Bonefish Grill, 11251 West Broad Street – The following violations were reported during a routine inspection: person in charge was cutting raw tuna and failed to wash hands before touching bins for food; front hand sink doesn’t have paper towels; a lime with mold on it was found in the walk-in cooler; facility could not provide a parasite destruction letter from salmon distributor; raw tuna incorrectly stored next to uncovered ready-to-eat salad; observed food debris on several spoons and scoopers hanging in clean dish area; temperature of tomatoes sliced the night before is elevated; temperatures of buttermilk for frying, half-and-half, bacon and pasta are elevated; three sanitizer bottles are unlabeled; fish incorrectly thawing in reduced oxygen packaging in walk-in cooler; containers of powdered sugar and flour throughout the kitchen and water on the cook line are unlabeled; bulk bins of flour and batter were left open and uncovered; person in charge was prepping food without a hair restraint; lemon peeler was stored in lemons and ice cream and batter scoops were stored dirty in the prep top; a few plastic bins are stacked wet; to-go trays in the to-go station are not stored inverted; unit at bar is rusted; low boy near fryer has a torn gasket; microwave handles are broken; rubber spatulas are melted; several cutting boards are stained; inside of ice cream freezer is dirty with food debris; slimy residue found in hand sink; pans above range stove top have heavy carbon buildup; two faucets need dual check valves; pipes at mop sink are leaking; lid of outdoor garbage receptacle is not closed; spider webs found on ceiling above fryer area; dish area floor and floor in walk-in are dirty; fan grates in dish room and fan guard in walk-in are dusty; floor drain at the bar is dirty; observed low grout on floors in dish area and in kitchen; lighting on the cook line is not bright enough.

Rosa Bell’s Café, 6802 Paragon Place – The following violations were reported during a routine inspection: facility does not have a certified food protection manager; observed food debris in hand sink (hand sink is for handwashing only); observed food debris and dust on prep table counters, on shelves in the back area, and inside reach-in cooler.

Domino’s Pizza, 550 East Laburnum Avenue – The following violation was reported during a routine inspection: wiping cloths are being stored in a sanitizing solution but the sanitizer level is at 0 ppm.

Sea By Thai Restaurant, 2452 Old Brick Road – The following violations were reported during a routine inspection: person in charge was not able to identify the “big six” foodborne illnesses and their associated symptoms; there is not verifiable employee health training; an employee’s drink on the cook line is uncovered; an employee entered the kitchen, used her phone, and then put on gloves without washing hands; an employee cooked raw beef and did not wash hands after; hand sink near the cook line is blocked; employee was using hand sink to dump water; shell stock tags are being kept but they are incomplete; raw eggs incorrectly stored over vegetables in low boy cooler; raw beef incorrectly stored next to raw chicken; cooked shrimp incorrectly stored next to raw chicken; observed an open bag of tempura on cook line and curry was in refrigeration uncovered; tea and soda gun nozzles are soiled; veggie peeler in prep top and knife at bar were both stored dirty; metal bins are dirty and greasy; temperatures of tofu cooked yesterday, drunken noodles, noodles on ice on the cook line, noodles in low boy and green curry are elevated; tofu and chicken wrap filling made yesterday are not date marked; Tylenol found on cook line; several sanitizer bottles are unlabeled; temperature of chicken wrap filling is elevated; tilapia is incorrectly thawing in reduced oxygen packaging; containers of salt and flour are unlabeled; cashews, oil and drinks at the bar incorrectly stored on the floor; plates and spoons on the cook line are not being stored inverted; observed an employee drying with a cloth at the dish machine (allow to air dry); light is out under the hood; racks in dish area are rusty; fryer baskets are broken and/or greasy; three-compartment sink’s sanitizer mechanism is broken; gasket on low boy is torn; cardboard incorrectly lining shelf under microwave; cutting boards are stained; three-compartment sink is not large enough for the largest utensil; facility does not have bleach test strips; outside bulk bins are dirty; chlorine at three-compartment sink is moldy; ice machine vent is dusty; trash bin in women’s restroom doesn’t have a lid; oil receptacles are not covered in the refuse area; the Y-valve at the mop sink needs to be removed; hose at three-compartment sink doesn’t have an air gap; mops are not being hung to dry; lights are out in the kitchen; wall paint is chipping and walls are dirty; caulking at three-compartment sink is chipping and damaged; ceiling tiles are missing in the kitchen and men’s restroom; ceiling fans in restrooms are broken and dusty and ceiling fan in kitchen is dusty; several ceiling tiles have water damage; mop sink is dirty; floor drain is full of debris; drip tray is missing on the hood system; hood system in kitchen is greasy.

