SpringHill Suites Richmond Northwest, 9960 Independence Park Drive – The following violations were reported during a routine inspection: kitchen hand sink doesn’t have soap; chemical dish machine has 0 ppm chlorine; temperatures of cantaloupe, salsa, spinach, shredded cheese and cream cheese on the line are elevated; temperatures of cream cheese in the reach-in and spinach in the kitchen are elevated; proper cooling methods not followed for melon in the reach-in unit (covered with saran wrap); observed multiple dead small flies in quat sanitizer test strip holder on wall above the three-compartment sink; air temperature of reach-in unit is elevated and needs repair; cabinet drawers in the kitchen are in disrepair; facility could not provide chlorine test strips during inspection; observed debris on shelves on the front line/back kitchen and in cabinets in back kitchen; floor fan is dusty; hand sink has low water pressure; faucet at three-compartment sink is leaking; observed chipping paint on wall and back of cabinets in the kitchen.

Jimmy John’s, 11301 West Broad Street – The following violation was reported during a routine inspection: one light over the sandwich line is burned out.

Hardee’s, 1120 East Nine Mile Road – The following violations were reported during a routine inspection: an employee drink without a lid is sitting by the drink station; observed an employee handle raw burgers and then touch clean utensils without washing hands; dried food debris found inside microwaves; temperatures of breading milk and chicken breading liquid are elevated; sausage patties in walk-in cooler are cooling in a covered container; observed dried food debris, grease and dust on several surfaces (outside of cooking equipment and refrigeration units, on gaskets and inside refrigeration units, on fry basket rack, drive-thru monitors, hood filters, fry station, inside and outside the hot holding cabinet, on speed racks and walk-in fan grates); observed heavy dust buildup on ceiling vents; observed grease buildup on floors of walk-in and throughout the kitchen, on mop sink floor and on walls throughout the kitchen and storage.

Taco Bell, 9420 West Broad Street – The following violations were reported during a follow-up inspection: cutting boards on the line are heavily scratched; floor tiles in dish area and cook line are cracked; observed low grouting throughout the facility.

Dunkin Donuts, 4050 Spring Oak Drive – The following violations were reported during a routine inspection: facility does not have a certified food protection manager; floor drain under the three-compartment sink is dirty.

Ledo Pizza & Pasta, 4925 West Broad Street – The following violations were reported during a follow-up inspection: raw burger incorrectly stored next to chopped onion and soup in lowboy sandwich unit in back kitchen; lowboy unit next to two-door freezer has a torn gasket; observed low grouting throughout the facility.

Peter Chang, 11408 West Broad Street – The following violations were reported during a follow-up inspection: walk-in cooler racks are rusty and have chipped paint; one light is burned out under the hood; cutting boards at back prep table are scratched and stained; observed grease buildup on side crevices of equipment on the cookline; hood filters are greasy.

Five Guys Burgers and Fries, 10841 West Broad Street – No violations reported during a routine inspection.

Arby’s, 4250 Pouncey Tract Road – The following violations were reported during a routine inspection: temperature of butter in the make table is elevated because it was overfilled; observed grease buildup on back surfaces of the fryers.

Silver Diner, 10890 West Broad Street – The following violations were reported during a routine inspection: observed cleaning materials in the hand sink in the corner of expo (hand sinks are for handwashing only and must remain clear); observed pink and brown film on crevices of ice machine; temperatures of hummus and tomatoes in the under counter fridge are elevated; white rack across from waffle station and beer cage in the walk-in are rusty; cutting boards on dish rack have deep scratches.

Arianna’s Italian Grill, 5107-5109 Lakeside Avenue – No violations reported during a follow-up inspection.

Hibachi Sushi & Supreme Buffet, 8087 West Broad Street – The following violations were reported during a routine inspection: hand sinks did not have hot water at beginning of inspection; rack incorrectly stored on top of hand sink near dish area (hand sinks are for handwashing only); did not observe a written agreement from salmon supplier that salmon is farm raised and pellet fed; raw eggs incorrectly stored over cilantro in left reach-in unit; raw chicken incorrectly stored over raw beef in right reach-in unit; raw pork incorrectly stored above peeled green onions and raw beef incorrectly stored next to sticky rise in both walk-in coolers; chlorine level at three-compartment sink is 10 ppm; observed food debris on multiple metal pans stored lean and on clean rack against wall in dish area; observed debris on can opener blade on prep table; time stamp for sushi rice left out is incorrect; manager states that steamed crab meat on the hot line is held on temperature rather than time (time control policy states that all hot foods on buffet line are held on time as a public health control); isopropyl alcohol incorrectly stored next to cooking wine and margarine on shelf next to convection oven; multiple white powders on cook line are unlabeled; observed openings on bottoms of back doors in back kitchen; many food items are incorrectly stored on the floor (potatoes, sauces and noodles in walk-in cooler, oil boxes on cook line, and beer at bar area); observed wet nesting of metal pans on rack in dish area and on prep table at cook line; observed torn gaskets/deteriorating gaskets on multiple units throughout the back kitchen; racks are rusty; leftmost fryer basket at fryer station is damaged; walk-in freezer door is not closing properly; several cutting boards are stained and/or scratched; observed debris on several surfaces (sides of grill at hibachi station, on racks and underneath counters at sushi station, on racks throughout back kitchen, fan guard in rightmost walk-in cooler, shelves at service station, walls in back kitchen, and floors and corners throughout service station and back kitchen); observed debris and grease in between equipment, on hood filters and under wok station on cook line; hose underneath dish machine does not have the proper backflow prevention device installed; faucet handle at three-compartment sink behind cook line is leaking; water is slow to drain at hand sink behind cook line; multiple pieces of equipment/items throughout back kitchen are not being used and should be removed; floor tiles are cracked at hibachi grill, service station on right, back of cook line, next to drink storage room, back of storage room and near back door in kitchen; observed low grouting on floors throughout the back kitchen; paint is chipping on the ceiling near cook line next to kitchen entrance doors; observed ice and trash on floor of walk-in freezer.
