Arthur Ashe Elementary School, 1001 Cedar Fork Road – The following violations were reported during a routine inspection: dried food debris found in microwave in dining room area; reach-in cooler and walk-in cooler door gaskets are stained black; walk-in cooler fan guards are dusty; there are dried food stains on kitchen ceiling tiles; ceiling tile near ventilation is dusty; there are dried food stains and debris on the wall under the serving line.
Crestview Elementary School, 1901 Charles Street – No violations reported during a routine inspection.
Adams Elementary School, 600 South Laburnum Avenue – The following violations were reported during a routine inspection: dried food debris found along rim of tilt kettle; temperature of milk in milk cooler is elevated (cooler was left open during service); wall fan in dish room is dusty; ceiling tile near ventilation and vent grate in ceiling are dusty.

Highland Springs High School, 200 South Airport Drive – The following violations were reported during a routine inspection: dried food debris found in microwave; mold found inside ice machine; kitchen hand sink is in disrepair; reach in cooler #2’s door gaskets are moldy; produce reach-in cooler’s door gasket is dirty; walk-in cooler wire racks are moldy.

Terra Nova Bar & Grille, 4700 South Laburnum Avenue – The following violations were reported during a routine inspection: buildup of coffee grounds found in hand sink behind buffet; temperature of day-old chili in reach-in is elevated because of improper cooling methods (stored in a deep, covered pan); egg bake and cooked rice in walk-in are past their expiration dates; no date marks found on cut melons and mixed fruit medley in reach-in or ham and corned beef in prep unit; breakfast and dinner menus don’t contain an adequate consumer advisory for eggs, burgers and steak that are cooked to order; multiple food items are stored directly on walk-in floor; fan grates and dry storage racks in kitchen prep area are rusty; slicer and ice machine are broken; cleaning is needed on rolling racks in walk-in, utensil storage racks, coffee prep area, and above the dishwasher; there are cracked and missing wall tiles under hood on cook line and behind stovetop; there are areas of low grout on cook line and dish area floor; wall paper is peeling in the dish room; door fixture in dry storage room is broken.
Hawk’s Fish & BBQ, 2903 Williamsburg Road – The following violations were reported during a routine inspection: there is 0 ppm sanitizer in three-compartment sink; observed food debris and grease buildup on fryer basket and utensils in dry storage rack; cooked pork butts in walk-in unit are not date marked; fryer baskets are broken; walk-in storage racks are rusty; observed buildup of flour and food debris on fans in kitchen prep area, hood system, and on top of ovens and fryers; hose is under pressure at mop sink without a proper backflow preventer installed; there is a slow leak on the filter system at three-compartment sink; paint is peeling on bathroom walls; ceiling tiles in dining area are stained.

Tasty Crab, 7801-A West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t name the “big six” reportable foodborne illnesses and the associated symptoms, nor could he verify that staff are trained on illness reporting requirements; facility doesn’t have written procedures for cleaning up vomit or diarrhea; hand sink at bar is being used as a dump sink as well; there is 0 ppm quat sanitizer at three-compartment sink; observed buildup inside soda gun nozzle at bar and unsweet tea nozzle in kitchen; dark mold-like substance found inside ice machine; temperatures of chicken wings, coleslaw in bowl and sweet potato fries in refrigerator unit on cook line are elevated; temperatures of raw shrimp, raw chicken, flounder and calamari on ice at cook line are elevated; no date marks found on cooked shrimp, cooked potatoes, sauces and wings; two chemical spray bottles are unlabeled; observed small flies in the dish area; ice scoop handle is in contact with ice at the bar; refrigerator is not maintaining foods at the proper temperature; walk-in freezer has a lot of ice buildup (air curtain is damaged and door doesn’t seal tightly); gasket of walk-in refrigerator prevents door from being shut completely; fryer baskets are damaged; there is grease on sides of the fryer; spray hose connected to water line under two-compartment sink doesn’t have a backflow prevention device installed; tiles are damaged in the walk-in freezer; wall is damaged at mop sink; several lights are out in hood above cook line; there is grease on the floor next to the fryer; floor drain in dish area is soiled; wet mop isn’t hung to air dry.
Moody Middle School, 7800 Woodman Road – The following violations were reported during a routine inspection: observed three dead American roaches in the dishwashing area; observed three small insects in two additional control devices.

Target, 9001 Staples Mill Road – The following violations were reported during a routine inspection: observed grease and dust buildup on wire racks and rolling racks in kitchen prep area near the oven; there is heavy dust buildup on hood system above pizza oven.
Wingstop, 9048 Staples Mill Road – The following violations were reported during a routine inspection: no certified food protection manager is present; person in charge couldn’t verify if a certified food protection manager is on the staff; heavy buildup of food debris and grime found on potato slicer; observed torn gaskets, standing water and a buildup of black mold on make table gaskets; green cutting board in dry storage is heavily scratched and stained; observed grease and dust buildup on green wire storage racks above three-compartment sink/prep sink and touch panel above fryers; there is a large amount of food debris present at floor drains of sinks; observed heavy grease buildup underneath three-compartment sink, on wire storage racks, behind equipment, and alongside coving of walls.
McDonald’s, 7118 West Broad Street – The following violations were reported during a routine inspection: an employee changed gloves but didn’t wash hands; ice bin has black buildup on it; temperature of sausage burritos in reach-in #5 is elevated because of improper cooling methods; air temperature of this unit is elevated; quat sanitizer concentration in buckets with wiping cloths is too low; fryer baskets have bent wires; several mops are not hung to air dry.
Pemberton Elementary School, 1400 Pemberton Road – The following violation was reported during a routine inspection: plastic utensil storage container is broken.
Skipwith Elementary School, 2401 Skipwith Road – The following violations were reported during a routine inspection: temperature of prepackaged meals with yogurt cups in milk storage reach-in unit is elevated; dead insects found underneath ice machine; observed two disturbed and full bait boxes behind three-compartment sink and reach-in unit; single-service knives, forks and spoons placed in trays with food contact surfaces exposed.
