does not have a certified food protection manager; multiple employee drinks and food incorrectly stored near food for customers; raw eggs incorrectly stored over cut cabbage in prep top unit and over noodles and next to onions in lowboy unit; raw shrimp incorrectly stored over sauce in lowboy unit; raw chicken incorrectly stored next to raw shrimp and calamari in reach-in unit; observed black mold (slight buildup) on side of ice machine in dry storage room; observed food debris greater than one day old on knives stored clean in utensil container on prep table; temperatures of squash, cooked broccoli and cooked chicken in prep top unit are elevated; multiple white powders on cook line are unlabeled; multiple dead bugs found on pest control trap/tape under prep sink; multiple boxes of oil, food and drinks are incorrectly stored directly on the floor; ladles on the cook line are not stored inverted; knives are not being stored properly; a scoop is stored in water on cook line but the water is not hot enough; refrigeration unit in dry storage room has torn gasket; cutting boards on cook line and in dry storage are heavily scratched; observed debris on racks, reach-in freezer units, racks and shelves in dish room, fans on shelf next to refrigerator, plates stored clean on shelf and in dry storage room; observed debris/grease under fryers, in between equipment, on hood filters and hood, and on shelves; floor tiles in dish room and in prep area on the cook line are cracked; observed a hole in the ceiling in dish room (a cardboard box is being used to cover the hole); paint is chipping on dish room wall; ceiling tiles in dry storage room are not easily cleanable; observed debris/stains on floors, walls and ceilings throughout the facility, especially under equipment on cook line and in dry storage room.

Apple Spice Junction, 1504 East Parham Road – The following violations were reported during a routine inspection: observed buildup greater than one day old on the slicer; spray bottles by three-compartment sink are unlabeled; two-door unit by dry storage and three-door unit that holds deli meat have torn gaskets; racks by dish area and in walk-in outside are rusty; observed some food debris in gaskets; fans in units are dusty; observed mildew/dust on racks in cooler outside.

Parham Doctors’ Hospital, 7700 East Parham Road – The following violations were reported during a routine inspection: hand sink in dishwash room doesn’t have paper towels; wire racks in walk-in cooler and warmer are rusty; observed dust and dried food debris on wire racks and shelves; grease buildup found on cook line equipment utensil bins; hood system and ceiling panels/vents are dusty; observed food debris on floor under equipment.

First Watch Restaurant, 11721 West Broad Street – The following violations were reported during a routine inspection: two spoons found in prep area hand sink (hand sinks are for handwashing only); observed food debris on wire baking rack stored clean; temperature of pancake batter is elevated; some plastic lids on clean dish rack have areas of deep pitting; cutting boards on the cook line are scratched and discolored; several areas are dirty (exteriors of cook line equipment, walk-in cooler racks and fan guard, top of dishwasher, cook line floor, and dish area wall); mop sink doesn’t have a dual check valve installed; observed splatter stains on mop sink walls and near back storage rack; mop is not hanging to dry.

Bon Secours St. Mary’s Hospital, 5801 Bremo Road – The following violations were reported during a routine inspection: temperatures of macaroni made yesterday and pancake batter made this morning are elevated; temperatures of pork on patient line, baked chicken in warmer and baked chicken on line are too low; temperatures of Monterey jack cheese on sandwich line and raw chicken on counter are elevated; no date marks found on two pans of sausage; facility does not have a written time control policy; water in squeeze bottle is unlabeled; pots are pitted and need to be replaced; bread racks are being used as shelving; holder for single-service cups is made of bare wood; no data plate found on Jackson; panini grill is soiled; ceiling vents are dusty; one area under the hood has cleaner residue; there are holes in the wall and paint is peeling in grill storeroom; coving tile is missing in the kitchen.

Deca’s Café LLC, 391 Azalea Avenue – The following violations were reported during a routine inspection: walls in the dry storage room are chipping and no longer smooth and easily cleanable; a wet mop is sitting in an empty mop bucket instead of being hung to dry.

Top’s China, 68 South Airport Drive – The following violations were reported during a routine inspection: facility does not have a certified food protection manager; observed multiple employees wash their hands at three-compartment sink instead of hand sink; hand sink doesn’t have paper towels; a hand sink is blocked by multiple buckets; soups in hot holding units are uncovered; temperatures of cooked noodles, cooked shrimp and raw chicken in walk-in cooler are elevated; walk-in cooler doesn’t appear to be working properly; proper cooling methods not used for egg rolls cooling at room temperature and fried chicken cooling in deep containers; observed dead roaches throughout the kitchen and mouse droppings throughout back of kitchen; egg cartons incorrectly used as a dish holder; observed menus incorrectly lining containers of egg rolls; shelf lining in walk-in cooler is damaged; cardboard incorrectly lining shelves and refrigeration; facility doesn’t have chlorine test strips for bleach sanitizer; observed dried food debris and grime on shelves throughout the kitchen, in refrigeration units and on/in paper towel dispenser; lights are out above the cook line; ceiling tiles at front of kitchen are damaged; observed dried food debris, grime and evidence of pests on floors, especially close to walls; hood system and vents are heavily coated in grease.

K&K Crabs, 3302 Williamsburg Road – The following violations were reported during a routine inspection: raw swai and whiting filets and shrimp incorrectly stored over ready-to-eat food; temperature of BBQ chicken made this morning is elevated because it was not cooled properly; temperatures of cooked chicken and sliced tomato is two-door reach-in unit are elevated; unit doesn’t appear to be working properly; eggs are cooked to order but menu doesn’t contain a consumer advisory statement; chlorine isn’t at the proper level in sanitizer bucket; menu lists catfish but the establishment is serving swai filets; two containers of white powder in dry storage are unlabeled; observed six live German cockroaches in the front service area behind the counter; facility couldn’t provide test strips for bleach sanitizer.

Olive Garden Italian Restaurant, 7113 West Broad Street – The following violations were reported during a routine inspection: observed an employee wash hands without using soap; the HACCP plan is not being completely followed; wheels to the fryer are greasy; interior of dessert freezer and interior of warmer are dirty; water is pooling behind the cook line well; there is a leak at the hose reel when water is turned on; spray foam is exposed in several areas of the kitchen, especially the dish area; walk-in freezer floor seam is open/loose.

Mellow Mushroom, 12171 West Broad Street – The following violations were reported during a routine inspection: soap dispensers at two hand sinks in the kitchen are empty; observed stuck on food particles on a few clean containers on the dish rack; temperatures of tomatoes, provolone and other non-TCS foods are elevated in the make table across from the flat top; this unit doesn’t appear to be working properly; temperatures of sausage and pizza sauce are elevated; a bottle of Tums and orange cleaner are incorrectly stored with foods; a fly strip with dead flies is incorrectly stored over food; containers of food are incorrectly stored on the walk-in cooler and freezer floor; containers are wet nested on the dish rack; mop sink doesn’t have a dual check valve installed; ceiling in kitchen is yellowed; ceiling vents are dusty.

Cities Bar & Grill, 8004 West Broad Street – The following violations were reported during a routine inspection: observed some dented cans on storage shelf; wings should not be covered when cooling; wings have been out on the rack for too long; a light is out in the hood system.

Popeyes Louisiana Kitchen, 4402 South Laburnum Avenue – The following violations were reported during a follow-up inspection: temperature of raw chicken tenders in the make table are elevated; several areas need cleaning (prep line floor, floor near three-compartment sink, hand sinks, racks of biscuit reach-in, rolling rack near dish area, reach-in freezer, and chicken prep tables); mop sink doesn’t have a dual check valve installed.
