E-Loteria Botanas & Ice Cream, 9022 West Broad Street – The following violations were reported during a routine inspection: facility could not provide a certified food protection manager certificate; sanibucket found inside hand sink in the dish room (hand sinks are for handwashing only); facility could not provide a parasite destruction letter from the tilapia provider; observed food debris greater than one day old inside of food processor, blender and juicer on shelf under the prep sink; temperatures of melon in gaspachos, pico de gallo, melons, refried beans, tamales and raw beef are elevated; the menu doesn’t have a consumer advisory statement for ceviche served on weekends and for raw shrimp with lime juice; multiple powders on cook line and a container next to the sandwich unit are unlabeled; knives incorrectly stored between crevices of hot holding unit and sandwich unit in front; wooden spoons stored on rack across from three-compartment sink are not stored inverted; soup cups on shelf under hot line, boats on shelf above sandwich unit, cups in dry storage closet and spoons above sandwich unit are all not stored inverted; person in charge states that cups for gaspachos are sometimes re-used (single service items cannot be reused).

Casa del Barco at Short Pump, 11800 West Broad Street – The following violations were reported during a follow-up inspection: one light is burned out under each hood system; wire rack in dish area has chipping paint; wire rack in walk-in is rusty; observed debris in prep area storage drawers; wall behind dishwasher in the kitchen is dirty.

Frank’s West, 11238 Patterson Avenue – The following violations were reported during a routine inspection: observed ice in the hand sink and an employee fill a pitcher of water for the coffee maker from the hand sink (hand sinks are for handwashing only); food debris found on slicer blade and blade guard; inside of ice machine has pink and black residue; no sanitizer is detected in the dish machine; temperatures of mozzarella, ricotta, sausage and sauce in the lowboy unit are elevated; lowboy unit does not appear to be functioning properly; knife incorrectly stored in crevice between refrigerator units; small green cutting board is worn; observed ice buildup in freezer because of leaking water; wood underneath microwaves is not sealed.

West Lake Chinese Restaurant, 11256 Patterson Avenue – The following violations were reported during a routine inspection: observed an open water bottle next to soups in food prep area on the cook line; observed an employee wash but not sanitize a small pot before filling with raw chicken; food splatter found in microwave; boxes of food incorrectly stored on walk-in freezer floor; jugs of oil incorrectly stored on floor in dish area; dish machine is not in good repair; faucet at back hand sink drips continuously; food splatter found on wall at food prep area near dish machine; wall across from dish machine is soiled; floor tiles on cook line are cracked; wall tile near three-compartment sink is missing; hood filter above fryer has grease buildup.

Caffespresso, 1127 Gaskins Road – The following violations were reported during a routine inspection: observed pink and black mold on panel of ice machine near dish area; quaternary ammonium level at three-compartment sink in dish area is at 0 ppm; provolone, fontina cheese, ham and mozzarella cheese are all past their expiration dates; hand sanitizer and gel incorrectly stored next to bowls used for customers; temperatures of eggs in lowboy unit and egg tart in display case are elevated because they were not cooled properly; facility does not have a working food thermometer; ice scoop incorrectly stored in ice at ice machine near dish area; forks, spoons and knives on counter are not stored inverted; whisk and utensils in utensil container are not stored inverted; clean utensils in the dining room are not stored inverted; to-go trays and plates are not stored inverted; multiple units in the kitchen have torn gaskets; cutting board for bread and cutting board next to panini press are both heavily scratched; wood shelf next to two-door lowboy and wooden wall above three-door prep table are not sealed; observed debris on shelves and racks throughout the entire kitchen as well as debris inside of freezer units; hand sink is slow to drain and does not have cold water; observed debris on floor, especially underneath equipment throughout the kitchen; walls throughout kitchen are stained; wall behind chest freezer in kitchen has chipping paint.

Taco Bell, 9420 West Broad Street – The following violations were reported during a routine inspection: observed food debris greater than one day old on two scoops and one ladle hanging above three-compartment sink; cutting boards on the line are heavily scratched; observed debris on sides and behind tortilla panini press, in utensil container and shelves under cash register, on shelves on the line, in lowboy unit on the line, on racks throughout the facility, on hood filters, in between equipment and under fryers on the cook line; racks on front cashier line are rusty; racks and pipes in walk-in freezer have accumulation of ice; observed low grouting throughout the facility; floor tiles in dish area/cook line are cracked; observed debris/stains/grease on walls throughout and trash/debris on floors under equipment and in floor corners throughout, especially the drive-thru area; observed stains and dust on ceiling, ceiling vents and light shield.

My Empanada, 1421 Blue Jay Lane – The following violations were reported during a routine inspection: temperatures of breakfast, beef and chicken empanadas are too low because the warmer they are stored in is not working properly; salsas made the previous week do not have date marks; chlorine level in sanitizer bottle used for food contact surfaces is greater than 200 ppm; ceiling tiles in storage area are not made of an easily cleanable material.

Doubletree By Hilton, 445 International Center Drive – The following violations were reported during a routine inspection: two hand sinks don’t have paper towels; hand sink at bar is blocked by a sanitizer bucket; facility could not provide a parasite destruction letter from the salmon provider; ice machine in service area has heavy black mold buildup on inside lip; no date marks found on grilled chicken in lowboy on the cookline or open mashed potatoes bag in walk-in cooler; grilled chicken in walk-in cooler is past its expiration date; salmon incorrectly thawing in reduced-oxygen packaging; ice bucket next to the ice machine in the service area incorrectly stored on the floor and uninverted; cutting board at bar is heavily scratched and stained; two-door reach-in cooler in the kitchen is broken; three lights are out under the hood; mop sink doesn’t have the appropriate backflow prevention device installed; floor in dish area has pooling water; wall behind rinse sink is dirty; cook line floor is sticky; mops are not being hung to dry; a ceiling tile is missing.

Sheraton Richmond, 5501 Eubank Road – The following violations were reported during a routine inspection: hand sink near dish area doesn’t have soap because the dispenser is broken; facility could not provide a parasite destruction letter from the salmon provider; ice machine has a layer of mold buildup; soda gun nozzle at the bar is soiled; temperature of chicken made the previous day is elevated; the consumer advisory on the menu is not connected to burgers by an asterisk; did not observe thermometers (besides high temp) in the kitchen; powdered sugar and sugar are not labeled; to-go trays are not stored inverted; facility doesn’t have test strips for chlorine or quaternary sanitizer; hand sink is dirty; mop is not being hung to dry; hood filters are greasy.

Hyatt House Richmond/Short Pump, 11800 West Broad Street – The following violations were reported during a routine inspection: no certified food protection manager credentials observed during inspection; person in charge is unaware of the written procedures for cleanup of vomit/diarrhea; kitchen hand sink doesn’t have hot water; observed debris on glass bowls in back storage room; ice machine has a pink/brown film on it; juice nozzles are coated in old dried juice; sliced cheeses and pre-boiled eggs do not have date marks; cutting board at make table and green board on storage rack are torn and discolored; facility does not have an irreversible thermometer available for use at the dishwasher; green rack in the walk-in is dirty; ceiling vents in the kitchen are dusty; hood filters are greasy.
