Table of Contents

Following are routine and complaint inspection reports for Henrico County food establishments inspected recently by the Henrico Health Department, according to Virginia Department of Health guidelines. Deficiencies are noted as “priority” or “priority foundation” (posing a direct or immediate threat to the safety of the food being served) or “core” (representing a failure of cleaning or maintenance). Inspections represent only a snapshot of the establishment on a specific date and time and may not be representative of its overall cleanliness, according to the VDH. Unless noted, all reported violations were corrected at the time of inspection or shortly thereafter.

OCT. 3
BJ's Restaurant and Brewhouse, 12010 West Broad Street – One priority violation reported (rice, rice/quinoa, lentil/bean observed at improper temperatures) and three priority foundation violations reported.
IHOP, 4840 South Laburnum – One priority violation reported (according to use-by date on prepared ready-to-eat food in reach-in fridge and walk-in, food already should have been discarded) and one priority foundation violation reported.
Little Asia, 3991 Glenside Drive – Two priority violations reported (egg and lettuce inside make table observed at improper temperatures, both were nested on top of other containers, milk observed at improper temperature in keg box; food-contact surfaces of equipment observed unsanitized because no sanitizer was being dispensed into dishmachine) and two priority foundation violations reported.
Little Szechuan, 10234 Staples Mill Road – No priority or priority foundation violations reported.
Ma & Pa's Diner, 5600 Williamsburg Road – One priority violation reported (raw foods of animal origin – steak – observed stored over ready-to-eat food in refrigeration unit) and two priority foundation violations reported.
Nine Mile Subway, 3606 Nine Mile Road – One priority violation reported (spray bottles observed stored next to clean dishes) and no priority foundation violations reported.
Nordstrom Espresso Bar, 11812 West Broad Street – No priority violations reported and one priority foundation violation reported.

OCT. 4
Atlas 42, 4032 Cox Road – Two priority violations reported (some moldy foods – tomatoes, cheese – found in refrigeration units; ham in prep top observed at improper temperature) and no priority foundation violations reported.
Hawk's Fish & BBQ, 3050 Williamsburg Road – Three priority violations reported (employee observed handling raw fish and failing to wash hands/change gloves before handling single-service items; raw fish observed stored above/adjacent to cooked potatoes in reach-in refrigerator; raw fish observed sitting out at improper temperatures) and one priority foundation violation reported.
Market Cafe, 10950 Nuckols Road – Five priority violations reported (cheese observed with black mold; raw eggs observed stored on top of naan; pizza observed at improper temperature; walk-in had elevated temperature, curd rice observed at improper temperature on service line; due to improper operation of low-temperature chemical sanitizing mechanical warewashing machine, equipment food-contact surfaces and utensils not sanitized after cleaning) and six non-priority violations reported.
Pizza Hut, 1164 Wilkinson Road – One priority violation reported (hose with spray nozzle attached to the 'Y' fixture observed at the mop sink) and one priority foundation violation reported.
Terra Nova Bar & Grille, 4700 South Laburnum – No priority violations reported and two priority foundation violations reported.
Thai Flavor, Food & Noodle, 4040 Cox Road – Two priority violations reported (some foods in large prep unit – tomato, fried tofu, bamboo shoots, beef, shrimp – observed at improper temperatures; rice noodle timers were not set or provided) and two priority foundation violations reported.

Daily Jolt, 7801 West Broad Street – No violations reported.

KFC, 4821 Williamsburg Road – No priority or priority foundation violations reported.
Zoe's Kitchen, 11801 West Broad Street – Two priority violations reported  (grilled veggies double-panned on line observed at improper temperatures; observed chicken salad on prep unit at improper temperatures) and one priority foundation violation reported.

OCT. 6
Highland Springs H.S., 15 South Oak Avenue – One priority violation reported (food in six-door refrigerator – mashed potatoes, gravy – observed at improper temperatures) and no priority foundation violations reported.
Little Caesar's Pizza, 5101 South Laburnum – No priority violations reported and one priority foundation violation reported.
Papa John's Pizza, 4744 South Laburnum Avenue – No priority violations reported and one priority foundation violation reported.
