Ledo Pizza & Pasta, 4925 West Broad Street – The following violations were reported during a routine inspection: certified food protection manager certificate is expired; hand sink in front of house doesn’t have hot water; raw ground beef incorrectly stored next to sliced green bell peppers and ground beef incorrectly stored over marinara in lowboy unit; raw shell eggs incorrectly stored above boxed mushrooms and sweet sauce containers in walk-in cooler; observed food debris greater than one day old on knives and ladle in utensil container stored clean on rack; observed debris on can opener blade and on plastic panel of ice machine near the dish area; temperatures of mozzarella in prep top unit and bag of spinach and sliced cheese in another prep top unit are elevated; no written date marks found on fettucine and pre-portioned nacho cheese made the previous weekend; bottles of personal medications are incorrectly stored on a cart with clean plates used for customers; boxes of pizza sauce incorrectly stored on the floor next to prep table; mozzarella cheese boxes incorrectly stored on walk-in cooler floor; utensils are stored in water on the stove but the water is not hot enough; disposable boats are not being stored inverted on top of the lowboy freezer next to the cookline; cutting boards on sandwich unit next to grill and behind faucet of three-compartment sink are heavily scratched; lowboy unit next to two-door freezer has a torn gasket; observed debris on many surfaces (on racks and shelves throughout, on filters, pipes, light bulb covers, inside of microwave, in between equipment, under grill, under and on sides of fryer on the cookline, on side of two-door lowboy freezer unit, on bottoms of pizza pans, shelf above dough station, handles of fryer baskets, top of dish machine, plates stored clean next to soda fountain machine); observed debris/stains on floors, walls and ceilings throughout the facility, especially behind and under equipment; low grouting found throughout the facility.

Wingate by Wyndham, 13991 North Gayton Road – The following violations were reported during a routine inspection: one light is burnt out in the kitchen and the lighting is dim; dishwasher isn’t working; observed old beer residue at the bottom of the beer cooler at the bar; hand sink in kitchen has a leak.

Hilton Garden Inn, 441 International Center Drive – The following violations were reported during a routine inspection: facility could not provide a parasite destruction letter from the salmon provider; inside of ice machine is dirty; burgers and salmon that are cooked to order are not linked with a consumer advisory on the menu; three cleaning products at the bar are unlabeled; two lights are out under the hood; top of dish machine has dust and crumbs; mop sink doesn’t have the proper backflow prevention device installed.

The Patron Mexican Restaurant & Cantina, 5990 Audubon Drive – The following violations were reported during a follow-up inspection: observed a sticky trap with about 10 German cockroaches; observed ice in front hand sink, peppers/food debris in service area hand sink and a cutting board blocking dish area hand sink (hand sinks must be accessible and they are for handwashing only); front prep unit is not working properly; temperatures of salsa and crema in prep unit are elevated; pork, chicken and refried beans made the previous day do not have date marks written on them; salsa is improperly cooling in deep bins that are covered; pork and black beans incorrectly stored on freezer floor; lighting at the bar is too dim.

