Picciotti’s Pizza, 9039-3 West Broad Street – The following violations were reported during a routine inspection: container of raw chicken incorrectly stored above a box of mozzarella cheese and next to a bag of basil in reach-in unit; observed food debris greater than one day old on knives on the wall near slicer and on utensils in rack next to prep sink; temperatures of mozzarella and marinara prepared the previous day are elevated because they were not cooled properly; temperatures of ham and roasted red peppers in low boy middle sandwich unit are elevated because the unit is not cold enough; pizza left out since 11 a.m. did not have a written time on it; chlorine sanitizer level at three-compartment sink is greater than 200 ppm; a bottle of medicine is incorrectly stored next to items that are used to serve customers; stainless steel cleaner incorrectly stored next to vegetable oil on shelf under prep sink; temperature of sliced tomatoes in reach-in unit is elevated; weather shielding is needed to prevent the entry of pests at bottom and side of back door; forks and spoons in rack on shelf in back kitchen are not stored inverted; to-go trays in front pizza area are not stored inverted; gasket is torn in reach-in refrigeration unit; multiple cutting boards need replacing; hood filters are dusty; observed debris under fryers, on sides of grill, in between and on equipment, on shelves throughout front and back of house, and on rack behind prep sink; observed pans with sticker labels stacked on rack near three-compartment sink; hand sink on cookline is slow to drain; faucet at prep sink has a leak; prep sink in back kitchen does not have an indirect drain; observed debris on floor under equipment and in mop sink area; observed splatter on walls behind prep table where slicer is located; floors throughout the facility have low grouting; wall is coming off at three-compartment sink; ceiling tiles above pizza prep area and in front of house are not easily cleanable; ceiling tile in back kitchen is not placed properly; ceiling has holes; multiple equipment/items are not being used and should be removed from the facility.

Lakeside Moose Lodge, 1207 Hilliard Road – The following violations were reported during a routine inspection: facility does not have a certified food protection manager; women’s bathroom doesn’t have a hand wash sign; eggs, ground beef and steaks incorrectly stored above ready-to-eat food; multiple prepped items (chicken salad, cheese sauce, deli meat, tomatoes) do not have date marks; observed dust and debris on racks next to cookline and on fans in units; two-door unit next to cookline needs cleaning; observed buildup on hood filters.

Dunkin Donuts, 2300 Hungary Road – No violations reported during a routine inspection.

Murat’s Institute & Café, 12456 Gayton Road – The following violations were reported during a routine inspection: several glass jars with pepper sauce at the front counter are past expiration date; these jars were canned, which is a specialized process that requires a variance, and they cannot be sold or given to the public; some containers of white powdery foods are unlabeled.

Honey Baked Ham, 200 Towne Center West Boulevard – The following violations were reported during a routine inspection: temperature of lettuce in the make table is elevated; spray bottle of water used for glaze is not labeled; walk-in fan guards are dusty; leak found under the make table.

Bernie’s Sub & Pizza Shop, 415 North Ridge Road – The following violations were reported during a routine inspection: slicer that hasn’t been used today has food debris on it; temperature of meatballs is too low; temperatures of provolone and sauce on the counter and sliced turkey in two-door unit are elevated; no dates found on opened bulk ham or salami; threshold and gasket of front two-door unit are both in disrepair; cardboard incorrectly being used inside the refrigerator; burger press is attached to a piece of wood (wood is not hard maple and cannot be used for food equipment); women’s restroom door is not self-closing; there are holes in walls; coving to three-compartment sink has gaps; an old Coca Cola fridge is not being used and should be removed from the facility; ceiling, floors and pipes along walls are dirty; ceiling vents and ceiling fan are dusty.

Which Wich, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: facility does not have written procedures for cleaning up vomiting/diarrheal events; temperature of two containers of meatballs is elevated because the container is over filled; insect light incorrectly placed on top of ice machine; trash can incorrectly placed on top of sugar bin so that the floor could be cleaned; plastic Ziploc bags are single-use and should not be reused; freezer walk-in door has a torn gasket.

Lanay Café, 2810 North Parham Road – The following violations were reported during a routine inspection: facility does not have written procedures for cleaning up vomiting/diarrheal events; seam between front two cutting boards has food crumbs; temperatures of ham and salami are elevated; lower make table air temperature is too high; bread racks incorrectly used as shelves; blue shelves and lower particle board shelf on the line are no longer smooth and easily cleanable; shelf/cabinet under hand sink is dusty; walk-in door is dusty; observed loose coving across from three-compartment sink.

Olive Garden Italian Restaurant, 4467 South Laburnum Avenue – The following violations were reported during a routine inspection: service area hand sink is blocked by a backpack; bar hand sink and dish area hand sink are missing soap and paper towels; hot water at hand sink in front kitchen doesn’t reach required temperature; soda gun nozzle at the bar is dirty; chicken shredder on the cookline is soiled with debris and not being cleaned between uses; temperatures of spinach artichoke dip, kale, shredded cheese and tomato sauce are elevated; no stickers to indicate time control policy found on seasoned tomatoes, lasagna, alfredo, tomatoes and lettuce; two sanitizer bottles at the mop sink are unlabeled; records for HACCP are incomplete on Dec. 7, 10 and 15; meat sauce incorrectly cooling in a deep covered container; observed two employees prepping food with uncovered beards and one employee prepping bread without a hair restraint; tongs on the cookline stored incorrectly; spoons, forks and to-go trays are not stored inverted; soiled cardboard under rack in dry storage needs to be removed; four lights are out under hood; several items are broken (drip wells on cookline, racks in dry storage, two low boy units on the cookline); facility does not have devices/test stickers to measure water at high temp dish machine; drip well and salad/soup plate units at front of kitchen are dirty with food debris; sides of equipment on the cookline are greasy; metal piece on wall near the back door is dusty; breadstick warmer is dirty; hand sink in back of kitchen is leaking; grease trap is soiled; observed debris surrounding dumpster and garbage cans were open on the side; floor on the cookline has pooling water; floors under racks in dry storage are cluttered with trash; ceiling vents throughout the facility are dusty; walls and doors are dirty; mop is not being hung to dry.

Zaxby’s, 5816 West Broad Street – The following violations were reported during a routine inspection: paper towel dispensers are empty at two back hand sinks and in the women’s restroom; temperature of raw chicken in the fryer drawer is elevated; observed dried food debris on several surfaces (sides of fryers, inside fryer cabinets, on breading containers, on gaskets of refrigeration units, on heat lamp on hot holding line; mop sink doesn’t have a dual check valve installed; multiple hood lights are burnt out; observed heavy mold/mildew buildup in mop sink area.