Arby’s, 5900 West Broad Street – The following violations were reported during a routine inspection: dried food debris found on can opener blade, inside microwaves on the prep line and on slicer blade on prep table in the back; temperature of rotisserie chicken pieces in hot-holding drawer and cheese in dispenser are too low; temperature of ice cream mix in the milkshake machine is elevated; cooked chicken in the walk-in cooler is not accurately date marked; fried onion straws that are on a time policy do not have a time mark; cup of sanitizer by the milkshake machine is unlabeled; observed several employees without hair restraints; observed an employee with a bandaged finger not covered by a glove; observed an employee with a wristwatch on; box cutters incorrectly stored in leather holders throughout the kitchen; tray holding milk jugs in the walk-in cooler is heavily rusted; blade on slicer in the back is in disrepair; mop sink doesn’t have the prep backflow device installed; observed dried food debris, dust and grime on several surfaces (outside of microwaves, on mix ‘n chill machine, knife holder, can opener holder, prep table post, prep table drawer and utensil bin, walk-in cooler fan grates and condenser coils, speed racks in walk-in cooler); dumpster lid is not closed; a light is out in the hood; back outer wall of walk-in cooler by the mop sink is in disrepair; observed mold and mildew on pipes at the mop sink; ceiling vents are dusty.

Tokyo Japanese Express, 10101 Brook Road – The following violations were reported during a routine inspection: back hand sink doesn’t have paper towels or soap; temperatures of veggies and fried rice are too low; dead mouse found on sticky trap under the front hand sink and animal droppings found in front cabinets; single-service cups incorrectly stored under piping of front hand sink; front cabinets are in disrepair; front grill plate is missing; ice machine doesn’t have a shield leaving it open to possible debris; facility doesn’t have a test kit for sanitizer at three-compartment sink; observed buildup on gaskets of every unit and on top gaskets of hot-holding for rice; observed buildup of debris in units and in containers holding utensils; coving tiles are in disrepair; caulking is missing near the mop sink; hood filters and wall behind the cook line are greasy; floor under equipment needs cleaning.

Five Guys Burgers & Fries, 1071 Virginia Center Parkway – The following violation was reported during a routine inspection: no paper towels at hand sink near dish machine or fry station.

Family Secrets Restaurant, 5310 Chamberlayne Avenue – The following violations were reported during a routine inspection: observed an employee wash hands in mop sink instead of hand sink; grime buildup found inside ice machine at bar; countertop in hallway into kitchen is chipped and the siding is falling off; cutting board is deeply scratched and stained; observed dried food debris, grime and/or dust on several surfaces (shelving throughout the kitchen, fan grate in two-door reach-in by the three-compartment sink, on push cart at entrance to kitchen); bar floors, especially under and behind equipment, have buildup of grime; bar floors are in disrepair.

Marco’s Pizza, 9681 West Broad Street – The following violations were reported during a routine inspection: manager was not able to identify the “big six” foodborne illnesses and their associated symptoms; hot water at hand sink near front lobby doesn’t get hot enough; quaternary ammonium sanitizer level is at 0 ppm at three-compartment sink; temperature of spinach in prep top unit is elevated; single-service trays on rack next to sandwich unit are not being stored inverted; cutting boards throughout the prep line are scratched; a cutting board on the line is made of wood and not easily cleanable; observed low grouting on floors throughout the kitchen.

Blaze Pizza, 4101 Dominion Boulevard – The following violations were reported during a routine inspection: facility does not have test strips for its veggie wash that is identified as an additive; observed small flies coming from the mop sink; walk-in fan guards are dusty.