Caffe Colao, 8901 Patterson Avenue – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager (manager failed the exam but plans to retake it); observed manager touch cooked vegetables with bare hands; hand sink next to sandwich unit doesn’t have paper towels; observed utensils in metal pan and sponges in hand sink next to hot line, an employee washing dishes at hand sink next to sandwich unit and utensils and equipment in back kitchen hand sink (hand sinks are for handwashing only); raw beef incorrectly stored next to cooked pork belly and cooked yucca incorrectly stored next to raw kibbeh in commercial two-door unit; observed pink mold on plastic panel inside ice machine; temperature of cooked beef made yesterday is elevated because it is cooling while wrapped in saran wrap; temperatures of beef rib on hot line and butter and fresh garlic on the stove top are too low; temperatures of several foods are elevated (shredded cheese, parmesan cheese, sliced cheese, raw pork, sweet plantain, spaghetti, raw shell eggs, ham, salami, potato salad and raw beef); multiple foods do not have expiration dates written on them (pork, empanadas, yucca, opened package of mozzarella cheese, spaghetti and cooked beans); menu does not have a consumer advisory statement regarding tres leches cake; proper cooling methods not used for soup or cooked chicken; observed catfish and salmon incorrectly thawing in reduced-oxygen packaging; several white powder containers are unlabeled; oil boxes, single-service boxes and boxes of wine, beer and soda are incorrectly stored on the floor; to-go trays are not being stored inverted; cutting boards throughout the facility are heavily scratched and stained; commercial two-door reach-in unit has broken door handles; sandwich unit in front of house has a torn gasket; drink cooler and display cooler in back kitchen are not working properly; observed debris/buildup/grease on lowboy unit and shelf in front of house, on light bulb cover on hood, on pipes, electrical outlet/connections, in between equipment on the cookline, on racks where dried goods are located, and on top of knife holder in prep area; fans throughout back kitchen are dusty; hand sink faucet in back kitchen is dripping continuously; water is dripping from underneath faucet at three-compartment sink; observed heavy debris and grease on floors and walls on cookline, especially under equipment; observed debris and litter on floors throughout the facility; walls in back kitchen are stained; ceiling near dish area is dusty; paint is chipping on walls in back kitchen.

Henrico Health & Rehabilitation Center, 561 North Airport Drive – The following violations were reported during a routine inspection: multi-quat sanitizer at three-compartment sink wasn’t at the proper level; time control is used for all items in hot well for breakfast, lunch and dinner but no written policy is available; plastic spoons, forks and knives are stored uninverted in cup; mop sink doesn’t have the appropriate backflow prevention device installed; one light is out in the kitchen.

Best Western Richmond Airport Hotel, 5300 Airport Square Lane – The following violations were reported during a routine inspection: facility does not have a certified food protection manager; hand sink doesn’t have paper towels or soap; temperatures of cream cheese, liquid egg and butter are elevated; wet wiping cloth is not being stored in a sanitizer solution; three homestyle refrigerators do not appear to be working properly.

Hardee’s, 4841 South Laburnum Avenue – The following violations were reported during a routine inspection: facility does not have a certified food protection manager; front hand sink doesn’t have soap; temperature of country ham in lowboy is elevated; gravy is on a time control policy but there was no written time indicating when to discard; a bag of mac ‘n cheese was thawing in the three-compartment sink’s “rinse” compartment while dishes and sanitizer were filled; thermometer outside the walk-in is broken; bag of sugar and a salt shaker above the prep line are unlabeled; Styrofoam to-go trays are not stored inverted; prep unit lowboy and walk-in cooler door have torn gaskets; several areas are dirty and need cleaning (containers at the chicken station, rolling racks, racks in dry storage, pit room, walk-in cooler floors and fans); gasket on walk-in cooler is moldy; sides of fryer and basket rack are greasy; broiler is greasy and dirty; observed food debris on bottom of lowboy cooler on the prep line; flat top grill has heavy carbon buildup; mop sink doesn’t have the appropriate backflow prevention device installed; ceiling vents are moldy and dusty; floors are greasy; employee’s sweatshirt incorrectly stored on the counter of the service area.

The Cocky Rooster, 12171-D West Broad Street – The following violation was reported during a routine inspection: observed grease buildup under the fryers.

Bowl America—Short Pump, 4400 Pouncey Tract Road – The following violations were reported during a routine inspection: facility doesn’t have written procedures for the cleanup of vomit or diarrhea; observed grease or debris on can opener blade, drawer handles, inside freezer on the cook line and on counter under the cook line; observed debris and crumbs under the front service area.

Firehouse Subs—White Oak, 4501 South Laburnum Avenue – The following violations were reported during a routine inspection: facility does not have a certified food protection manager; three-door reach-in unit has torn gaskets; inside cooling units and far hand sink on the line need cleaning; bread toaster has heavy crumb buildup; black mold is growing behind the three-compartment sink faucet; three-compartment sink faucet is leaking; floors are dirty with food debris; mops are not being hung to dry.
